REPORTER: The reporter/ Owner /Amada Senior Care (John) called with concerns for the victim, Juanita. Juanita can perform her daily ADL’s, but she appears to be very weak. Juanita lives alone. Laquanda Harper (perpetrator) was Juanita’s caregiver with Amada’s Senior Care. On yesterday (05/12/2016), Ms. Harper verbalized that she borrowed 100 dollars from the victim, but she paid her back. The victim caught Ms. Harper going in her purse (unknown when). Ms. Harper told the victim that she was looking for her social security card. The reporter said Ms. Harper brought her boyfriend over to the victim’s home, but Ms. Harper stated that she only brought him over twice. Upon our arrival, we were met by a Hispanic female adult identified as by her Nevada drivers license as Maria Ochoa. Ochoa stated that she and her sixteen-year-old daughter (Jasmine Henriquez) had been in a been in a verbal argument that turned physical. Ocha stated that argument started over Henriquez wanting to go to a party and Ocha told her no. Ochoa stated that when she told Henriquez she could not go, she became angry and said: "even if you tell me no I'm still going". Ochoa went on to say that she caught Henriquez trying to sneak out of the house at about 2230 hours. Ochoa stated that she confronted Henriquez and attempted to prevent Henriquez from leaving by pulling her shirt. At this point, Ochoa stated that is when Henriquez grabbed her right wrist with her left hand and began to punch Ochoa in her right arm with her right closed fist. Ochoa stated her youngest son Jonathan Ochoa (12/13/006) was present at the time of the incident. Ochoa stated that Henriquez took off in an unknown vehicle and left in an unknown direction. Ochoa had a Nevada plate of 371-08B, However that plate did not give a return.
The victim was unable to provide a possible suspect. However Nita Dobbs did state the house directly behind hers was vacant and a house nearby, 170 Palo Duro Drive was often rented out for short periods of time. Nita Dobbs stated she does wish to pursue charges. I advised Nita Dobbs this case would be forwarded to an investigator.
The reporting party (RP) received call from Diego Baca with Kings County CPS, 1400 W. Lacey Blvd., Hanford, CA 93230 (559) 852-2345 who reported that on 12/21/16 at approximately 3PM while conducting a placement visit for youth Andrew Yanez DOB: 12/18/01 the youth and the social worker took a walk. During the walk Andrew disclosed that he and a staff member named Shawn Wilcox were involved in an altercation. According to Andrew at approximately 2:45PM he was in the office of the ground home with 2 other staff members named James Boston and Nicole Juarez. Andrew was requesting the staff members to take him to the store to use is Christmas gift cards, when staff member Shawn arrived and asked Andrew to exit the office. Andrew said no because
On this date worker received a call from Ms. Val Wilson, RN at Alacare HH. Ms. Wilson has been Mrs. Brown’s nurse and been in the home multiple times. During Ms. Tara Brown time as caregiver the home was clean, food cooked, and Ms. Brown received her medication. According to Ms. Wilson Tara moved in during the summer after Larry Brown, son of Ms. Brown locked Ms. Brown out of her home. Tara told Ms. Wilson that her father, Larry Brown was MR and suffered from Schizophrenia. She also reported seeing bruises on Mrs. Brown’s arm where Mr. Brown would grab her. According to Tara Mrs. Brown received money after the passing of her husband Mr. Brown. However, Mr. Larry Brown got the money, spent it, and is now talking about filing bankruptcy. Due

S/L Walker was dispatching for evening shift at 08/29/15. S/L Walker receive a call for a welfare check at KC-219. The daughter, Karen, usually talk to her mother at around 2100 hr. But tonight could not reach her. S/L Walker took down the information and send an officer up to the apartment. S/L Walker called the apartment while the Officer was en route. There was no answer by phone. S/L Walker notified the Officer. S/O EMT Funes was the reporting officer. When S/O EMT Funes arrived on the scene, there was no answer by knock. S/O EMT Funes entered via her master key because the door was lock and search the apartment. There was no one home and S/O EMT Funes left a white tag #28798. S/O EMT Funes cleared. On 08/07/15 at approximately 1820 hours, my partner (Ofcr. Vanlenzuela No.38161) and I (Ofcr. Nunez No.36181) were working North Hollywood patrol, assigned to unit 15A84 W5. At that time, we responded 10849 Fruitland Dr for an “Attempt 211 Suspect.” Comments of the call indicated the P/R was the wife of the victim Melvin Salinas. Suspect was described as a male black “transient” armed with a knife tried to take a vehicle from the parking garage Inc# 150807003722.
