On 11/08/2015, at approximately 0201 hours, your Affiant and assisting Officers were on State Route 924 near the Girard Park investigating a vehicle accident. Officers had marked police vehicles with the emergency lights activated which were stationary on both sides of the roadway. During that time Officers observed David J. Yashinsky Jr. operating a green Subaru sedan southbound on State Route 924. The vehicle displayed a Pennsylvania registration of HXB6301. Shortly later Officers observed David J. Yashinsky Jr. driving the same vehicle as previously described northbound on State Route 924. Officers are familiar with David J. Yashinsky Jr. and had prior knowledge that his driver's license was suspended. Officer Moyer was forced to place handcuffs on Yashinsky's wrist in order to detain him. Your affiant approached the vehicle and requested the front seat passenger John M. Najunas Jr., to exit the vehicle. Najunas agreed to exit the vehicle and while doing so an open Milwaukee Best Light beer beverage was observed on the floor of the vehicle between his feet along with two six packs of Bud Light glass bottles. David J. Yashinsky Jr. had an odor of an alcoholic beverage emitting from his breath while he spoke. Before any requests were made Yashinsky said "I know I shouldn't be driving, but could you not tow the car." Your affiant requested David J. Yashinsky Jr. to submit to a series of psychophysical divided attention tests; none of which he was able to successfully complete. On Monday, May 23, 2016, at approximately 12:45 a.m., I, Trooper Bradley Brachear, received a telephone call from Sergeant Robert Ventura. Sgt. Ventura advised Trooper Bradley Otten of Illinois State Police District 13 requested my assistance with a two-vehicle crash on Illinois Route 4 approximately 0.23 miles south of Norton Lane in Tilden Township, Randolph County. I responded from the Lebanon area to assist. The crash resulted in one fatality.
I positioned the patrol vehicle behind the Ford ranger and conducted a traffic stop, using the overhead light bar and siren. The Ford ranger pulled over on the south curb of 17th St. at the intersection with Capp St.
One of the vehicles involved pulled away from the accident and headed north on Sprinkle Road. Two witnesses called this in and gave a description of the vehicle. From this description the vehicle was pulled over on the corner of East Main Street and and 26th street at 14:07.
According to police reports, officers were dispatched to a hit and run call in the 700 block of Vassar. A vehicle has struck two parked vehicle and had left the scene. Officer Seachris arrived and made contact with Officer Wheeler who was with the victims of the accident. Officer Wheeler advised that the vehicle was in the 600 block of Vassar. Officer Seachris proceeded to locate the vehicle, a maroon Chevy pickup with Kansas tag of 005HEY parked in front of 646 S Vassar. The vehicle was registered to a Hispanic female at 628 S Vassar. On 07/15/2015, at approximately 2305 hours, your Affiant was on routine patrol on Main Street (SR924) when I observed a silver Chrysler sedan parked in the One Hundred Block of South Main Street (westside of the street) near the intersection of Oak Street. The vehicle displayed a Pennsylvania registration of JRJ-5342. The registration was checked utilizing the Pennsylvania Department of Transportation vehicle record abstract which revealed that the license plate was assigned by the department to KYLE ROBERT & SARAH LYNN FOGARTY for a 1998 Chevrolet. I then verified the status of KYLE and SARAH FOGARTY'S driving licenses both of which were suspended by the Department of Transportation.

RA Zeigler decided to contact ADRL Jean Barnard once again to clarify on what we should do. ADRL Barnard said not to search the room for alcohol, only document what they see. RA Shaw, RA Zeigler, and RA Bokar found a case of 24 12 oz. bottles of Miller Lite, with five in the case and two in the trash. On the floor and in the trash we found 11 cans of Natural Lite beer. OPD Officer Frazee supervised the RA’s while they were taking away the alcohol. However, OPD Office Frazee handled the alcohol and disposed of it after the RA’s collected it. RA Bokar called up Hanby Resident James “Jimmy” Carpenter, who was in Clements Lounge all night with RA Bokar watching the game, to explain what was happening in the room. This officer approached the Newland who was sitting in the passenger seat. This officer detected a strong odor of an intoxicating beverage, about her breath, eyes were bloodshot and watery and speech was slurred and mumbled. Newland was unable to stand without assistance. “Officer Groome attempted to stop a 2003 Honda Civic on Oliver Avenue in the city of Chesapeake for stolen license plates. Officer Groome activated his lights and siren for the driver to pull over. The driver of the vehicle did not stop, ran multiple stop signs and red lights, and was driving in excess of 80 mph. The driver continued driving through South Norfolk area into the city of Norfolk. The vehicle came to a stop on Reservoir Avenue in the city of Norfolk. The driver was identified as the accused and the passenger got out of the vehicle and ran. The Norfolk police responded to the area with their K9 unit, and the accused was hiding underneath the crawl space of a house. The accused was found in possession of a dime bag of marijuana and the vehicle, property belonging to Paul Brady, which had been reported stolen on 09/26/2015, in the city of Chesapeake. The accused is not a licensed driver. The accused stated that he received this vehicle from a “friend.”
In summary, on 03/29/16 at 1855 hours I was patrolling the area of 1200 S 54th Ave., at which time I observed a vehicle (red 07' Lexus IL Z161188) traveling northbound in the wrong lane to pass a vehicle at a high rate of speed. A traffic stop was conducted on the vehicle occupied with three (3) male subjects.
On 10/28/2015, at approximately 0232 hours, Captain O'Neill and your Affiant observed a red Chrysler PT Cruiser (PA Registration JWA5451) traveling south on State Route 924. The operator of the vehicle was observed and identified as Dawn HERRING. HERRING'S driver's license was expired and suspended for driving under the influence; the information was verified through a certified Pennsylvania Department of Transportation record. HERRING was observed traveling under the posted speed limit and repeatedly swayed her vehicle throughout the lane. Your Affiants activated the emergency lights on the marked patrol vehicle and stopped HERRING at the entrance of the Transfer Station.
I made contact with Powell, Jermaine L. DOB 02/02/77, who related he had a verbal argument with his wife (Powell, Denise DOB 02/24/81 over the discipline of their child (Powell, Nathan L. DOB 09/18/12). Jermaine related that Denise has bi-polar disorder and takes medication. Jermaine further related she has mood swings causing them to fight. Jermaine related he and Denise were separated and recently reconciled one (1) month ago. While I having a conversation with Jermaine I observed signs of impairment, he had an odor of an alcoholic beverage emanating from his breath and person.
On Sunday, November 5, 2017 at approximately 7:26 AM, I, Officer Raul Marrero and Ryan Holroyd were dispatched to 820 SE 9th Ct, in reference to a suspicious vehicle complaint. Dispatch advised of a white Pontiac car that was parked in the driveway. The complainant/homeowner, James Robert Caves Sr., didn 't know who the vehicle belonged to.
