On Friday, 12/25/2015, Cpl. T. Jones and I were working uniformed patrol in a marked black and white police vehicle. At approximately 1340 hours, we responded to a domestic violence call, located at 2040 S. Bon View Ave. While on our way to the location of the call, the dispatcher said a subject threw the reporting party, Erika Alvarez, down the stairs. Upon arrival, I saw several OPD officers attempting to gain access into the residence. The front gate to the residence was locked and the front door, which was located approximately 7 feet behind the locked gate, was open. Alvarez said the argument was over she not being able to find the key to her vehicle and not wanting to take Hernandez to a park. Alvarez said Hernandez kept arguing and started to walk down the stairs. When she tried to pass him on the right side, walking down the stairs. Alvarez said Hernandez elbowed her on the left side of her back. As a result, she lost her balance and almost fell to the ground. Alvarez said she had to hold on to the wall, located down the stairs, to prevent her from falling. Alvarez did not have any visible injuries and did not complain of pain. Alvarez said she has an active (Expiration of restraining order: 03/28/2017) restraining order against Hernandez. The restraining order mandates Hernandez to stay 100 yards away from Hernandez. I asked Alvarez why Hernandez was staying at her residence. Alvarez said she felt sorry for him, and let him stay in her residence for the last two days. Alvarez said she would not let Hernandez to come back to the residence because he has not changed his behavior. Alvarez said Hernandez was calling her names such as, “Bitch, whore, and skunk.” Alvarez said she and Hernandez have been boyfriend and girlfriend for approximately 2 years and they do not have any children. Acevedo said he was sleeping in his room. He said his room is located on the second floor of the residence. Acevedo said he woke up to a noise, which was similar to someone falling down the stairs. Acevedo said he came out of his room and saw Alvarez and Hernandez arguing over a lost car key. Acevedo also said the argument was over Alvarez not wanting to take Hernandez to a park. Back on October 3, 1974 at approximately 10:45pm two Memphis police officers were on patrol and received a complaint from dispatch to respond to a burglary in progress (Tennessee v. Garner, 1986). Officers Leslie Wright and Elton Hymon took the call and responded to the location in question (Tennessee v. Garner, 1986). After arriving, a neighbor told the two officers she heard glass breaking next door and “they” were breaking into the home (Tennessee v. Garner, 1986). On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
I notified dispatch that Officer Albert and I were out at the RSC. I parked our patrol car on the north side of the RSC and entered the north door. I heard Officer Smith on the radio say he was out with the subject near the Credit Union. I observed from across the RSC Officer J. Smith and Officer R. Faison standing next to a black male all the way by the Book Store. As I walked closer, I could see the black male with both of his hands up in the air and he was getting down on both of his knees. When I arrived on scene, I contacted the caller identified as Jordan Lundy. Lundy stated he was with his girlfriend and they went to the dispatched address to speak with her cousin. Lundy was inside the residence where a female confronted him. The female was identified as Alesha McCool. Lundy stated McCool was his girlfriend's cousin. Lundy said McCool began yelling at him calling him a thief and other names. Lundy let McCool yell at him because he wanted to amend the relationship with McCool. Once McCool stopped yelling, Lundy said he turned around and walked out of the house. Lundy advised McCool followed him out of the house continuing to yell at him. Lundy reached the street and he told McCool "Fuck you" but she continued to follow him into the street. McCool approached Lundy in the street where she pushed him with both hands. Lundy informed me he was pushed so hard he "flew" backwards but stayed on his feet. Since Lundy stayed on his feet, he stated McCool walked up to him and slapped him across the face. On 11/02/2015, I, Chad Agnew, was working as a patrol officer for the Wichita State University Police, Wichita, Sedgwick County, Kansas. At approximetely 2222 hours we went to Omega Ct. to assist Sgt. Garwood since he was out on a suspicous character and another call on Omega Ct. Sgt. Garwood advised us to go to the Delta Delta Delta house at the end of the block and speak to the reporting party. I went and made contact with Julie Nolte and Dakotah Gilmore who were inside the residence. We went to a back room and closed the door.
On Monday, 11/30/2015 at approximately 1755 hours, Officer Lauritzen and I responded to intersection of Holt Blvd and Plum Avenue to assist Sergeant Ortiz to subdue a suspect who was fighting him. Upon arrival, I saw other OPD officers trying to handcuff the suspect, later identified as Kevin Larry. Larry was on his stomach and he was yelling that he did not do anything. Larry was resisting arrest by turning his hip side to side and was trying to pull his hands to the front of his body. At approximately 1505 hours, I interviewed Inmate Hernandez. I read him his Miranda Rights from my MCSO issued Miranda Rights card. I asked if he understood his rights as they were read to him and he stated, "Yes." I then asked him if he would voluntarily answer my questions and he stated, "No." The interview ended at that time.
The reporting party (RP) stated resident Sofia Miramontes age 27 DOB: 11/23/88 has developmental delays, deaf in right ear and is bipolar. The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety. The Officer informed the Director of agency Sherry Winston that the resident was coherent and walking to her boyfriend’s home. The RP contacted the facility and spoke with a staff
I responded to 410 South Brundidge Street in reference to a suspicious circumstance call. Upon arrival, I spoke with Michael Peacock. Peacock advised that he heard a loud "boom" and someone going through the residence. Peacock stated that he just thought it was one of his roommates. Peacock stated that someone started kicking his room door and he grabbed his knife and ran to the bathroom. While in the bathroom, Peacock called 9-1-1 and locked the door. Officer Roman and I were in full police uniform and driving a marked patrol vehicle when we responded. On Sunday, December 4, 2016 at 10:30 pm, I, Sergeant Bever responded to a man with a knife call at 5221 Rue St Germain. I spoke to Janice Johnson outside when I arrived on-scene. Janice said her son, Chadd Johnson was inside the house and had a knife. I went inside and Officer Etheridge and Officer Hunter were attempting to speak to Chadd, who barricaded himself in a bathroom. Chadd would not listen to Officer requests for him to exit the bathroom. I went outside and looked through a window into the bathroom. I could see Chadd standing and talking on the phone to someone. Shortly afterward, Chadd set the phone down, grabbed a razor blade and started to cut one of his wrists. I then ordered officers to forcibly make entry into the bathroom and
