Before visiting CTC my future plans and goals were not clear. After visiting the CTC I had a road map of my future and I’m very excited for the years to come! The program at CTC that I’m interested in is health care cluster half day my junior year, and introduction to health careers half day my senior year. Ever since I was really little I have been around first responders, firefighters, EMTs, care takers, and nurses. Monday December 15, 2014 was the first time I ever got to meet my 6 month old nephew Brice. I had to drive to Hershey medical center and I knew he was in critical condition because he was born with hypoplastic left heart syndrome. We originally did not recognise that the child had Foetal Alcohol Syndrome. The child was a slow developer and one who would have huge tantrums. We didn’t think anything of it but just thought some children develop skills slower than others and tantrum wise we put it down to, some children are just difficult but would eventually grow out of it. We only started suspecting something was different when the child started going to pre-school. If I had not had to go to work; then the child would have not attended pre-school but gone into school, when they would reach school age.
On 04/14/2016 at 2315 hours your Affiant was dispatched to 109 E. Center St. Apartment Upper for an overdose. Upon arriving on scene EMS was tending to the overdose patient. Your Affiant spoke with a female in the apartment who identified herself as Tiara Pleva while Stamets spoke with the male who lived in the apartment Aaron Campbell. Pleva stated Conrad shot up with heroin. Your Affiant asked where the heroin if any left and empty baggies are, and needle Conrad used. Pleva stated she does not know where any of the stuff is.
In the healthcare field, we are learning every day, and this makes us better equipped to serve our patients. I want to be able to grow in this facility and continue learning every day, and that is done through hard work and dedication. I’m a great team player and I want to be able to grow and help others grow as well. There is always room for me to grow as a person and I know I can do that by working together with others around me. During my clinical rotations at Texas Childern’s, I had the great privilege of working in a few different areas.
The biggest influence of my life has been the Dr. Bill Neches Heart Camp for Kids. Heart Camp is a weeklong summer camp for children who live with Congenital Heart Disease. I have been attending this camp since the age of eight and have worked as a junior counselor the past two years. As I have grown older and learned more about my heart problem, I realized that I wanted to work with children who are faced with similar trauma. By attending Heart Camp, I have had the wonderful opportunity to speak with many nurses who work at Children’s Hospital of Pittsburgh. In the waiting room, waiting for my twelfth surgery, the nurse could not get the I.V. to work. She had tried many times before on earlier visits, and it wasn’t working again. Tears were about to flow. Tension set in and was preventing the I.V. from functioning. Right as I was about to cry, a kind lady came in, held my hand, taught me techniques about calming down, and helped get a numbing shot. The I.V. went in the next time! I was in awe of this angelic figure. The moment I found out she was a Child Life Specialist, I decided this is what I want to do with my life; help children get through hard times in the hospital. I lived in Toronto basically my whole life. Living in Toronto TTC (the public transit system, also known as the Toronto Transit Commission) is a must. Whether it be going to and from work or making a trip to the Eaton Centre with some friends. With my years of riding the TTC I have definitely had a, if not many gendered experiences. Rush hour is definitely the busiest time to get on the TTC, it’s the time of the day where everyone gets on. From the business men, to the teenagers to the homeless all trying to get somewhere. In the few minutes or the long hours on the TTC we all have some way we put on a performance whether we know it or not, trying to protect or show the world our gender and sexual identity.
PER REPORTER: The children are in CPS custody. They are being abused. Per reporter, CPS won't give out any information about the children and their location. Reporter believes the foster parents are abusing the children because that's who take care of them. The children come for a visit every Tuesday. Last time, they had bites all over them. It's some type of bug bites. They said the children were taken to the hospital. This time, Bailey's left eye was swollen and red. It looked like someone hit her in the eye. They said Bailey was bit by a bull gnat. Reporter has a picture of it. This was seen on Wednesday, July 5th. They said the child was taken to see a nurse practitioner. Reporter stated he is not buying that and has a feeling that something is going on with the children.
