Dr. Losee,
Jennifer Devault is scheduled for surgery (palatoplasty) on 11/30 after she was not able to make her last surgery date (10/3) due to illness. If you remember, this is the patient that had been lost to care and had to childline to get them into a clinic appointment. We were then working with WVU to get care transferred to their team before it was ultimately decided that the family had to continue care here because of their state insurance.
A surgery consent form was signed the last time Jennifer was seen in clinic (8/30). Will you be requiring a pre-op appointment before the surgery? During the clinic appointment, we had talked about having the pre-op before the surgery, but again that was closer to the original surgery date.
In “The Pitfalls of Plastic Surgery,” by Camille Paglia, she argues that American surgeons are being too narrow minded with their work because they use the same models as a representation of what their clients can look like. She suggests that getting plastic surgery has become such a normal thing in America and that society pushes women to get work done in order to get that “perfect” look. Paglia also points out that plastic surgery has “leveled the playing field” with wives and their husbands mistresses (694). Also, she believes that plastic surgery is not only critical in keeping a job, but also in keeping a relationship. Although I agree with some of Paglia’s arguments, I view some of the issues differently such as, needing plastic surgery to keep a marriage, that plastic surgery is for non intellectuals, that Hollywood’s environment encourages women to change their looks, and that nothing good can come out of plastic surgery.
Client’s last medical appointment was in June 2014 and her latest dental appointment was September 2014.

I reviewed the providers claims. Wolfe Surgery Center is an Ambulatory Surgery Center. They’re billing codes that are not on that Fee Schedule, therefore all claims are pending with error. I will have the claims released.

It states the procedure to be performed along with any possible complications that may occur. Educating the patient prior to signing the informed consent is required by law. Documentation of the surgical time-out should also be done to make sure that the entire team has an understanding and agrees with the procedure to be performed on that specific patient by a specific surgeon.

“I’m now facing 12 months of non cancer IV chemo to regulate my endocrine system. It has weakened almost to the point of failure due to all the physical stress to my body. I need this time to receive expensive treatment to heal, get back to work, and write my book project about 50 years of medical disasters…oh and by the way…the 4 heart surgeries were due to defects I was born with…I was not smoking and eating pizza every day in my mother’s womb (laugh).”
Answer: Yes, there is a problem with gastric bypass surgery and extended release medications. The absorption of the extended-release medication will decrease after gastric bypass surgery, immediate release form medication should be recommended to this patient. I will recommend bupropion immediate-release tablet, take 100 mg tablet by mouth twice daily for 3 days, and may increase to take 100 mg tablet by mouth three times daily.
1. I really enjoy this statement, because the determining factor of continuing medical treatment or hospice is really based on the priority of the patient. In Sara's case, her priority was trying to beat her cancer at all cost and live a long life for her family and Vivian her newborn daughter. Until the end Sara had the mentality that she would beat her disease, therefore dying was not an option for her even though it was inevitable in her cause there for hospice would not be good for her. In order to qualify for hospice, you need to be aware that you are terminally ill and dying, and have a doctor certify that you have less than 6 months left to live. Once someone comes to the conclusion that there is not much time left, they soon prioritize the memories and time that they have left and leave behind treatment and focus on palliative care instead so they can be pain-free as
Informed Consent is obtained before a surgical procedure and in simple terms it refers to the permission granted by the patient or his guardians to the medical team to conduct a surgery. This consent is important because surgical procedures can often lead to complications and they do involve certain risks, therefore the patient must have complete knowledge of all that he might encounter during or after the procedure. The surgeon or physician who is responsible for conducting the operation usually obtains the consent but all healthcare providers involved in the process also play important roles in this connection. The role of a nurse in the process of informed consent is largely dependent on the nature of the treatment and the laws of the organizations. Kay Ely-Pierce, MA, RN, JD (1999) writes, "The nurse's part in the process of informed consent is variable. If the consent is being sought for a nursing procedure or treatment, it is the nurse's responsibility to provide the material information, assess the patient's decision capacity (ability to understand the information and make a rational decision based thereon), and execute the appropriate documentation of informed consent or refusal." (Family and Community Health, pg. 78)
They spoke with many surgeons and financial institutions to get second opinions and explore their options. After many meetings and phone calls, the options for a low-income family to get the necessary surgery for Julie were minimum. One day, Susan’s friend brought the solution they needed, in the form of information about the Shriner’s Hospitals. Shriner’s Hospitals match children who need expensive or necessary medical care with a sponsor who funds the operation. After contacting Shriners hospitals, filling out the necessary paperwork, and waiting a week, a sponsor was found. They went to meet the sponsor and to thank him. During the meeting, they discovered he had sponsored many children and had asked nothing in return except to meet the families involved. After they left the sponsor, they paid a visit to the hospital for a tour and to set up a date for the procedure. During the visit, they learned that the procedure would involve breaking her femur and placing four rods in the bone to attach an external separator that stretched the bone as it healed. The surgery was scheduled for the following week, Julie was
