Community health nursing is a vocation; striving to improve the health of the community by promoting, stabilizing and protecting the health of individuals, families and identified aggregates. We are able to have the furthest reach when we can isolate a specific deficit of an aggregate through assessment and subsequently the development of a community support program. Community support programs promote health, access to services and self-advocacy.
External communication to patients and their families holds our organization accountable and shows that we are committed to quality care. People tend to trust healthcare organizations that are transparent with their reports, both positive or negative. Examining points of weakness will help our team members evaluate their processes and perhaps redirect their efforts. Units that succeed in meeting the organizational goal for a certain period of time, will be recognized and hopefully influence other departments to do the same.
I am a native of Rochester, I have lived here since 1964. My experience/focus on community collaboration has led me to many volunteer opportunities. My passion for outreach allowed me to start Imagine Me Community Outreach Inc. in 2014. As the Executive Director for Imagine Me Community Outreach, I am responsible for the everyday operations of the organization; I create the programs, research grants, and recruit volunteers. Our organization is unique because 65% of our work is performed outside of the office, making real connections with individuals looking to change their lives. Introduction- To be effective, a modern nursing must balance patient care versus staffing; procedures versus patient load; egos versus patient need; and, unfortunately fiscal budgeting versus appropriate care. The modern nurse must realize that there position is one of continual multiple horizontal priorities, and act accordingly. As managers, regardless of the subfield, we will be expected not only to understand the organization's budgetary concerns, but to actively manage them as well (Lumby and Picone, 2000). By the very definition of nursing, however, the contemporary nurse must be one of the focal points towards overall healthcare and maintenance for the community. Much like regular maintenance on a vehicle, if nurses can lead a health promotion activity that promotes wellness, over time there is a far greater chance of diminishing high healthcare and emergency room costs. By educating the population about risk management and proper care, then, health goals are more likely to be met.
Street Outreach is one of the many programs GMC offers to the homeless community it serves. Several times a week, the outreach team, made up of one or two GMC staff members, a psychiatrist or a medical doctor go into the street to provide medical care, and create beneficial relationships between the center and the individuals. Recently, GMC added a spiritual aspect to the street outreach program, where a clergy or member of the congregation accompanies the team in an effort to promote spiritual growth amongst the consumers.
Community Engagement Specialists are task with contacting Medicare and Medicaid recipients, educating them on their current medical plan and explaining their plan options. Out touch points with the members include the process of questioning the members needs to determine their concerns. Having member information as a knowledge base provides the foundation of the initial conversation. I would be unable to converse with the member without the members basic information. A further problem that often arises in service-learning courses is found in both the reluctance and inability of some community organizations to initiate contact with institutions participating in community-engaged programs. According to Eli Goldblatt in his article “Alinsky’s Reveille: A Community-Organizing Model for Neighborhood-Based Literacy Projects,” a majority of service-learning programs are “heavily oriented toward student- and faculty-based outreach into underserved communities,” therefore, “we seldom hear of community-based learning projects initiated by community partners themselves” (275).
