Given the prevalence of back injuries within all sectors of the community, the potential roles / opportunities for physical therapists to improve community health is nearly limitlessness. Elementary teachers are at a particularly high risk of a lumbar injury because their work environment requires considerable lumbar flexion as well as prolonged periods of sitting to grade papers or develop lesson plans.26-28 After discussing her work environment with the patient, an educational opportunity was recognized to potentially help teachers prevent back injuries. This approach initially uses patient generated forces in the form of active range of motion (AROM), usually to the end of the available range. If the patient generated forces are successful to improve the patient’s presentation, then there is no need to employ manual therapy techniques. If the patient generated forces are unable to fully resolve the patient’s symptoms, then manual therapy techniques are utilized; however, the initial manual therapy techniques are applied with the least amount of force possible in an attempt to resolve the patient’s symptoms, and are only progressed to a more forceful technique if the patient’s symptoms failed to resolve with the initial technique. In other words, manual therapy is only used when the patient is unable to resolve their symptoms through end range AROM.23(p446) The MDT approach offers an additional safeguard. In this case, although the error started from the transmission of the discharge note to the home health care agency, Physical Therapist and Physical Therapist Assistant made number of mistakes in terms of clinical practice and delegation. First of all, completion and accuracy of patient’s physical therapy record is a professional responsibility of a supervising Physical therapist.1(p4) However, the PT failed to read the hospital discharge referral and documented incomplete and inaccurate plan of care. As a result, important information such as un-cemented prosthesis in situ and weight bearing limitations were not documented. In addition to that PTA elected to follow a protocol for cemented hip arthroplasty without referring back to PT for the missing information in the assessment. According to Texas Board of Physical Therapy examiners rule, “PTA may not specify and/or perform definitive (decisive, conclusive, final) evaluative and assessment procedures.”1(p5) Hence, it was also a mistake of the PTA to conclude the assessment and elect the inappropriate plan of care.

The patient was compliant to all aspects of treatment and the home exercise program. There are no known alternate explanations of the outcomes of this case report. However, in comparison to the case report by Caldwell et al25., the patient in this case report displayed a faster decrease in pain and return to normal function indicated by 0/10 VAS, 0% neck disability and ability to perform all tasks for work at the last day of treatment, 3 weeks from the first day of physical therapy. The Back & Neck Care Center’s hierarchical structure is comprised of eight individuals (Appendix B). At the top are Drs. Gregory Tarnow and Carol Baric-Tarnow. Dr. Tarnow is the primary provider of chiropractic diagnosis and treatment. He also executes educational community outreach programs to attract new patients. The business manager, Dr. Carol Baric-Tarnow, oversees patient records and billing, banking, and all of the Human Resources functions. Human Resources Functions include recruitment, training and development, compensation and benefits, and employee relations. Next in the hierarchical structure, Associate Dr. Gregg Brady, is the secondary provider of chiropractic diagnosis and treatment. He was recently hired to accommodate the increasing number of patients and to attract new clientele. Next, and supervised by Drs. Tarnow and Baric-Tarnow, is three office staff members. Their principle responsibilities include scheduling, processing patient records and bills, performing rehabilitation, and selling products. Finally, Drs. The results of this study show that both treatment interventions are just as effective in pain reduction and increased spinal mobility following a single treatment. A post boc correlation (relationship between variables) analysis was performed in order to explore the relationship between changes in pain and lumbar extension motion. Interpretation of Results: Data from the study suggests that ambulation with the PBWS is a valid means for pain relief and functional improvement. Mark is a six-year-old boy, the youngest of four siblings. The oldest, Caroline, was my closest friend so when she asked me to watch over her four siblings while her and her parents attend a church function, I openly accepted. I arrived to their home at 6 pm exactly, just as they requested, and entered their home. Sitting there were three children on the living room floor. There was Helena, Daniel, and Mark all sitting and trying not to kill each other over whose turn it was to watch TV. Mark stood out to me the most, he was different. He was born with a disorder called Down Syndrome. This condition occurs when an individual is born with either a full or partial copy of chromosome 21. I lead to learn about physical therapy through Mark and how beneficial it can really be. When he was born, their doctors advised that Mark will be needing different variations of therapy in order for him to learn to do things independently. Physical Therapy (PT) taught Mark
