On November 20, 2015 worker visited Selby Ridge Rehab and NH, for the purpose of monitoring Mr. Parker’s situation. Mr. Parker is still in the physical therapy section of the NH but will be moved to a long-term bed soon. When worker arrived, Mr. Parker was lying in bed watching TV. He was dressed appropriately with good personal hygiene. Worker introduced herself and explained the reason for the visit. Mr. Parker remembered worker from the previous visit but could not remember worker’s name.
Worker gave Mr. Parker his belongings from his home and asked if he had any friends or family members willing to help move his belongings out of the home. Mr. Parker stated there was no one and agreed to allow Hana Home have the belongings.

On this 9/3/15 worker visited the residence of Mr. Alvin Higgins, for the purpose of making first victim contact. Mr. Greg Nunn, RN with New Beacon Hospice greeted worker outside of the residence. Mr. Nunn stated he called the residence about 3 1/2 hours before arriving and Ms. Betty Griffith was not home. However, her daughter Gina Craven and her boyfriend, Kenneth Woods was at the home. When he arrived, the door was unlocked and Mr. Higgins was lying in his bed located in the living room. After being there for several minutes, Mr. Woods came into the living room and asked if he needed anything. Mr. Nunn told him no and Mr. Woods walked toward the back of the house. Medically Mr. Higgins diagnoses are late stage Alzheimer's. He is bedbound, hard of hearing, and unable to communicate.
Once Dy. Cavalier returned to work on Saturday May 14, 2016, he was again assigned to M&N control. Half way through his shift he was advised by Cpl. LaMotte that he needed to contact medical and asked them if they could come and check on Ducre because he was acting strange. Dy. Cavalier contacted medical and spoke with nurse Sterling and she advised him that she was eating and that she would let the others know and someone would come. For the remainder of the night Dy. Benson made extra rounds to check on inmate Ducre. Upon the time that Dy. Cavalier’s shift ended no one from medical had seen Ducre.
Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.

On this date worker visited Shadescrest NH, for the purpose of case monitoring. When worker arrived Mr. Hopson was lying in bed sleeping. He was appropriately dressed with good personal hygiene. During visit worker was unable to wake Mr. Hopson. Worker spoke with charge nurse and learned Mr. Hopson was doing well. Mr. Hopson became long-term Medicaid approved in Sept. beginning in August.
On this date worker visited Walker Rehabilitation in Carbon Hill Al., for the purpose of monitoring Ms. Phillips placement. When worker arrived Ms. Phillips was in bed sleeping but woke when her name was called out. Ms. Phillips was dressed appropriately with good personal hygiene. Ms. Phillips did not remember worker from previous visits but was friendly with worker. During visit, Ms. Phillips talked about her roommates and appears to have adjusted well to her new living arrangement. This was a concern for Ms. Phillips because she went from a room for two into a room of four. Ms. Phillips stated she enjoys the food but doesn't participate in activities. Ms. Phillips stated the only time she leaves her room is to eat and get a bath. Worker

When worker arrived, Ms. Salter was lying in bed watching TV. Ms. Salter was happy to see worker and excited about her Christmas presents. She informed worker that her sister was checking on having her transferred to the Cordova NH because they did not like the Administer, Chester at Shadescrest. Ms. Salter stated she had begun to experience small seizure due to the stress of Kathy Harville, her ex-significant other and Chester. After enrollment into the program was complete, HARRIS nor MCGHEE presented HILL with any questions of how the program worked, timesheet approvals or submissions. HILL monitored any changes in DANIEL’s need for services through face to face monthly visits which lasted about 40 minutes. Changes in the need for services may be due to an admission to the hospital, a long-term nursing facility or relocation to another state.
