Understanding Inter-Professional Health Care

Inter professional working is formed from different health and social care professionals working together towards a common goal to meet the needs of a service user. It is about developing relationships within and between organisations and services involved in planning and delivering care and support to the service users we support. By working collaboratively it brings together different types of professionals to share their particular knowledge, experience, skills, occupational values and perspectives to improve service

The issue of interprofessional working is currently one of key importance in the field of health and social care (Moyneux, 2001). Using the 6 stages of Gibb’s Reflective cycle (1988) I am going to demonstrate my understanding and explore the importance of interprofessional working as well as discuss barriers and facilitators for team working. A healthcare system that supports effective teamwork can improve the quality of patient care, enhance patient safety and reduce workloads that cause burnout among healthcare professionals (Oandasan, 2006). The 6 stages of Gibb’s cycle include description, feelings, evaluation, analysis, conclusion and action planning for future practice. I am going to reflect on the preparation work which was carried out each week for the group summative presentation and the importance of communication within the group.

Another important aspect of healthcare is effective interprofessional practice. This allows practitioners from different disciplines to work together to provide the best care for patients. As time goes by, people get infected with new diseases or current prevailing diseases. Consequently, new drugs are produced in an attempt to overcome these diseases, which results in patients with complex health needs. The complexity of the patients’ healthcare therefore needs to be addressed by more than one discipline. Interprofessional collaborative care is a type of health care that involves people from different professions working together and relying on each other to provide effective care to patients. Interprofessional collaborative care is important and predominantly a focus of the health care reform because it improves healthcare outcome for the patients and reduces disagreements between different professions. I was able to experience interprofessional care when my grandma was diagnosed with cancer. The physician and the pharmacist worked together in ensuring that she got the proper medication that would not have any side effects on her. In addition, the physician would call the pharmacy every two weeks to check on my grandma’s progress and to ensure that she had picked up her medications. Both my grandma and I were grateful because we felt that our needs were taken care of appropriately. Moreover, interprofessional health care is important in a curriculum because if helps the students to be better prepared to work as a team. This essay aims to provide a critical analyse upon interprofessional working in health care presenting its importance and identifying issues and barriers that impact on the patients ' treatment.

Hall and Waver ( 2001) defines Interprofessional as a group of professionals from different disciplines such as nurses, doctors, pharmacist that are working and communicating with each other while providing their knowledge, skills and attributes to enhance and support the contributions of others. Suter et al (2009) also said that the ability to work in an interprofessional team to convey collaborative, patient-centered care is an important aspect of professional practice that involves a possession of a particular set of competencies, such as communication skills. In relation to (Weinstein et al., 2003) Collaboration is the collection of knowledge, skills, values and motives which transforms to effective practice when applied by practitioners.

“All health care disciplines share a common and primary commitment to serving the patient and working toward the ideal of health for all.” (American Association of Colleges of Nursing, 2014, p. 1) There are many different professional members in the healthcare system. Each of them, have a specific specialty and responsibility to the patient and play an important role in the patient’s overall plan of care. “The scope of health care mandates that health professionals work collaboratively and with other related disciplines. Collaboration emanates from an understanding and appreciation of the roles and contributions that each discipline brings to the care delivery experience.” (American Association of Colleges of

Kaakinen et al (2015) discusses the two types of health care teams that will be involved in the patient’s care: multiprofessional and interprofessional. The multiprofessional model is an older model that does not focus on holistic care. Care is fragmented with an autocratic leader, vertical communication, separate goals of the professionals involved, and families are peripheral to the process (Kaakinen et al., 2015). The preferred model is the interprofessional model emphasizing a team approach, holistic care of the patient, horizontal communication, and involvement of the family (Kaakinen et al., 2015). The nurse should keep this collaborative approach in mind when dealing with any patient case, involving all teams associated with the patient for the best outcome.

As a way to promote interprofessional collaborative practice the HPHA IPC model recommends the implementation of an Interprofessional practice council, unit action councils, interdisciplinary care plans, documentation, and “bullet” rounds (HPHA Interprofessional Practice Model, 2010). Similarly, Weller, Boyd and Cumin recommend supporting interprofessional collaboration with protocols and procedures, using procedures that encourage information sharing among the whole team, such as checklists, briefings and IT solutions (2014).

From this class I understood that Interprofessional Education (IPE) is when two or more profession learn from, with and about each other. This collaboration is important in order to fulfill the patients’ needs that are becoming more complex, multi-faced and challenging day by day. It is also clear to me that no single profession in healthcare can adequately address all the demands of patients. Therefore, partnership between teams is required to provide a safe, timely, effective and equitable patient care. To have a smooth collaboration, it is necessary to learn from each other’s specializations, strength and experiences. Interprofessional practice is a collaborative practice where multiple health professionals work together in health services to provide comprehensive services to their patients, families and communities to get a more effective result by improving the quality of work. The collaborative practise is basically used by the nursing team or other health care workers who are the member of interprofessional team. “A call for interprofessional team and collaborative practice development has been sounded across Canada because this model is viewed as the way to ensure that all professionals and providers can practice to the full potential of their role and competencies” (Potter, 2014). Interprofessional collaborative practice is a way to ensure that human health resources are used properly which help to decrease the duration time for achieving a quality care.

“Interprofessional working involves complex interactions between two or more members of different professional disciplines. It is a collaborative venture in which those involved share the common purpose of developing mutually negotiated goals achieved through agreed plans which are monitored and evaluated according to agreed procedures. This requires the pooling of knowledge and expertise to facilitate joint decision making based upon shared professional viewpoints” (Barrett, Sellman & Thomas, 2005, p18). How individuals collaborate and work towards a common goal for the benefit of the patient is essential for a swift recovery.

Teamwork is vital in healthcare.  When all participants are engaged in a program, goals are successfully achieved. Being able to communicate and work collectively as a team requires an appreciation for each other’s area of practice.  Every team member has an important role and being acknowledged provides a sense of responsibility and accountability. Essentially, inter-professional collaboration helps ensure that the patient is getting care that is not only accessible but also comprehensive.  The plan of a patients’ care includes active participation by all health care professionals working interdependently in accordance to the patient’s preferences, values and beliefs. The health care team accomplishes the goal of meeting the patient’s medical needs by delivering evidence-based practice.   To deliver quality care, the patient should always be involved.

　　　　An effective care for patients can be achieved through the collaboration and team work within the certain health care teams to provide a focused and shared learning in health professional education, which can help enhance the outcome of the patient’s needs.1 Working with different professionals and care givers can lead to issues. This can be looked upon so that the patient is given good service and enhanced care.
