Brittney was aggressive towards staff; she refused to stay in the safe space in medical and attempted to exit the medical safe space. Staff was able to step into Brittney path; Brittney began punching, pull and pushing staff. Staff refused to allow Brittney out of their eye sight. Brittney sat of the air mattress and put her blanket over her head removed her tongue ring en embedded it into her right ear. She also removed sutures from her left arm.
On the video, the nurse was caught slapping the patient 4 times in the abdomen, pushing and pulling the patient recklessly, and then slapping the resident in her face to finish the work.
On January 19th, 2009, Ian Andrews was given a seven-month suspension from the nursing profession. A year prior, Andrews was working as a full-time practical nurse at a mental health and addictions hospital located in Ontario. The client affected by Andrews ' actions was an elderly individual, who was diagnosed with autism and presented occasional periods of rage and fury. The client’s care plan involved managing periods of aggressiveness through staff support and the use of restraints, as necessary. On the day of the incident, staff nurses noticed that upon the client’s grunt, Andrews punched the client in the face several times, as the client tried to cover his face. Andrews then took the client into the bathroom where thudding noises were heard. Upon arrival, the nurses noticed Andrews standing over the client uninjured, with the wall covered with traces of blood and a broken faucet resting in the sink. Later in the evening, Andrews ' left a progress note within the client 's chart which described that the client had injured himself due to a period of restlessness. Upon patient assessment, a wound was noted on the client’s bottom lip along with minor lacerations present at the back of the client’s head.
While during round today on Thomas Unit Justine P disclosed that she had been collecting sharps all day. Justine were able to pull several pieces of plastic ware (Spoon/ Fork), a stitch from her injured wound, from her bra also and a rubber band. Campus Supervisor was able to retrieve all items from her. Also Justine disclosed that she removed all the stitches from her wound and began to embed new items into her wound. Nursing was contacted for assessment.
On Thursday, August 27,2015, @ 5:29 pm ,Tyneice Daishae' Key( DJS Youth ) was redirected by Dennis Thomas ( Senior Residential Staff) to return to Tours Unit. Tyneice Daishae' Key( DJS Youth ) became verbally aggressive towards Telethia Menefee ( Residential Staff). Tyneice Daishae' Key ( DJS Youth) also began to curse and threaten to hit April Smith ( Supervisor of Tour Unit). Tyneice Daishae' Key( DJS Youth ) was redirected to her room to take time away. Tyneice Daishae' Key( DJS Youth ) was able to return to the unit and go to her room. Once in her room Tyneice Daishae' Key (DJS Youth) began to use her chair to hit the door with. This adverse event should be escalated properly so that the administration and other doctors are aware of the outcome. This death could have been prevented, and others should be able to learn from this. We don’t know the full story from this short problem described in the book, but many questions arise from the situation. Was she completely aware of the risks? Did she know she was not a good candidate for the surgery?
I arrived to work well rested and ready to start the day. I had just returned after a two week long vacation. On 11/16/15 I spoke with Nelda Shaver the owner/director at the operation. Ms. Shavers stated she was in Queen City at an operation she is opening there. Ms. shavers stated Versie Crossland was in charge while Brittany Shavers was on bus route that day. Nelda stated Brittany called her around 3:00 and told her Daniel had fell and cut his forehead. Nelda stated she then returned to the operation, but by the time she arrived Daniel had already been picked up.

Also, Mrs. Glynn informed the doctor that he was hurting her and he told her that it was because she wanted to save tooth # 32 which it was just a wisdom tooth. T he following day which it was Saturday, September 17 in our facility in the North, another issued took place with another patient her name is Jimenez Saida was schedule for extraction 16, 18 and 19, but on this day Dr. Sadati only performed extraction on tooth # 18 because while he was extracting this tooth Dr. Sadati yelled to the patient “Open your fucking mouth” this patient speaks English and Spanish and she understood. Furiously Mrs. Jimenez got up from the chair and walked to the front area and complaint about how rude and disrespectful how she was treated by the doctor. In this occasion our dental assistant Dwight was assisting the Dr. and he witness this incident. Both of this patient’s have not come back to our office due to the fact that they were not treated with respect, but I am sure that they would be willing to testify these
Shydae was talking to staff about an incident that happened early today with Jessica. Shydae became agitated with Jessica when she began to bang her head on the wall. Staff attempted to redirect Shydae several times. Shydae went into her bedroom and grab a weighted vest and removed a weight from it and throw the weight at Jessica . Staff encourages Jessica to go to her room but she refused and return to the milieu. Shydae became aggressive towards Jessica ; staff intervenes quickly to avoid any physical altercations. Wanda was yelling and pacing the unit. She began trying to hit staff member Pamela Goode. She did not calm down with verbal redirection. Provider Joshua Burgett, NP determined she needed IM medications. She refused to voluntarily receive the medications so PerryLee Chambers, BHT and Michael Osgood, BHT did a backward carry hold to get Wanda to her room. She was assisted to the bed and held to receive IM medications of Ativan, Haldol, and Benadryl. Ashish Lal, BHT and Shane Dewitt, BHT were also present to assist if needed. Once the injections were given she did not resist or fight so she was immediately released. Neither Wanda nor staff were at all injured.
Case :- A 17-year-old female presents to her local hospital 's E.R. at 7:45 in the morning on a school day. She appears apprehensive and uncomfortable. She winces and splints her chest to the right side when she coughs. She is examined by the attending resident.
Staff Disrespect: One thing that is sometimes overlooked as an owner is that even though you are an owner, you have to show respect to your players, your staff, and your potential staff. If you disrespect them, insult them, yell at them, or just just plain rude to them, then they might leave you, and then tell others that, which would not be good for you.
As I walk into the bathroom on Pelletier I witness Brittney W, bang her hand against the wall, mirror and storage bin. I attempt to process with Brittney, asking probing question and also offering support. Brittney refused to talk and continue to bang her hand against the wall, mirror and storage. I placed my hand to block and insisted that if she needs to hit something hit my hand. Brittney became agitate and walk out of the bathroom and began to pace up and down the hallway.
