Omari is a 5 year old AA/M presenting with symptoms of angry outbursts and disruptive behavior at school and at home. Grandmother stated that when Omari "doesn't gets things done his way", like being first in line, or win in a game, etc., then he explodes in a tantrum. When in an angry outburst, individual can throw things, kick, and challenge authority figures. Grandmother explained Omari has swing to hit even the principal. For that reason, he has been suspended twice this year, 2 days each. GM disclosed he is very smart and doesn't have any problem academically. GM explained she and her husband have been taking care of the individual and his brother for the last 2 years. Individual's father lives in NC and his mother's lives with her dad.
Child C is a seven year old boy. Having read through the referral form I am aware that Child C had been displaying aggressive and angry behaviour in school. He comes from an unsettled home life and has experienced a high level of domestic violence within his family home. Child C has spent time living with his grandmother
Family: Gabriel is 12 year old Hispanic male who lives in Fords NJ with DCP&P resource parent Mr. Ronny Chirichello and two foster siblings. Gabriel has been in Mr. Chirichello home for the past 19 months. Gabriel has adjusted well to Mr. Chirichello home. Mr. Chirichello holds Gabriel accountable for his acting out behaviors in school and give him appropriate consequences (i.e. taking away his cell phone, no TV, games system or outside time, etc). Gabriel responds well to Mr. Chirichello directives and house rules. Gabriel continues to have ongoing difficulties with emotional boundaries with his bio-mom. Gabriel and his sister does not have the best sibling relationship. Gabriel mention to Mr. Chirichello that before his brother passed away he and his sister was close but sine their brother’s death they do not get along.

Presenting Problem: Dejon has a diagnosis of ADHD- CT, Disruptive Mood Dysregulation Disorder, and R/o Learning Disorder. He has presented with being verbally and physically aggressive towards his siblings and mother, and he requires constant adult supervision. He is impulsive and he lacks the skills needed to be trusted in the community. De’jon’s poor judgment leads him to be put in situations where he is either harming others or setting himself up to be injured. He was involved in a shooting of a 7 year old child in the head with bb gun in Marcy 2016. He intentionally obtained the bb gun. He also threatened bodily harm to school staff and was involved with shoving a teacher this past school year. He has poor coping skills and he becomes explosive and dangerous to others when he is in frustrating situations.
Youth is currently attending Dieruff High School. Youth has a history of not attending school. Youth struggles with assignments in school, which is hurting his GPA. Youth struggles with a negative peer association. Youth struggles with controlling his anger and it lead to him getting into an altercation with a sercutiy guard at school. The incident cause the youth to be detain and placed on probation.
Gwenyth came to school on 09/03/15 with bruises on her left arm and the child indicated to the reporter that she had bruises on her buttock area; the bruises do not require any medical treatment. Gwenyth told Mr. Temple on 08/31/15 her mother asked her if she’d put clothes in the wash. Gwenyth thought she had and told her mother yes but later remembered she hadn’t; Ms. Daly thought the child lied to her and whipped her with a belt on her buttocks area, hit her with a wooden spatula on the arm until it broke, and grabbed her by the neck threw her on the floor and this is when she punched the child in her nose. John presented with anxiety, volatile anger, and potential conduct issues at the age of six. The mother reported that John has witnessed multiple disruptions in the family due to parent’s addictions and divorce. John’s mother went into a recovery center and was released to a transitional housing center approximately one and a half years ago. His mother had visitation rights and John would stay with her in the housing center. Prior to being transferred to my caseload, John was diagnosed with F41.1 Generalized Anxiety Disorder with a rule out of F91.1 Childhood Onset Conduct Disorder, and was seen by a licensed clinician at another facility for approximately one year. His treatment goals previous to being transferred were to display developmentally appropriate behavior and reduce his aggression toward others. John was resistant to treatment.
This is a DD waiver waiting list transfer case from RBHA. ISC met at the home of Ja'den along with his mother (Quannajia), 2 year old sister and one month baby brother his other sibling was asleep. Ja'den is a 5 year old who attends Glen Lea Elementary School. His mother reports he has been in trouble at the school for hitting his teachers (this is not a new behavior). He also curses at the other children his mother reports neither she nor his father curse around him.
Mr. Karimi is 28 years old guy with no previous criminal records and works at windows delivery company in Langley. Mr. Karimi is married to Mrs. Kia and they have a six month old little son. Mrs. Kia is 21 years old and completely depends on her husband as she does not have a job and new to Canada.
Jose is a 16 year old Hispanic male who was born in Mexico and migrated to the US at the age of 10. He is medium build with brown eyes and is approximately 5 feet tall. Jose is currently residing with his mom and 10 year old brother in Marcy projects. He attends Midwood High School and is in the 9th grade. Jose was referred to my agency by his mother who sought services to address his frequent outbursts in the home. Jose’s mother reported that Jose punches the wall and throws objects around the home when he gets upset. She further reports replacing three TVs over this year as a result of Jose’s aggressive behaviors. Kristian Ali complains of being depressed most of her life. She stated that recently she has had two suicide attempts, one which led to her admittance to the Psychiatric Unit at the Eric Williams Medical Sciences Complex. She has a current diagnosis of Major Depressive Disorder and Panic. Her treating Psychiatrist suggested that she should be assessed for Cluster B personality. The first two sessions with Kristian has led to the diagnosis of Persistent Depressive Disorder with Major Depressive episode moderate. This is because her history indicates that Kristian has been in a depressive state as far as she can remember but has still been able to function. My client, whom I will call “Jay,” is an eight-year-old, African-American boy, who was referred to our agency by his mother who wanted him to get “any services he could get for free.” He has some emotional and behavioral deficiencies, and does not handle stress well. He has frequent outbursts in class, and reacts without thinking. He needs to work on his interaction skills, and develop strategies that will help him use his words instead of physical reactions when he is frustrated. Because his trigger is frustration, he needs to learn to respond in a positive manner to his peers and teachers. Also, “Jay” needs to learn how to think and act independently.
