Ms. Drake has been diagnosed with morbid obesity and has difficulty caring for herself. The victim could do more but chooses not to and doesnt suffer from any mental imcapacities. On 10/07/15 the victim was discharged from Winston Medical Center and became angry. Ms. Winston wants to remain admitted to receive care from the hospital but refuses recommendations that have been made for nursing home care. The victim has wheelchair and wheel chair ramp accessibility at her home and her daughter also assists in making sure her basic needs are met. Ms. Drake is non compliant with the care she needs at this time. It is the hospital's professional opinion that the victim does not live alone and chose nursing home placement to help with her ADLs.

The reporting party (RP) stated 55 year old Bernard Niemczyk DOB: 11/3/60 was found in his wheelchair in the middle of the street without any pants. Bernard was admitted to the home at the address listed as an independent resident on 12/19/15 and was found in the road on 12/20/15. Bernard was taken to Desert Regional Medical Center on a 5150 hold. When Bernard was admitted to the hospital he required total care. He was unable to walk or perform any of his activities of daily living. The RP stated Bernard should not have been accepted into an independent living facility. Prior to Bernard placement in the home he had been a patient at Riverside Hospital from 10/15/15 to 12/19/15.
In early February of 2017, my eighty six year old grandmother fell and broke her arm in two places. Since then, she has been in a rehabilitation center called the Notting Hill of West Bloomfield. My family wanted the best care and recovery for my grandmother. After extensive research, my family found that Notting Hill was rated very highly for its physical and occupational therapy, and it did. REPORTER: The reporter/niece (Deshannon) called with concerns for the victims, Louise and Lorinn. Louise can not walk or dress herself (needs assistance with ADL’s); the reporter said she is very confused. Lorinn is disabled (needs assistance with ADL’s). The patient is an 89-year-old gentleman who is brought to the ED via fire department and with complaints of altered mental status. The patient's primary caregiver is his wife at home. This morning when the wife called for the patient he did not respond so she called the police. The police found the patient in bed, awakened him and he was alert and oriented therefore Adult Protective Services was called to evaluate the situation. APS arrived later in the morning, and found the patient babbling and not making any sense so they then called and sent the patient to the emergency room. The patient has a history of congestive failure, hypertension and he also has a colostomy. There is no clinical documentation as to why he has a colostomy.
The reporting party (RP) stated resident Juanita Knee was placed in the facility by Dignity Healthy on 11/23/16. While present in the facility the resident was hit by a caregiver during the night (name of caregiver was not provided). Subsequently the resident is currently terrified. Additionally the resident has not been given her medications and her oxygen has not been set up. According to the RP the resident has not been able to make her own medical decisions. On this date worker received a call from Ms. Latasha Toney, SW at Princeton Hospital. She stated Mr. James Uptain was admitted to the hospital following behavior issues at Ridgeview NH. The nursing home is not willing to readmit Mr. Uptain and finding NH placement has been difficult due to his behavior issues. According to Ms. Toney Mr. Uptain is verbally aggressive, encourages other patients to misbehave, smokes cigarettes in nonsmoking area, rude to other residences, and waits on female patients outside of bathroom.

The RP stated Julie alleged a staff member named "Mimi" pulled her hair and broke her neck area during the months of July and August. The Julie was asked to provide dated she stated she could not remember. The Day Care Program did not observe any injury to Julie's neck area. Most elderly peoples biggest fear is giving up there home and independence. If a person is mentally competent, no one can force them into a nursing home. As long as a person has the mental capacity, not even a Power of Attorney can force them into a situation they do not agree with. Mrs. M’s new Physician has the opinion that she needs to be in a long-term care facility because due to her age her health will continue to decline. Linda is very knowledgeable of what it means to have good customer service skills, and have customer satisfaction. Linda is always respectful, honest, and considerate of others. She communicates well through oral, written, and emails. In case #16-262-00006 where the autistic 19 year old was handcuffed, the RA’s father sent Linda a thank you for her expertise and kindness in talking with the police. He also thanked Linda for understanding his anger, but still being an advocate for the RA. In case #16-189-00141 the report alleged that the RA’s husband/AP had removed the RA from the nursing home, and he would not allow family members to visit the RA in her home.
