On 11/02/2015, I, Chad Agnew, was working as a patrol officer for the Wichita State University Police, Wichita, Sedgwick County, Kansas. At approximetely 2222 hours we went to Omega Ct. to assist Sgt. Garwood since he was out on a suspicous character and another call on Omega Ct. Sgt. Garwood advised us to go to the Delta Delta Delta house at the end of the block and speak to the reporting party. I went and made contact with Julie Nolte and Dakotah Gilmore who were inside the residence. We went to a back room and closed the door. I spoke with Julie first. Julie told me that they had a member of Delta Delta Delta who was suicidal. She told me the member's name is Pharasa Thanasith and that she goes by her middle name Paulina. Julie explained that Paulina has been depressed for awhile and she thought she was getting help but realized she has just been hiding it very well. He said that his friend, Jesse Oliver recieved a phone call from Paulina on 11-2-15 at approximetely 0020 hours. Dakotah said that he was with Jesse in Dakotah's dorm room when he got the phone call from Paulina. Jesse got off the phone with Paulina and told Dakotah that they needed to go over to the Delta Delta Delta house because Paulina said she cut herself and she was bleeding and she couldn't get it to stop. Dakotah said they arrived at the Delta Delta Delta house approximetely 0030 hours. I asked Dakotah if Paulina said anything to him while he was there. He said that she didn't talk to him alot. He said that he physically cleaned up her arm and did basic first aid. He said there were about seven cuts on her left arm. He also said they were deep but he didn't think they needed stitches. CW went to 1261 Everton Dr, Apt 9 Akron, OH 44307 because they were called out by Akron Police Department (APD) for allegations of abuse towards 2 children. CW Jonathon Henry and CW Sonya Cole pulled up to the apartment complex and observed that there were over 15 people sitting outside. Both CW could tell that this was the correct address because there were 3 police cars that were sitting outside. As the CW's were walking towards the apartment complex an APD officer asked if the two workers were from CSB. CW Cold responded that they were. The officer then told CW's that there were not 2 children, however, there was 3. The officer stated that there was one child in the bedroom sleeping that they did not know was in the home. CW's were also told by the officer that Mr. Brian Burrell (AP) was in the home and that he was very agitated. The officer told the CW that they had called him down, however, suspected that he would get upset after entering the home. The officer also said that he already knew that the kids were in the custody of CSB and that he would most likely not be happy that the two CW's were at the home. The officer the led both the CW upstairs to apartment 9.
On 010/05/2016, I, Chad Agnew, was working as a patrol officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately1415 hours, officers were dispatched to the Rhatigan Student Center for a suspicious character. Dispatch notified all officers that they were still gathering more information at the time but the suspicious character was a black male with crutches. They also said the subject was yelling someone had a gun.
On 05-11-16 at approximately 1800 hours, I was dispatched to a concerned citizen in the justice training center neighborhood. I arrived at 1810 hours and a white male adult with a mustache approached me, later identified as W01-Walker. He said his neighbor was being a nuisance because the couple living there are always arguing. W01-Walker said, “It is a necessity that you go talk to my neighbors” and “this neighborhood was livable before they moved in.” I radioed dispatch that there was a possible domestic violence dispute next door and told W01-Walker to stay inside. My partner arrived and we went next door.
Whilew on patrol I was dispatched room # 1F340, Upon arrival, I met with Hackett who stated that Veteran Susan Buggell had been sexual assault. Buggell was located in room #1F337, where I made contacted with her. Buggell stated that between 0300hrs, and 0400hrs, while at a 1211 old FM 440RD Apt #1-202 Killeen TX, 76549 she was sexually assaulted be a friend, who she identified as Anthony Williams. I asked the Buggell if she had taken a shower and she stated “no”. “I did change my clothes.” I then asked were her closed were at and she stated at her home. On 26 June 2017 at 1516 hours, Officers were dispatched to the area of 305 East Penn Street for a theft of a vehicle. Officer were advised the actor was a Rhiannon Meyer and that she had stolen her dad Dodge truck green in color with a plow on the front. and was last seen heading towards Main Street. Officer began to check the area of Main Street, when Unit 191 called out with the vehicle at 100 Pennsylvania Ave, in Shenandoah Heights. Your Affiant (Patrolman William Moyer) along with Patrolman Tyler Dissinger responded to Unit 191 (Chief Livergood) location. At arrival Chief Livergood had MEYER in custody along with the stolen vehicle. MEYER'S was still yelling about someone having a gun in the window of the residence and was not making sense

At approximately 1909 hours I was dispatched to Shocker Hall in regards to a Suspicious Character call. Dispatch advised there was a male which attempeted to get into Shocker Hall. He was described as a white male, blue jeans, gray sweat shirt, and had a green dwarl string backpack.
