SUBJECTIVE Mr. Marchand is patient followed for hypertension, and gout, and hypothyroidism. He has generally been under better control with his hypertension, but he has had persistent foot pain, worse on the right than the left that appears likely to be gout involvement of his second MTP on the right. The pain that he has in his left foot is a little proximal to the MTP joint, but in the same area and it may be in the transverse fascial band on the forefoot. His repeat uric acid was 5.9, which tells me he is in a range where this is very well could be a gout attack. His blood pressure is under good control and I hesitate to change his hydrochlorothiazide, but he does need better control of his uric acid and I think we will have to increase
A progress note from Dr. Chweyah, dated 06/26/2017, indicated that the claimant presented for follow-up of gout, muscle weakness, and weight loss. He had ongoing weakness of the legs with knee pain. He stated that her weight drop had stopped. His current weight was 210 pounds. Objective findings showed a pulse rate of 105 with a blood pressure of 92/60. His BMI was 28.56. He was diagnosed with an acute pain in both knees, muscle weakness in the legs, chronic gout, and weight loss. A referral to neurology and medications were recommended.

Mr. Markham is seen this afternoon at MCCRC on 03/02/2018. Mr. Markham is seen this afternoon after dialysis, which has exhausted him and he says his blood pressure dropped and he felt terrible as if he might pass out. He does not know why they were so aggressive. Other times, he has come back from dialysis with diastolics in the range of 110. His discharge is tentative for next Wednesday. He has had no chest pain. He has had some pain in his left knee but it has neither been warm, hot, or with atypical prednisone effusion that he has with gouty attacks. Mr. Jones has started him on allopurinol 100 mg daily.

The buildup of uric acid can lead up to: sharp uric acid crystal deposits in the joints. A sign of gout that is most common in people is getting a nighttime attack of. The symptoms that it can cause swelling, tenderness, redness, and a sharp pain. People can also get gout in their big toe, foot, ankles, knees and other joints. The person that has gout can last a few days with pain or even a few weeks before their pain even goes away.
Archibald Edward Garrod was welcomed in the city of London on November 25, 1857 to Alfred Baring Garrod and Elizabeth Ann Colchester. Archibald’s father was a prominent physician.
