There is one role of organisation responsible for commissioning social care services which is the local authorities, since the local authorities order the social services to achieve a personalised support that will promote health and wellbeing. Local authorities work with other organisations such as NHS partners.
Health and wellbeing boards are the role of bodies which is responsible for integrating health and social care. Health and wellbeing boards are for local health and social care leaders. Their aim is to integrate public services for the health and social care sector and they are tasked with reducing the inequalities in providing treatments to use, encouraging greater integration and assessing the needs of their local population.

A multi-disciplinary team have professionals from the same service with different roles and they work together to support an individual. It is important that an individual gets a good service and a better outcome from a service provider. Team must be aware of conflicts that may occur between professionals, patients, service providers and service users. Multi-disciplinary teams are important because they are created based on an individual’s specific needs and meeting the needs of an individual can help the individual in many ways as it helps improve their health and wellbeing. It is important records are accurate, up to date and clean with no chance of being misunderstood because it could lead to the wrong treatment or sending information to the wrong person or address which could lead to a random person seeing the confidential information.
Advantages and disadvantages of maintaining confidentiality and managing information:

It could bond trust between a patient and the staff taking care of the patient. Easy to break the trust and relationship between a patient and the person taking care of them when breaking confidentiality.
All information about the patient is kept secured and safe, so it will not be seen by another person. It could lead to potential threats to the patient when information is carelessly shared.
