Good Afternoon Dr. Stein-Albert,
Just want to bring to your attention, some patient complaints that I’ve been getting about Dr. Beaulieu over the last two days.
• Patient Dayonah Bailey came in with her mother on Monday requesting to switch her provider, when I inquire the reason she said that Dr. Beaulieu felt asleep at her computer and never answer any of their questions.
• On Tuesday Kristina Leach parent requested to switch provider she was moved to Ms. Brooks, her compliant is that Dr. Beaulieu did not address any of her concerns or questions, all she did was typed away at her computer.
• This morning patient Arianny Martevicente accompanied her brother to his doctor’s appointment, at the time of registration requested to switch her provider, when I asked why she stated that “she want a doctor who is going to listen to her concerns and someone who is not going to give her anxiety.

Beaulieu three weeks ago, she stated this was her third time coming in to pick said form that is need for today in order for him to participate an away game at Cookman College today.
• Malik Reynolds (former patient of Dr. Balaoura) came in today to see Dr. Hardoon and urgent care for Ms. Brooks, she stated that she wanted to switch her children to Ms. Brooks when I asked who was her provider and why she wanted to change she stated she use to see Dr. Balaoura and was very happy, but was transition to the “Haitian doctor”, said she saw her last year and requested prescription to be sent to the pharmacy and it wasn’t done. Several phone calls was made to the provider with no result. never came back for her follow-up
Patients who request for their own doctors should have their wishes respected as a matter of a patient's rights. At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.
Service Coordinator (SC), Jennifer Stoker met personal staff, Aiesha Crayton at the home of consumer Jonathan .SC asked was Jonathan meet his outcome would like is medical expenses to cover. Aiesha noted he has Medicare and Medicaid which cover all his medical expenses. SC asked if money covering his want and needs. Aiesha noted yes. He wants and needs are being meet. Jonathan wants his cell to be paid every month. Aiesha noted he cell is being paid every month. SC asked is Jonathan maintain good health. Aiesha noted he when to his PCP on June the 1st. She noted he is health and there was no change in medication.
On 11/17/2017 I received a phone message from Dr. Mendelson’s office as a reminder that Mrs. Raney is scheduled for an appointment on 11/21/17 at 2:50p. I recontacted this office as I had her scheduled for an earlier time. The staff rechecked the physician’s schedule for that day and I was told that she could be seen at 2p. I notified Alimar Transportation of the new time and the nursing facility Cherrywood.

SC, Jennifer Stoker contacted provider, Laverne Nyaido via telephone. SC introduced herself and told provider that she was Audrey’s SC. SC informed Laverne SC is outside her the address 947 Angie LN. In Desoto, Texas and no is answering the door. Laverne informed the SC Audrey has moved to a new address. She informed the SC the new address is 900 North Young BLVD. Desoto, Texas. SC asked about Audrey doctors’ appointments and behaviors. Laverne noted she is out of town and don’t her doctor’s appointments available at this time but will call SC once she back in town. She did noted Audrey has not have any behaviors issues.
Writer called Constance to find out if she has heard from the Visiting Nurse Association (VNA). Constance said yes and that they scheduled to meet on the 9th of this month. Constance said she will be seeing her pain managing doctor on the 8th. Also Constance said Medicaid approved for her to use long term care. Care manager was glad to hear that. SC completed monitoring phone call with Pa on 10/30/2015. Pa reported that she is doing well. PA reported that she is receiving services as outlined in the ISP. She remains satisfied with services and feels that they meet her current needs. No fall, hospitalizations, change in health status or medications reported by Pa. Pa reports that she is doing physical therapy evaluation and therapy at Nova Care at 2301 Broad St for: knee, neck, upper thigh, lower back joint pain, muscle weakness, and problems with balance and gait. Pa has follow up appointment with Rheumatologist and Optometrist for evaluation and treatment of vision problems. Pa last visit with PCP was on 10/29/2015. We have investigated your concerns regarding this issue. Our office contacted the office of Sunrise Medical Clinic and spoke with Maggie the Office Manager. Maggie advised us that she was unaware of any issues. Maggie stated that you normally are seen by Dr. Meta Girish however he was on vacation so Dr. Yakoob and his nurse filled in for your appointment. Maggie stated you will no longer be scheduled with Dr. Yakoob or his nurse. Maggie confirmed she will address the issues you experienced with the providers nurse.

My wife went to an appointment to re-do her PAP due to a lab error at the Robison Clinic on 07/17/2015 at 1245hrs. Due to traffic construction in Fountain she showed up at 1250hrs. The lady at the front desk was helping my wife during check in. LPN Dennis O'Connor came out of the office and told my wife that she was late with a bad attitude. My wife requested to talk to one of the Spanish speaking nurses that helped her during her last visit that work at the clinic because her English is not good. LPN O'Connor replied that there was nobody that works in that clinic that speaks Spanish and again with a bad attitude. I know Evans has a translation phone system available for patients. Why the nurse did not call that system to communicate with my wife to explain to him the reason she showed up late?
