Circumstance: Ms. Smalls (MHP) made face to face contact with Ms. Cordova (caregiver) and Richard to complete monthly home visit. Action: MHP complete a walk-through of Richard’s living area. MHP checked Richard’s bedroom door to ensure it locks, no noticeable safety issues, and bed was neat. MHP, Richard and caregiver review new goal and objectives again to ensure Richard was clear about the expectations. MHP requested Richard’s monthly paperwork and receipts. Richard explained he did not have any completed paperwork and did not think it was fair that he had to complete the paperwork. The caregiver explain Richard only provide two receipts from last month. Richard expresses not wanting to work on goals and the steps. MHP and caregiver reviewed
