This week has been crazy busy. We have had all kinds of animals, with all kinds of issues. We've been so busy that our appointments have been back to back, with little time between. This kind of sucks for the patients since we only have two rooms, and usually one doctor on staff, but we are pretty efficient and everyone gets in and out in a timely manner. Of course with the priority being that everyone gets the treatment they need and their questions answered. This week had a lot of cool cases, one of which was my first real emergency case. There was once previous one, but it turned out to be less emergent and that's for another post. This was after we started to get some of the bleeding under control, but it ended up requiring a mini tourniquet of sorts to go around the mass because it just wouldn't stop bleeding. We were stuck between a rock and a hard place because Checkers' being such an old dog, had a much higher chance of dying during any surgery to remove this mass, which is why it wasn't removed before, but also the tourniquet wasn't a permanent fix, and if it started to bleed again checkers would most likely bleed to death. So what do you do? Well the owners elected to proceed with surgery, but with blood work first. The surgery was scheduled for the next day as long as the bloodwork came back fine. Well, nothing in life is ever one hundred percent planned, and sometimes life throws you, or in this case Checkers a curve ball. Her blood work came back as being anemic, and with signs of possible cushings. Not good. Surgery was postponed one week and we are hoping the new blood work in a week will be better and that the tourniquet made of sutures holds. On a happier not the Quaker Parrot I wrote about on my first blog post came back in today to have one of his sets of hobbles removed, except this time he didn't need a new pair. Week eight was the same as always. I began the day at eight in the morning to receive patients, print out superbills and doing the usual. I will soon be I find my clinic a bit repetitive, but I like it because I learn by repetition. As I was taking in patients a woman told me her son had the Flu. The dog is now restless and in agonizing pain and very irritable. The owners keep saying that he sounds like he's choking they wonder if he just ate the bone wrong. The white blood cells haven’t come back for me to see what I've been doing. The owners still can’t tell why the dog is choking and irritable running into doors not knowing why. They took him to an “emergency vet” but they didn’t test for me. They just said he had gotten into something and it “would be gone in a few days” Little did they know this dog would be ferocious and attacking innocent dogs.

The patient was then urgently transferred to the closest facility with vascular surgery ready to perform emergent surgery to repair her abdominal aortic aneurysm (AAA) dissection for which the patient and family were not aware that the patient

The build up of air, fluid, or food in the stomach may lead to a rotation of it around itself, causing a constriction. Reducing the blood flow to other regions of the body may result in cell death, and depending on how long the clinical signs persist for, certain tissues may become necrotic. Rapid diagnosis and treatment is essential for a good prognostic for the animal. Mortality rates in GDV cases are approximately 15% (Marsolais, 2017) and the most important cause of that is the delay in receiving veterinary help once the rotation has occurred. That mortality rate would increase even more as the critical signs persisted for more the 6 hours, if the dog showed cardiac arrhythmias prior to surgery, if removal of a portion of the stomach was necessary due to loss of blood supply, and requiring removal of the spleen. Therefore, even though GDV might seem deadly, it is important to remember that it can be easily converted as long as help is sought in the beginning stages of the condition. It was slow on Monday, Tuesday, and Wednesday. There was two surgeries on Monday. It was two kittens getting neutered. On Tuesday, it was regular appointments, such as nail trimming, or cleaning the animal’s ears. On Wednesday, there was an emergency case with a dog that had a heat stroke. He started seizures when he came in. However, the doctor noticed that it wasn’t a heat stroke. They did x-rays on him and there was fluid in his chest. Sadly, the next day he passed away.
