MHP checked Richard’s bedroom door to ensure it locks, no noticeable safety issues, and bed was neat. MHP, Richard and caregiver review new goal and objectives again to ensure Richard was clear about the expectations. MHP requested Richard’s monthly paperwork and receipts. Richard explained he did not have any completed paperwork and did not think it was fair that he had to complete the paperwork. The caregiver explain Richard only provide two receipts from last month. SC placed call to Pa’s CG and friend Teresa Lim and for monitoring phone call because none of the numbers on file for Pa was in working order. She reported that the Pa is doing well and is taking a break from radiation and chemo. She Provided the SC with Pa’s new phone numbers. SC asked about Pa’s service and Teresa reported that the Pa is receiving her PAS service specified in her care plan. Teresa reported that the Pa is happy with her current service and do wish to make any changes right now. The Pa is satisfied with her services and feels they are meeting her needs. She processes her assigned orders handedly and places herself in a position available to process legislative inquiry research for resolution as requested by local management. Alicia is an even tempered individual and works with all personality types in a positive professional manner. Her flexibility in regards to working several cases in tandem at her desk allows for prompt response to phone calls and email communications from management. SC competed home visit with Pa on 1/22/2016. Present at the visit was Pa’s sister who is also his informal caregiver Gilda. The Home was very clean and well keep. All utilities appears to be in working order. Pa reported that he’s experiencing unsteady gait frequently, but he had no falls. He reported that his sister and his niece (primary DCW Giyani Gram dtr of the informal CG) is helping him with meeting all of his ADL/IADL needs. Pa reported no outstanding medical visits, hospitalizations, ER visits, surgeries or changes in medications. Per Pa’s sister his health conditions are stable with very minor physical changed.
