Around 3:30 worker received a call from Kim Graves, admissions coordinator. She stated she was at Devita Dialysis and was in the process of getting Mr. Smith back to the nursing home. She asked worker to visit the dialysis center and talk with Mr. Smith.
Worker arrived at the center and Mr. Smith was finishing dialysis. He was not very alert or oriented inside of the building. Due to this worker called supervisor Matthew Mitchell and asked if a court order to the NH or hospital was possible this late in the day. Mr. Mitchell called attorney John Trice and learned all of the Judges were gone for the day.

Trey Cavalier was assigned to M&N control for the night. That morning (which was May 14, 2016 because he was working nights) about 30 minutes to an hour (he could not give exact times because log books were being picked up for the investigation) before 4:00 a.m. Dy. Cavalier was notified by Dy. W. Benson that Brian Ducre (who was a new move that was escorted to M&N on a wheel chair) was acting strange and that medical needed to see him. Dy. Cavalier called the medical department and spoke with Nurse Minor and advised him of the situation. Shortly after Nurse Minor came off of N01 and said that inmate Ducre had mental problems and wasn’t taking his medicine, that there was nothing the Nurse could do for Ducre and that he would ultimately be okay. He reports he contacted her father concerning the patient residing within the family home, but the father has said no. In addition, he reports he has attempted to contact her uncle but has been unable to make contact with him. He reports he has attempted to make contact with her said friend who is considering allowing her the opportunity to reside with her but she has not answered her phone and he has been unable to leave a voice message. He reports no one wants her in their home, and the patient has “burned her bridges” with family members. He reports her family would benefit from counseling. On 11/17/2017 I received a phone message from Dr. Mendelson’s office as a reminder that Mrs. Raney is scheduled for an appointment on 11/21/17 at 2:50p. I recontacted this office as I had her scheduled for an earlier time. The staff rechecked the physician’s schedule for that day and I was told that she could be seen at 2p. On this date worker spoke with Devita Dialysis and Fresenius Medical Center for the purpose of linking Mr. Bagwell with a dialysis appointment that would work with Class Trans transportation to and from dialysis. Neither place had an opening that would work with Class Trans schedule. He was placed on a waiting list at Devita.
