Learning Team Introduction Over the last several years, a wide variety of health care organizations have been facing tremendous challenges. This is because they are using different models that will impact the underlying quality of care and the focus of staff members. To determine the most effective approach, there will be an emphasis on a number of areas to include: examining the community model, identifying three categories, comparing this with nursing practices, contrasting these ideas to Mihaela Colea views and discussing the importance of advocacy projects. Together, these different elements will provide specific insights that will highlight the best practices for a modern day health care environment. The Community Model and Historical Developments The community model is focusing on giving stakeholders more of say in how health care solutions are provided. Under this approach, they will play a part in working with administrators, doctors, nurses and patients to identify the best solutions. At the heart of this strategy, involves using primary care providers who can cover the basic issues surrounding someone's health. This is a part of historical trends that were focused on reducing the underlying costs for these services and increasing the total amounts of quality. One of the aims of the Patient Protection and Affordable Care Act (ACA) of 2010 is improved integration and coordination of services for primary patient care. The patient-centered medical home (PCMH) is one of the approaches by which improvements can be established. The patient-centered medical home model is particularly well-suited for people who have chronic illness. The design of the patient-centered medical home model departs substantively from traditional reimbursement policies, in that, the ACA provides for incentives and resources to enable care coordinators to be directly recognized and compensated for their care coordination work. Care coordinators are most often registered nurses who through their work that aligns with ACA engage in quality improvement work, cost-effectiveness measures, and patient advocacy. To bring the ACA model to a human scale, the authors present a case study of a care coordinator at a patient-centered medical home in rural Maine. These changes were the improvement of research and evidence-based practice, health awareness, prevention screenings, and community-based wellness promotion and applied. This law or Obamacare, as it is commonly called, was designed to cover the 48 million Americans, including about 1 million in New Jersey who did not have health insurance. It is envisioned to provide seamless, affordable, quality care that is accessible to all. Great emphasis will be placed on transforming our current “sick care” hospital system into a community “health care” system of prevention and health promotion.
