This week in clinical, I met most of my short-term goals from last week. I became more comfortable on the unit and was able to be more familiar with the environment. I heard morning report and change of shift report, I participated more in medication administration, and felt more comfortable with myself and my role as a student nurse. I had many opportunities to talk to patients and was able to talk to everyone on the psychiatric side and engaged them in conversation. This week I met my goal of becoming more aware of myself and was able to reflect on my communication when talking with patients. Another short-term goal I want to add is giving PPD tests and flu shots to patients who do not have them. This past week, I was able to watch one being administered, and hope that I can give a few next clinical. Along with hearing report and giving medications, I was able to be more active in general. I talked to more people, saw the admission process, and went outside with the patients. All of the patients on the unit have their own unique life stories and illnesses. Getting to communicate with and get to know the patients is enlightening. I decided to go onto the psychiatric side of the unit this week and immediately connected with the patients. By the end of the shift, I genuinely feel like I made connections with almost everyone on the unit. I was able to talk to the patients and look deeper into their problems. One of the main goals that I worked on throughout multiple shifts was my time management skills, regarding charting and patient notes. I first set a goal of having my charting done
Overall, I do not feel like a great deal of improvements needed to be made in my delivery of care. I felt comfortable communicating with my nurse and the health care team, assessing the patient, and educating family members. My nurse did not feel well, so she started off the day a little on edge and irritated, but through showing that I was willing to assist her in any way possible, she was more than willing to assist in my learning. I could improve my delivery of care by asking my nurse more questions and looking up more information in the patient’s chart. I noticed that the layout of the NICU was quite different than last week, so I did not have a computer to look up patient information, but rather, I looked alongside my nurse.
My learning needs were supported this week by the nursing staff on the unit allowing me to help with assessing and treating their assigned patients .In addition, my clinical instructor discussing ways to improve out charting.
During clinical this week we were shadowing a nurse on our clinical unit. I had a couple personal goals at the start of the day. First off I wanted to get comfortable with the unit. I wanted to learn where everything was located in case my patients, throughout the rest of the quarter, asked for something. I also wanted to learn how my nurse prioritized patients. While I am aware that I will only be taking care of one patient on any given day this quarter, I know that learning this skill will be useful in my nursing career.
One thing I noticed today was that they really are stuck in times when they were younger. There was this on lady that kept telling us that everybody was sick and that she was going later. When she was talking she would talk as two different people. A few of the residents told me that they appreciate us visiting them that it makes there day especially the ones that don't have family that comes and sees them.
At this point I know that I am confident, I can critically think and independently plan, organize and provide quality care to my patient. I have made a tremendous improvement on medication knowledge, application and administration and these competencies will be demonstrated in my clinical rotation at Holy redeemer. My second goal specifically is to get a position in the human resources department with my current employer within the next six months. Gaining a position in the human resources department puts me one step closer to achieving my long-term goal of becoming a human resource manager. The goal is measurable because I plan to apply to any position I see posted in the human resources department that interest me and align with my professional goals. The goal is attainable because I have access to our job postings website and I plan to check this website for updates at least twice per week. This is a reasonable goal because I sometimes have downtime at work, which would allow me to use that time to search for new postings and even apply. This goal is time-bound because I gave myself a deadline of six months to achieve it. With my updated resume, experience and now my current course of study at Western Governors University, it makes me a desirable candidate for a position in HR.
I met a few objectives this week such as being able to communicate well with my staff and peers and demonstrating professional behaviors. An example of how I demonstrated objective 2, was in the practice lab, I efficiently explained to my peers which Personal Protective Equipment is needed during a current precaution such as droplet and airborne. I also demonstrated objective 2 when we had our one on one meeting and I told you about being nervous for clinical as I have had bad experience with dementia patients. I appreciate you reassuring me that I will not have a combative patient as I truly do want to have a positive clinical experience as well as a therapeutic relationship with my patient. I demonstrated
I was able to be more confident and assertive with all of the tasks that I performed in clinical this week, which is a huge improvement for me.
To begin, this week has definitly been a roller coaster ride. When I first arrived on campus, I was automatically geared to return to my old habits. However, I decided that would not be the best thing for me. As for this week I can say that I've accomplished many of my goals. This is the last day at CC. I had fun because we played bingo with the patient. It was fun to them as well. At some point it is hard to figure out who they are. Because they look normal, but underneath are a lot of psychological issues. Thus, I think working with psychiatric patients are not easy because they are able to manipulate and give the answer which the other want to hear. Their minds may have some issues but what I recognized through my clinical hours in CC is that they might deceive us. I think one of the most important skills for a psychiatric nurses need to be discreet to distinguish between the true and false story.
Throughout not just the past two weeks, but this placement as a whole, I have developed more confidence in my nursing skills. As a result, I now feel more capable in my abilities as a nursing student/ future nurse and am able to more effectively communicate with my patients.
The experience working with him and the other residents has solidified my passion for helping others overcome adversity and function at their highest potential. Throughout the months I gained tremendous amounts of patience. Just as importantly, I was able to apply a multitude of de-escalation techniques and behavior modification strategies so that residents could lead a more positive day-to-day life. I found clinical this week more challenging this week than normal. My patient has been in the hospital for a while, however, he was disoriented to his situation. He didn’t remember having an abdominal surgery and thought he was still in the hospital for his back surgery. He also had no energy from not eating properly and wanted to be left alone to sleep. Before the first day of clinical my goal was to focus on teaching. After interacting with him on the first day, my goal for the second day way to try to connect with him and encourage him to take more control of his care.
Throughout these weeks working on the mental health units I was able to go to many different units and ECT treatments. During the first couple of weeks I was having a hard time starting a conversation and feeling comfortable talking with the patients by the end of these weeks I have improved. I was able to speak to multiple patients and make them feel safe and comfortable. One week I was able to see what happens when a patient has an episode that endangers the other patients and then escalates to the nurses; where they couldn’t calm the patient down. They had called a psych emergency and about 15 men game to help turn this situation into a better one. This showed me how things can escalate but at the same time with help it can be detained.