At approximately 1040 hours, on 27-Jul-15, site security was dispatched by POC to attend Acute 3 for Domestic Dispute. The writer, S/O P.Jo, S/O A.Toor and S/O M.Moradi responded to the call and upon arrival the officers met with one of the clinical staff members, who informed the officers that a lady walked into the unit with two kids, one of the kids was about five year old and the other one was in stroller. The lady told the staff member that someone has been following her for three blocks and they need to call RCMP. After telling this the lady walked into the washroom with her kids, the clinical staffs ran after her and asked her to come out, but she didn’t listen and slammed the door on her face.
Upon arrival, I made contact with the victim, who was identified as Manzo Silva, Alicia DOB 10/10/84. Alicia related that she spoke little English and felt more comfortable speaking Spanish. Ofc. Calderon #291 was dispatched to the scene to assist with translation. Alicia related that she has been dating Jorge for approximately one (1) year. Alicia related that Jorge has physical abused her in past on at least four (4) occasions. Alicia was unable to provide exact dates of the incidents. Alicia related on 12/03/17, she had a verbal altercation with Jorge because he thought she was cheating on him. Alicia related that Jorge damaged her cell phone (Galaxy 8) during the verbal altercation. Alicia further related that he struck her with closed fist on the left side of her head, left, and right thigh. I took three (3) photos of Alicia, which were later placed into inventory. I observed redness on the left side of her face. Alicia related that Jorge left her residence in his vehicle.
On Monday, February 6, 2017 at 5:26 pm, Angeline Reynolds came to the Gautier police station and reported she had just been robbed. Reynolds was traveling south on Martin Bluff Rd, when she pulled off the road near the water tower by Interstate 10 to check and see if she had a flat tire. While doing so, a black male driving a red Chevrolet Colbalt (2004 or 2005 model), pulled over and asked if her brother-in-law, Bryce Reynolds was in the vehicle. Reynolds told the male that he wasn't and he walked over to her vehicle. The male had a handgun in his waist band and had a hand on it while he took $204 from her driver door pocket that was open. The male told Reynolds, "It's best you not come after me" and he left the area in his vehicle. Reynolds then called 911 while driving to the police station to report this incident.
On 6/9/16 at about 3:30 AM, HR Assistant Janine Kadelak on duty Shift Supervisor (S/S) Ana Penaloza about an accident that occurred in the C-Lot. S/S Penaloza and S/S Enmanuel Cabrera quickly made their way to the C-Lot to gather the information needed and take picture of the damage. Upon arrival, S/S Penaloza and S/S Cabrera were met by victim (VIC) Amazon Associate (A/A) Deborah Eisenhower and person of interest (POI) Integrity Associate (I/A) Stephanie Curley (curleysc). REPORTER: The reporter/ Social Worker (Crystal) called with concerns for the victim, Ezekiel. Ezekiel has suffered from a stroke, encephalopathy, and his new baseline is that he’s alert but oriented to self only. The reporter said Ezekiel knows who he is, but he gets confused. Ezekiel can perform his daily ADL’s. Mr. Tuders came into the hospital for alter mental status, and he was found to have encephalopathy as well as having a stroke. The victim has a history of alcohol abuse. On 02/03/2016, the reporter had a meeting with the family (Shane and Ali), and they said that it wouldn’t be safe for the victim to come back to the home. The worker contacted Irma Drew; who is maternal aunt of Saniya Daniels. Mrs. Drew stated “Saniya was at her great-grandmother Norma Drew’s home Mrs. Drew noticed Saniya had a knot on her forehead being concerned she asked Saniya how she got the knot on her forehead. And Saniya said she was hit. Norma the asked Gerald Daniels how Saniya got a knot on her forehead and Gerald said she fell playing with her cousin. Beforehand I asked Saniya too and she told she fell. Well my mother didn’t like the reason she was giving by Gerald and reported the incident to DHS. On December 30, 2016, C-1 reported to members of the Metropolitan Police Department that at the she became involved in an argument with her son Jamari Hawkins over his living arrangements hereinafter referred to as D-1. D-1 swept C-1’s property off her bedroom dresser and slapped C-1’s cell phone out of her hand when C-1 attempted to call 911. D-1 also threw a small glass in C-1's direction. D-1 then initiated a physical altercation with C-2 who is the boyfriend of C-1, which resulted in C-2 suffering from minor injuries. D-1 then left the location on foot, going eastbound in the 5500 Block of Call Place SE.
It was reported that resident Susie Elbert (age 52, DOB 10/28/63) has been observed sitting in a vehicle (Silver Toyota Camery, License #7PDB535) parked outside of 10 San Pablo Ave., San Rafel for up to 5 hours at a time while caregiver Marc Anglade is inside of the apartment. It is reported that the apartment is the home of Marc's mother. The resident has Down's Syndrome and dementia as well. Caregiver Gloria stated that she has only seen this happening for the last wee, but neighbors told her that this has been happening for a really long time and nobody knew what to do about it other than to confront Marc. Today Gloria contacted RP, as she realized that she was a mandated reporter, to report that the resident had been sitting in the car