On 08/05/2016, I, Dillon Dickerson, badge #155, was working as a Patrol Officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately 1116 hours I was dispatched to Corbin room 156A to take a report of a lost or stolen wallet. I was riding at this time as a two man unit with Officer Faison #116. We arrived at Corbin at 1117 hours and made contact with the reporting party.
I arrived on scene at 01:20 hours at 418 George Street, Gorham, IL 62940 and seen Deputy Jarrod Kreid #2302 speaking with a female identified as Misty L. Wright (F/W, DOB: 04/09/1978). Wright was standing beside her vehicle, parked in the driveway at the above address. I located Haug standing across the street. I walked over to Haug and asked him to explain what happened.
On 10/02/2016, at 0431 hours your Affiants were dispatched to 11 1/2 West Oak Street where the caller, David Grady, reported his ex-girlfriend broke into his home and was not authorized to do so. Grady indicated he had left his home on 10/01/2016, at approximately 1545 hours, to attend a class reunion and afterwards went out with friends. While at a friend's home his ex-girlfriend was there and asked if she could come over. He replied maybe later and that he would let her know. Shortly later she left his friend's home and he began receiving text messages from her which he showed Officers the messages. She messaged him saying his door was locked and he replied I know. On February 26th, I, Officer Albert, was working as a Police Officer for the Wichita State University Police Department. At approximately 0213 hours I was dispatched to Lot 3 on the campus of Wichita State University for a report of a theft. I arrived in Lot 3 at approximatley 0216 hours and met with the calling party Brandon Charles Feik. I introduced myself to Feik and asked Feik for his personal information. I asked Feik to tell me about the theft that occured, Feik stated he had a bunch of his belongings in his car after removing them from his girlfriends apartment earlier that evening. Feik said he noticed he was missing a black duffle bag that was laying in his front passenger seat after leaving Kwik Shop at 4802 N. 21st St. N. and arriving back onto campus in Lot 3.
On Tuesday 06/20/17 at 1158 hours I was dispatched to a physical domestic which occurred in the 2800 block of D ST SE in the City of Auburn, King CO, WA. Dispatch stated the reporting person was a school bus driver, Tamie Scholter, but she had relayed the information through her transportation center. Dispatch stated a male and female were in a physical fight on D ST SE near D PL SE, and that a male had pulled a female's hair.

At approximately 1400hrs on Friday, March 18,2016 I arrived at 7810 Shaver Rd for my internship with the Portage Department of Public Safety. When I arrived I went and put on my vest and then met with Officer.Burck who was just finishing up his lunch at 1409 hrs we left the station and then headed over to 10114 harris rd to a 911 call hang up with no answer to a call back by dispatch we knew a 82 year old male was living there with his son so we went to make sure everything was ok. We arrived at 1428 hrs and or knocked on the door no one answered the door so we got a hold of the son and he gave us the the password to the garage door the officers went in and then came out and said everything was ok he just misdialed. We left and then responded to a full arrest at 1446 hrs. Which is someone who has stopped breathing fully The paramedics on scene couldn't bring him back so they pronounced him dead. The death investigation would take awhile so a sergeant came and picked me up and took me to Officer Houser. While with him we rode around his district and did traffic until around 1710 hrs and then he brought me back to the station and then I left the station around 1720
R/Os responded to the listed location in reference to a check welfare. Comp. called the PD and stated that she was concern about her grandaughter. Comp. does not live close by and her son had not answered phone calls all day. Upon my arrival I made contact with the father of the child who stated that he lost track of his phone and just realized that his mom had been calling. R/Os made sure that the daughter was safe. On April 05, 2016 at approximately 1518 hours, I was dispatch to 2004 Pine Street in reference to a death investigation. Upon my arrival, I met with Detective Campbell who was the lead Investigator at the scene. Detective Campbell stated that the victim neighbors reported a strong smell coming from the victim’s (Cortiz Lazarz) residence. The neighbors called 911 and Officer Volk was the first responded officer on scene. Defective Campbell and officer Volk stated that the victim located on the living room floor, fazing the south from the residence. Detective Campbell requested I process the scene for evidence and documentation.
