R/s Bradley was threatening workers that were working on the home. R/s Susan is staying her a hotel. R/s Susan had five day eviction for Bradley, he got extension to hold things up. ALLEGATIONS: R/s Susan Byrd has Schizophrenia and high blood pressure and she complains of stomach and chest pains as well has back problems. R/s over the summer while Md. Byrd was in treatment her son, Bradley moved into house. R.s Bradley is volatile and he threatened to burn down the house with Ms. Byrd inside. R/s Bradley and his girlfriend, Lisa are smoking ice in the home and they fight each other.

She was taken to Deacon’s Hospital Intensive Care Unit to pump out the stomach contents and also to neutralise the effect of the overdose. Karen had declared that she never wanted to go to the hospital should her condition gets worse. When Karen was now aware where she was, she complained to the author who apologised and said he would not let her pass on when he can do something about it. On this date worker visited Shadescrest NH, for the purpose of monitoring Ms. Salter's current placement and delivering Christmas presents. When worker arrived, Ms. Salter was lying in bed watching TV. Ms. Salter was happy to see worker and excited about her Christmas presents. She informed worker that her sister was checking on having her transferred to the Cordova NH because they did not like the Administer, Chester at Shadescrest. Ms. Salter stated she had begun to experience small seizure due to the stress of Kathy Harville, her ex-significant other and Chester. Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.

Through his referral we have discovered that Ernest has many mood changes and anger issues that are unresolved resulting in resentment from his father’s suicide and his mother dressing him as a girl. William reported Mr. Saunders told him he wanted to kill himself. He reports for the past 3 week he has notice Mr. Saunders has been coming to work under the influence and more depressed. The mother-n-law reports Mr. Saunders has been staying with her and he has informed her as well of putting a gun into his mouth and thoughts of hurting others. Mr. Saunders was confronted about these claims and states he can not recall states due to being under the influence. Introduction: Anna Garcia, age 38, was found dead in her home at 9:56 a.m., the scene was processed at 10:20 a.m. She was found near a knocked over table facedown with blood coming from somewhere on her body, most likely her head area. The EMT pronounced her dead and a investigation began on whether or not this was a crime. She recently divorced her now remarried ex-husband whom does not get along well with. The ex-husband’s wife has not been known for being nice or fond of Anna Garcia, making her a suspect.
R/s in October 2014, Toni Smith filed a report that her son Taylor Murphy (16) was in relationship with their 27 year old neighbor Daisey Burrell. R/s the investigation was closed with no findings due nondisclosure from Taylor. R/s Taylor was at William S Hall for in-patient treatment before receiving services from SADAC. R/s Taylor was using meth and he is now receiving drug &alcohol counseling and his drug screenings had been negative. R/s Taylor is also receiving psychotherapy education. The reporter stated on 07/05/15 Ms. Smith returned to pick the children up and after they began screaming and hollering because they did not want to leave with her Mr. Course told her they could stay with him. Per the reporter, Ms. Smith became upset and pulled a knife on Mr. Course; the children were present.