This week the COTA brought along a self-pedal crazy cart with us when we went to pick up MR from his classroom. The vehicle immediately caught MR’s attention, and as he approached the cart the COTA stopped him by requesting him to greet us. After a quick greeting, MR sat in the cart and swiftly used both his legs to pedal down the hallway while using both arms to steer with the levers. Once we got to the door of the therapy room, MR stopped pedaling, got out of the cart, and opened the door to enter the room. His session started with developing oral motor by blowing on a pinwheel 10 times. MR blew at the sides of the pinwheel instead of the front. On 12/29/17 I met Ms. Becquette at the office of Dr. Saleh. She arrived ambulating with crutches. She had on an ankle brace and a full leg brace. The pin sites have all healed. X-rays done today showed that there is bone healing, the fractures have not fully healed yet, there is also some osteopenia, which we would expect from the dis- use of the bones. She can only bend her knee to 26 degrees, the ankle she cannot bend it is dropped and contracted, and she is able to wiggle her toes. The numbness she has had to the leg and foot is gone. The EMG has not been done yet because of Mom's schedule. She said she would call and make an appointment soon. Physical therapy is making some improvement. Dr. Saleh said he wants her to work really hard in therapy, he said it would be a last resort to go in and release the adhesions to the knee and the Achilles tendon for the ankle. Dr. Saleh said he feels she can work through this. She is allowed to bear weight as tolerated. The new brace has not arrived yet and is supposed to be in next week. We did address the attendant care and Mom was hesitant to decrease to 8 hours a day. Ms. Becquette will be going back to public school on 1/19/18. Physical therapy will continue even after her return to school.
R/s today Miracle was visibly upset in class. R/s Miracle reported that she is being molested by her 29-year-old brother, Jeffery. R/s according to Miracle, her mother is trying to go back to court so Miracle doesn’t have to go back to visit her father.
PER REPORTER: Erin stated that Zinna and Nancy brought Lesleigh in to the emergency today on December 14, 2016, because they were referred to by the child’s pediatrician. Erin stated that the name of the pediatrician that referred them to bring the child to the emergency room is All Children’s Clinic located in Coldwater, MS. Erin stated that according to both Zinna and Nancy the child had been complaining about vaginal pain since Sunday after she returned from her father’s (Lavell) house. Both Zinna and Nancy told Erin that when the child returned home Sunday she told them that Lavell licked and touched her vagina. The child also told them that Lavell told her to “turn your legs around I want to eat eat”. Erin also stated that according to Nancy the child started acting out this past summer. Nancy then gave an example of and incident that took place during the summer when Lesleigh crabbed her breast, and she told her not to do that. Nancy stated that Lesleigh’s response
Ever since I was a little girl, I wanted to work in Oncology. Specifically, I wanted to work at St. Jude’s Medical Center. All I have yearned for in life is to become a person that can take away the pain that someone is going through just with a simple smile and a conversation. The pain won’t necessarily vanish, but to be able to take it away even for a second and to keep one’s mind off of what is mentally and physically harming them would be successful in my eyes. Through this semester, I acquired the knowledge and tough lesson that I am not perfect and that nothing is guaranteed.

This MSA mentioned above, continues to slander my name around this VA. This entire incident started approximately a month and half ago, where worked in the Endocrine/Renal clinic and was questioned about makes made by Marlene in Sara NP Endocrinology clinic, by Sara. Marlene emailed Sara and told her that her mistakes were made due to the nurses in endocrinology treating her badly. This was relayed to the endocrinology nurse, who shared it with me. I just put that aside and did not thrive on it. It was then told to me by 2 other clerks (other units) that Marlene was very upset with me because I treated her so badly when she works our floor. Again, I did not make a fuss about it. While I was covering for Toni Ayers (nurse manager) one day, I received a call from Katina-MSA supervisor on some other business, and I alerted her that I have been told by several people that Marlene is upset with me, and what was shared by her. I only wanted Katina to hear my side of the story, because I contine to hear these same stories around the clinic. I have ALWAYS helped any new clerk that presents on this floor, due to multiple clinics and it can be confusing. I have never said anything out of the ordinary to Marlene, other than I am willing to HELP her, and told her my extension if needed.
On Thursday, November 3, 2016 at 0943 hours, I, your affiant Patrolman Joshua J. Pastucka and Patrolman David Stamets were dispatched to 119 N. West St. for a male trespassing. Stamets and I arrived on scene and made contact with the caller Ann Zbyradowski. Zbyradowski said a male that attempted to get into her yard is currently out back hiding under a rug on her neighbors deck. Zbyradowski escorted Stamets and I to her back yard and shown officers the deck where the male was hiding. The deck was attached to a home on N. Jardin St.
