Successful CC =2 CM received a call from Mrs. Patterson (Caregiver) regarding today, 4/17/17, 30-Day CFT meeting. Mrs. Patterson requested to reschedule CFT meeting time due to time conflict. CM and Mrs. Patterson agreed to meet today at 11:45am. CM will follow-up with Maria Flores (FSO Partner) to inform her of new time. CM contacted FSO partner to inform her of new time due to time conflict. FSO partner will be present for 30-Day CFT meeting.
The pt Poteat, Edward L is supposed to tx today at unit 1087. The unit 1087 is requesting the referral source to send a shortened and missed treatment report in order for the pt to be accepted. This report is not one of the requirements that is needed in order for a pt to transfer to a FKC unit. Also the referral source stated the she sent the report several times and received several conformations and clinic 1087 is stating that they never received it. The referral source and the unit 1087 have been in contact and I am just now finding out about this. Please let me know if the pt is accepted and to see what we can do regarding this patients
UCM:CPSW received a text from Morgan (Fits) reporting that she has received the referral from Amanda and that she is available on Monday's and Friday's at 11-1pm. Amanda will start on 1/28/17 at 11am Brookdale location. Also, CPSW called foster parent and Amanda about Monday appointment and that Morgan will contact them regarding supervisied visit. Goal 1-2

UCM:CPSW received a text from Ms. Borkovec reporting that she has scheduled an appoitnemtns with Phyllis Wheatley and her first intake appointment is on11/29/16 at 3pm. Also, CPSW reminded Ms. Borkovec to schedule with her mental health assessment and let this worker know time and date for her 1st session. Goal 1-2
Word had spread quickly on Fort Huachuca that contact had been made with Washington D.C. and the President of the United States. Everyone on post had questions but there were very few answers forth coming. It was fortunate that General Watkins and Colonel Clayton happened to be on post when contact was made. General Watkins was more than happy to take the lead on the call with Washington as the post commander was at North Island visiting Admiral Meyer. There had been some initial contact with someone claiming to be Corporal Byrd from White House Communications. It was lost for some unknown reason only to return almost two weeks later.

During spring break we had multiple practices for track. On the 24th right after practice Mr. Mahoney and I met up to cover all the bases. There are two big meets coming up that we had to prepare for. One being a CYO meet that is a little in the future. During the five hours we met I had to research and figure out where these certain schools were located, what their phone number was, and how many kids are on their team. I then had to enter all that info into a website called penntrackxc.com. Once those teams are entered then they can be claimed by their coaches and entered into those meets. After that I had to rewrite and type up a document in Microsoft word about those CYO teams that could be sent to the head of penntrackxc.com to authorize the info.
Devin, in her email says (morning), "Amber Leitao missed the take home group yesterday due to an issue with her car. She never misses sessions with me so is it possible to not put a no show for yesterday so that I can put her in the February 2nd group." It's commendable that the patient attends to her counseling sessions since 12/12/16. However, her note contradicts her comment because it reports that the patient was stuck in traffic, not having a car issue.
