SSA placed phone call to Kim Budke-Provider. We discussed that the nurse care manager went ahead and met with John this morning with Kim being able to attend. She shared that John’s case manager with Lifepointe was going to attend but did not make the meeting. She shared that Jean is the nurse case manager with Cincinnati Home Care and they discussed finding John an activity to engage in during the daytime hours. She shared they discussed ADS programs in Williamsburg and John expressed interest in Cane Run Farm. She shared that they also discussed increasing John’s in home nursing services up to 14 hours per week to address his high blood sugar levels. Julie Gurnick is a broadcasting major at Point Park University from Jeannette, commuting an hour to campus each day. She was interested in Point Park University because it offered a good program in communications. Gurnick also holds an associates degree in business communications form Westmoreland County Community College. After graduating from Point Park University Gurnick aspires to become a sports caster. Gurnick is currently a free-lance employee for the Tribune Review as well as a dietary aid at St. Anne Home.
On this date worker visited Shadescrest NH, for the purpose of case monitoring. When worker arrived Mr. Hopson was lying in bed sleeping. He was appropriately dressed with good personal hygiene. During visit worker was unable to wake Mr. Hopson. Worker spoke with charge nurse and learned Mr. Hopson was doing well. Mr. Hopson became long-term Medicaid approved in Sept. beginning in August.
On September 21, 2012, ACEDS Case Action Update/Display documented by Social Service Representative, Richard Gordon stated that Ms. Williams arrived to the agency to apply for Supplmental Nutrition Assistance Program (SNAP) benefits, and Medicaid for her child only. During the visit, CARE interface revealed that Ms. Williams had an open Medicaid case in Maryland (#47403882500). Maryland Social Services also reported that she was employed by CPR Medical as of August 9, 2012, and paid bi-weekly $850.00. Ms. Williams also reportedly received unearned monthly income from SSI in September 2012 in the amount of $440.34, and in October 2012, in the amount of $370.54. As a result, Ms. Williams was denied expedited Foodstamps pending paystubs and her application was marked as "pending."
On this date worker visited Walker Rehabilitation Center in Carbon Hill, for the purpose of monitoring Ms. Blankenship's placement. Worker spoke with Ms. Ana Dyer and learned the family was getting assistance with housing and transportation from a local church. Ms. LaVone Yarbrough and her husband run a ministry that helps family reunite. However, Ms. Dyer is concerned about discharging Ms. Blankenship to the home with narcotic and according to Ms. Dyer, Crimson's boyfriend lives with them. Worker has not been able to conform. Ms. Dyer stated when Ms. Blankenship discharged from the NH she would need a doctor and transportation to the doctor appointments. In home health care, the patient is assessed to determine what types of services are needed such as skilled nursing care, therapy, home health aide services, and medical social services. The case mix adjustment is based on this assessment along with the patient's current condition. There are 153 case-mix groups that the patient can be classified under. In a skilled nursing facility the case-mix adjustment is assigned based on the resident's assessment along with the relative weights staff time data associated with their care. In a skilled nursing facility the PPS also allows for a geographical adjustment based on the hospital wage index for the facility's location. There is also a three-year transition adjustment that is part of the PPS for skilled nursing facilities. This a blend of the facility-specific payment rate and the federal case mix adjusted rate. Home health care PPS does not allow for this transition adjustment or the geographical adjustment. Instead there are adjustments for outlier payments, beneficiaries who only need a few visits, and beneficiaries who are readmitted within the 60-day episode.
