One of the recommendations for healthcare organizations to employ in an effort to reduce the number of errors is to advocate for voluntary error reporting nationally while conducting research and developing tools for patient safety. This way, information about errors can be gathered and prevented from reoccurring at health care sites and by health care providers. Voluntary error reporting will act as a warning of potential or actual errors and suggest ways to avoid them in the future.

It is shocking to know that every year 98000 patients die from medical errors that can be prevented(Kohn, L. T., Corrigan, J. M., & Donaldson, M. S. (Eds.), 2000). Medical errors are not a new issue in our healthcare system; these have been around for a long time. Hospitals have been trying to improve quality care and patients safety by implementing different strategies to prevent and reduce medical errors for past thirty years. Medical errors are the third leading cause of death after heart disease and cancer in America (Allen, 2013). In addition medical errors are costing our healthcare system an estimated $735 billion to $980 billion (Andel, Davidow, Hollander, & Moreno, 2012).

The beginning of a new graduate nurses career rarely begins easily, there seems to be a distinct disconnect between the fantasy of what it means to be a nurse and the reality of bedside nursing. Nursing school seems to feed into this disconnection, in that it does not prepare the new graduate registered nurse (NGRN) for their professional practice. What nursing school does is give the NGRN the basics, a peak into what is to come. The NGRN has to find out for themselves what the true meaning of being a nurse is and if it fits their preconceived ideas.

Nurses must achieve a higher level of education in order to meet the demands of evolving health care and the changing needs of patients (Rosseter, 2012). Nurses will need to be prepared for a broader scope of practice and to maintain quality within the health care setting. Associate degrees are still valuable tools when used as stepping stones to baccalaureate programs and performing the technical aspects of nursing. BSN-educated nurses are equipped with a more diverse skill set that assists them in leading and reacting appropriately in critical situations. In addition, they have the opportunity to pursue advanced practices. Baccalaureate programs can help develop a good or adequate associate degree nurse, into an outstanding nurse.

Causes of major medical errors have many different factors and influences. This includes why the patient was being seen to allow such an error, what medical guideline or guideline’s that where not followed that caused the error, what could have been done by staff members to prevent the error, etc. When errors take place, repercussions follow such as the cost incurred to the patient or patient family members, fines the medical worker must pay, and most importantly what is the patients status/prognosis. Not all patients prevail and make it through such awful medical errors.

Nursing research can be defined as the knowledge that is developed and built on the foundation of scientific inquiries of clinical practices (Grove and Burns, 2013). As part of the United States National Institute of Health (NIH), the National Institute of Nursing Research (NINR) was established to promote wellness and to prevent diseases, to improve the overall quality of life across populations, to eliminate disparities, and finally to set the directions for end-of-life, palliative research. This essay will explore the history of the NINR in terms of its location, dates of operation, overall purpose, mission, and Strategic Goals. The essay will then outline the NINR 's current initiatives, describe the organization 's priorities in terms of funding. Training opportunities available through the organization will also be described. Finally, the experience attained by group members through doing the assignment will be explored and summarized.

Such degrees inherently attract older students. While there are many studies regarding mature student educational satisfaction and retention, few publish indicators of later professional success. If the goal is to decrease the nursing shortage, I suggest that proving mature students are likely to endure at the bedside is more important than proving they succeed in school.

The need for expansion in their roles for higher levels of education and an improved system of education are required for nurses now for meeting the diverse needs of the patients and to provide better care. Improving the education system and achieving a more educated work force is increasing with nurses having baccalaureate degrees , that is, a traditional Registered Nurse to Bachelor of Nursing program or a traditional 4 year Bachelor of Nursing program or a program that allows seamless transition from Associate in Diploma Nursing to Bachelor of Nursing.

Errors occur in health care as well as every other very complex system that involves human beings. The message in “To Err is Human”, by Archie Cochrane, was that preventing death and injury from medical errors requires dramatic, system wide changes. Health care professionals have customarily viewed errors as a sign of an individual’s incompetence or recklessness. As a result, rather than learning from such events and using information to improve safety and prevent new events, health care professionals have had difficulty admitting or even discussing adverse events often because they fear professional censure, administrative blame, lawsuits, or personal feelings of shame.

Medical errors are avoidable mistakes in the health care. These errors can take place in any type of health care institution. Medical errors can happen during medical tests and diagnosis, administration of medications, during surgery, and even lab reports, such as the mixing of two patients’ blood samples. These errors are usually caused by the lack of communication between doctors, nurses and other staff. A medical error could cause a severe consequence to the patient in cases consisting of severe injuries or cause/effect any health conditions, and even death. According to recent studies medical errors are not the third leading cause of death in the United States. (Walerius. 2016)

Errors pervade in our lives whether it is our home, in our workplace, or in our society. The effects of healthcare errors have impacted all our lives either directly or indirectly. The healthcare system is expanding at a rapid pace and it is vital for all healthcare professionals, including nurses, to keep up with an already complex system that takes care of a multitude of diagnoses, diseases, and diverse patients. One way to better address the needs of the increasingly multifaceted healthcare system is to increase the higher educational level of nurses including growing the number of nurses with a baccalaureate degree to 80 percent of all nurses. As of now, there are still many associate degree and diploma degree nursing schools around and many prospective nursing students choose to take this route because of the cheaper tuition and shorter time. The knowledge needed to practice nursing has grown to include health policy, ethics, public health, research, healthcare delivery improvements, as well as leadership skills. Nurses are part of the multi-disciplinary team and must work together with healthcare providers who possess masters or doctoral degrees. Nursing school prepares nurses with the basic information they need to know in order to practice safe nursing. However, modern nurses are faced with ever increasing demands. In today's modern world with plenty of technology, it is hard to believe that we cannot figure out how to reduce Medical errors. The issue of medical error is not new in health care organizations.
