Person-centred values are about focusing on the individual and their needs it helps us to realize that although individuals can have the same medical conditions it doesn’t mean they require the same support and care. Being person-centred is being able to understand about the individual including their needs, culture, means of communication likes and dislikes. Values can be defined as what is important in the life of the individual you need to think about their individuality, independence, privacy, dignity, choice and respect.

1.4 - Explain how using an individual’s care plan contributes to working in a person-centred way
Care plans are completed with the individual and any other relevant members such as friends and relatives they concentrate on an individual’s needs and their choices. Care plans are created around person centred values and needs they hold information about an individual’s medication, medical conditions, mobility, communication and spiritual. The care plans are reviewed frequently to ensure the individual’s needs are being meet and that any changes are updated to ensure needs are still being meet. They look at what the individual can do for their selves and ensure that they are given the opportunity to do it their selves to help maintain independence.

Sources of information are from the individual their selves, family and relatives of the individual, other professionals for example GP’s, social workers, nurses, referring to documents for example another care plan if they have transferred from another care home or from community care to residential care they will have a care plan from there.

2.2 Demonstrate ways to put person centred values into practice in a complex or sensitive situation.
In looking at this model I have found that many people have views that based on discrimination and prejudice they are embedded in today’s society, the attitudes and the surrounding environment often focuses on what a person lacks in terms of disability and focuses on condition or illness or a person’s lack of ability. To combat this prejudice within making space we use person centred care planning it is my responsibility to promote the use of this and to ensure that the support workers who deliver it are fully trained to do so. In this report I will be investigating how care services meet the needs of individuals by firstly analysing the needs of an individual using care services. Then I will go on to explain the roles of the care planning process in identifying needs, and explain the features of a positive care practise. After explaining that, I will analysis positive care environments and evaluate how they meet needs before finally explaining the role of legislation in promoting a positive care environment.
Staff are required to make an entry in to an individuals care plan once in twelve hours. This entry is in the daily life and review and will contain details regarding medication administration, dietary and fluid intake, elimination, mobility, mood, behaviour exhibited and any changes or deteriation of the individual. There is also a requirement to record visits from doctors, nurses and other health proffessionals.

Person centred values means the people whom we support are treated as equals and are involved in all aspects and areas of their care and that they are respected and valued as individuals. This means being involved in their assessments, care delivery and support planning, basically person centred values is making sure all approaches, policies and procedures and care practices put the residents at the centre of the day-to-day activities. It should also include the residents and their families in the planning and maintaining of this.
It is important to review care and support plans as people’s needs change. By including the person, their family then everyone knows what is happening and the family can help to monitor mood swings and behaviour. The individual and the family can express their views and preferences and any relevant risk assessments may be done with everyone involved. By monitoring the individual, a decision can be made as to whether the changes are effective and if the best care is being given to encourage independence and promote dignity.
It is necessary to involve the individual in the plan of care and support. Encourage the individual to make choices. This includes their needs, their culture, their means of communication, their likes and dislikes, wishes and feelings, advance directives, beliefs and values, involvement of their family and other professionals. This should be considered and documented. Also, there must be evaluation in assessing effectiveness in the plan of care.

Developing a self-care plan is an essential tool within the health care profession. A plan not only helps create balance in one 's life, but it also assists in maintaining professional standards as a service provider. In this type of work can be taxing on an individual 's mental, physical and spiritual mentality, as such, can affect the mannerism in which a professional engages with their clientele. As a social worker it is our responsibility to maintain the ethical values of our profession. The following is a discussion of my intended self-care plan for the remainder of this semester in order to sustain my commitment and values to my clients as a professional. As well, to help maintain my own stability in my personal life and in achieving my goals for this semester.
