The reporter stated John was has been diagnosed with bipolar disorder, ADHD, marijuana abuse, mood disorder, severe mood swings, disruptive behavior, border line dyslectual disorder, basic personality disorder, mental incompetance; John is unable to function on his age level and has a very low IQ; John is unable to read, write, and count. Presenting Problem: Dejon has a diagnosis of ADHD- CT, Disruptive Mood Dysregulation Disorder, and R/o Learning Disorder. He has presented with being verbally and physically aggressive towards his siblings and mother, and he requires constant adult supervision. He is impulsive and he lacks the skills needed to be trusted in the community. De’jon’s poor judgment leads him to be put in situations where he is either harming others or setting himself up to be injured. He was involved in a shooting of a 7 year old child in the head with bb gun in Marcy 2016. He intentionally obtained the bb gun. He also threatened bodily harm to school staff and was involved with shoving a teacher this past school year. He has poor coping skills and he becomes explosive and dangerous to others when he is in frustrating situations.

CPS received CPS referral #0677-3482-7506-1042519 regarding minor Natalie Thompson (DOB: 10/22/01). On 4/7/17, minor threatened to kill herself by jumping in front of a train because she was upset over what happened in her new placement at the group home. Minor stated that she no longer wanted to live and did not want to return to group home. Minor stated that she expressed to staff that she wanted to be left alone, but staff disregarded her request and held her. Two female staff held her back so she proceeded to strike them. During the restrain, staff grabbed minor by her arms, legs and shirt. One of the staff members (unknown) pulled her hair and held her tight so during the restraint causing the minor to sustain multiple bruises on her body.
Presenting Problem: He has 2 Kempsville Acute visits, endorses anger and thoughts of suicides. Reported hx of Neurodevelopmental problems most recently as consistent with Autistic Spectrum Symptoms. He reported having intense anger toeards his parents but denies homicidal thoughts, acknowledges suicidal thinking. On May 27, 215 he pulled a knife on his mother, no injury occurred. Mother states he has been increasingly agitated. He has been posturing at his mother yeling clinching his fist at her as well as sneaking out and lying. Mother states he obtained the knife from his room and threatene her life. He states his mother swas chasing him around with a broom which is why he grabbed the kife. He states he is depressed all the time. He a Hx of suspensions for fighting while in school and he was the aggressor.
R/s there is concerned that Ms. Brown wasn’t able to follow directions when asked to take off her clothes and someone had to instruct her how to do it as if she was a child. R/s Ms. Brown said Mr. Madden said for her not to tell anybody because he will her.
John presented with anxiety, volatile anger, and potential conduct issues at the age of six. The mother reported that John has witnessed multiple disruptions in the family due to parent’s addictions and divorce. John’s mother went into a recovery center and was released to a transitional housing center approximately one and a half years ago. His mother had visitation rights and John would stay with her in the housing center. Prior to being transferred to my caseload, John was diagnosed with F41.1 Generalized Anxiety Disorder with a rule out of F91.1 Childhood Onset Conduct Disorder, and was seen by a licensed clinician at another facility for approximately one year. His treatment goals previous to being transferred were to display developmentally appropriate behavior and reduce his aggression toward others. John was resistant to treatment.

James and Samantha where referred to the school behavior specialist. Truly that is only a start to the process. The aunt also needs help on why she is in a shelter for abused women and children. There is going to be different types of support needed, from, school, to understanding why their parents are in prison and know living in a shelter and a new school.
Presenting Problem: Kaleb is a 12 y/o white male presenting ADHD-CT, ODD, and Adjustment Disorder with anxiety and depressed mood. Kaleb was alleged locked in a closet as a form of discipline for lying, stealing and wetting the bed with only a mattress, pillow and bed sheet allowed where CPS was involved. He was required to perform numerous military style exercises as a form of discipline. He has been engaging in assaultive behaviors at school, and at the Seton house. He has demonstrated behaviors in foster care placement to include cursing using racial slurs, and threatening to run away. He is currently placed back with his biological father and stepmother. He also engages in lying, stealing, manipulating, fire setting, abusive behavior
Please review the case notes for this patient. This is a DCF case and her teenage kids has been removed for the second time. She is recommended for mental and substance abuse treatment from DCF. Deborah works for SalusCare in the prevention department and she has a lot of information about this case that she will put in the SalusCare medical record file. The information that Deborah has access to is from the DCF data base.
Psych/Mental Health: Tina is diagnosed with F91.3 Oppositional Defiant Disorder. Youth is not on any medication at this time. Tina was hospitalized back in Tina was hospitalized back in March 7th, 2017 for inebriated and out of control. She was discharged on March 23rd, 2017 from the CCIS unit. She then was stepping down to the Tina stepped down to the Trinitas Adolescent Full Day MICA program on 3/24/17. Tina successfully completed the MICA program and was step down to the Trinitas MICA group on 6/20/17. Tina continue to miss her weekly group sessions at the MICA program.
PER REPORTER: Rhonda said today when she contacted Teayrra to offer her case management services to help her with her thirteen-year-old son (Donavin). She said Teayrra told her, she does not want the child in her home at this point due to him being a danger to her and her other three children. According to Teayrra, Donavin threatens to kill her as well as his siblings and attacks his sisters physically. It is unknown if Donavin ha ever left any bruises or scars on the children or what he actually hits them with. She said she has also found knives hidden in Donavin’s room causing her and her other three children to sleep in locked room or in the car due to them being afraid of him.
