Abstract The paper compares and contrasts the observed and evaluated role of the nurse practioner in the emergency department to that cited in professional guidelines, theory, and research. The paper evaluates what it entails for a nurse practioner to learn about their role within the health care system. I contacted Shannon Hamilton, FNP, MSN, for a knowledgeable discussion to explore the role of the advanced professional nurse in the emergency department. Ms. Hamilton is a novice in the field, having only practiced for two years, however she is able to give a fresh insight on the role of the advanced practice nurse in the emergency setting. ED ACNP have been practicing in emergency care settings for at least 30 years. Since that point, some ACNP programs have added the necessary components to their curricula so that graduates could function similar to ENPs caring for patients across the life span. Studies found that while some NPs work in fast track areas, other NPs worked in the main ED, providing care to acute and critically ill patients with a primary focus on stabilizing acute and critical care conditions. These early ENPs were successful in establishing practices in multiple EDs, creating the way for other NPs. The Emergency Nurses Association Scope of Practice for NPs in ED settings identifies that knowledge in advanced physical assessment, ability to triage and prioritize, emergency preparedness, stabilization and resuscitation, crisis intervention, and consistency across the continuum of care are essential aspects for practice.

As an acute care nurse practitioner I hope to utilize my experience as an Intensive Care Unit nurse along with clinical knowledge and skills acquired through a graduate level program to provide the highest quality patient care. In my current role, I place high value on patient-family centered care and advocacy as well as commitment to lifelong learning. I have built my nursing career upon these values. I am excited to begin the journey of becoming an acute care nurse practitioner and to further my education and expand my scope of practice in the field of critical care, for which I have already developed a passion.
Healthcare systems and the way safe, quality health care is delivered are continually changing to better serve patients and communities. Professional nursing practice is a large component in the healthcare system today. Back in the 1960s, professional nursing leaders tried to adopt the bachelor degree programs as the only educational track to become a registered nurse (Creasia & Friberg, 2011). Due to nursing shortages and demands this motive did not hold fast. Individuals entering the nursing profession today must first decide which educational pathway to take to become a Registered Nurse (RN).
In the traditional health care model, formal learning is not complete when a physician receives their medical degree and white coat at the end of their academic program. Medical school graduates are required to complete an accredited hospital residency program to become licensed to practice medicine, (Jolly, Erikson, & Garrison, 2013). However, acute care nurse practitioners (ACNPs) are not afforded, nor expected to complete, the same educational requirements. The current expectation that a new graduate ACNP learn their advanced nursing role “on-the-job” is unacceptable and leads to decreased job satisfaction and preparedness. In the every-changing climate of health care, the development of required residency programs for ACNPs should be maintained as a priority.

This study will include nurses that are currently working in emergency department with at least six months of experience with varying ages and levels of education. The emergency nurses would be required to complete a formal training on understanding the Emergency Severity Index (ESI) algorithm and the complexities in accurately assigning a triage level. Many people believe mandatory residency programs for newly licensed nurses are long overdue. The knowledge needed to practice nursing has grown to include health policy, ethics, public health, research, healthcare delivery improvements, as well as leadership skills. Nurses are part of the multi-disciplinary team and must work together with healthcare providers who possess masters or doctoral degrees. Nursing school prepares nurses with the basic information they need to know in order to practice safe nursing. However, modern nurses are faced with ever increasing demands. New nurses must know how to collaborate with the healthcare team, manage their priorities, stay organized and recognize life-threatening situations. Many new grads are overwhelmed with the demands of nursing, which can lead to compromised patient care and safety. Formalized, supervised guidance will improve patient care and retain nurses. The study also recommended doubling the amount of nurses with a masters or doctoral degree by 2020 in order to provide an adequate supply of nurses who are able to assume nurse faculty, primary care providers, and researcher positions.
