CSP, MHS and Dnyshia discussed recent relationship and demonstration of healthy boundaries. Response: MHS explained Dnyshia is her baby and the youth does not talk much. FS arrived to (client) Quinton Garrett's home to discuss and improve Quinton's behavior and progression towards his goals. FS explained to mom that the week approaching is the last week of camp. FS also reminded mom of Quinton's behavior during camp. FS informed mom that services are not nearly as effective if she doesn’t implement structure in the home as advised. D-The patient arrived late to her counseling session. Reported stability on her current dose and denies the need for a dose increase when offered by this writer. This writer addressed with the patient about her attitude about learning about her results yesterday as she appeared to be in a hurry and nonchalant about the result. According to the patient, he was affected by the news and says, " I had left over cocaine and threw it away.....I am done with that stuff." The patient updated this writer on her plans to change her mental health provider from CHR to the Hope Center as the patient dislike her psychiatrist at CHR. Case management services were provided through a face to face interaction in Santa Ana. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo and the youth. WYP Ngo checked in with the youth’s father. The youth is behaving well this morning. The father promised he will play with the youth after the one on one session. The father is concerned about the check for daycare. WYP Ngo will update the father as soon as possible about the check. WYP Ngo took the youth to get his incentive (frozen yogurt). The youth liked the treat. WYP Ngo reminded the youth to work hard toward his incentives. The youth wants to work for shaved snow for the next incentives. The youth had trouble with accepting no as an answer. WYP Ngo ignored the youth
R: MHS arrived today and noticed member was very depressed. MHS inquired on reason f or members current mood “I just have a lot on my mind lately,” states member. MHS encourages member to express her concerns and the things that seem to be bothering her. Member informed MHS that she has been very depressed and considering back home with her dad. Member states the reason for her depression at this time is due to the fact that she feels home sick and feels that moving out of the house with her dad was a very poor decision.
Intervention: CSP and Reighn discussed ways the youth is expressing her feelings at home and school. CSP assisted Reighn in processing her thoughts about upcoming court date and adjusting to having another youth in the home. CSP addressed MHS concerns the youth behaviors are regressing.
A, B), A, B) MSTT met with Wilbur and Quaiesha to examine how they control their anger in a stressful situation. MSTT was able to have Wilbur examine his triggers and emotions during those stressful times when he and Quaiesha engage in verbal altercations. The subject then changes because MSTT was informed Quaiesha went into her father room without permission on two separate occasions after we spoke about it just a few weeks ago. When the clinical team arrived Astrid was in her room. Astrid walked out her room, pass the clinical team, two times without greeting the clinical team or engaging in eye contact. Mom called Astrid over to join the clinical team at the dining table. Astrid did not comply, it took two verbal prompts for Astrid to sit at the table. Astrid sat across the clinical team with her head looking down into her lap. The clinical team probed greetings and Astrid did not respond. The clinical team presented Astrid with some preferred activities, such as coloring and drawing. These were the only activities Astrid and the clinical team engaged in until the end of the assessment. Astrid's communication were probed. Compliance was probed first. Astrid did not complied with the one step instruction, look at me, nor did she talk unless reinforcer was removed. Even when the reinforcers where moved, Astrid talked in a low voice, similar to a wisper. It wasn't until the clinical team told a few sarcastic jokes and acted silly did she engage.

Addressing the tears- The patient reported that her ex-boyfriend Steve or Steven from this clinic, patient did not disclose his last name returned back to this clinic and she's upset because she had asked him yesterday to not return to this clinic. She then reported, he cursed at her in the lobby area and embarrassed her. Patient reported, she walked away in tears and waited for this writer to get her. Suggestions were discussed about how the patient should handle the matter moving forward and how she needs to not allow negativity in her life and avoid engaging in any sort of relationship with patient
Worker attended a psychiatric appointment for Zoey Emery, Dr Sultana. Sultana suggested that Zoey stay out of school to attend touchette for monitoring medication and outpatient. Sultana could not do a evaluation because it would be bias due to Zoey being her client. Sultana made notice of Zoey's erratic behavior. Worker stated that Zoey's highs and lows have slowed down since the release for the hospital . Worker wanted the cuts to be noted. Worker is not around Zoey 24 hours like Fran. Fran states that Zoey is low, not listening, and acting out. Sultana believes the behaviors are because of teenage attitudes. Sultana will not adjust medication. Sultana suggested that Fran received counseling to deal with Zoey's behaviors. Sultana questions
Her statement coincided with her affect during a session. When discussing her complex trauma she presented emotions of anger. Samantha would become red, spoke rapidly with a high pitch, shook uncontrollably and gave detailed stories. However, when she spoke about her mother and son, Samantha was hesitant about the information she shared. At the beginning, she would pause and her affect became flat, her eyes would water up, she had a soft voice, and her body would freeze. The information Samantha disclosed about her mother and son was fragmented with missing information that she had no recognition of. She was only able to discuss about her mother and son to a certain point before she became disengaged from the conversation. Samantha reported
Destiny is getting more aggressive after seeing her DSS Christmas presents at her mother’s home and the mother refuse to give her the presents. Destiny told Ms. Beaton it was unfair. Destiny state she know there is no schedule visit this weekend. Ms. Beaton express this has cause more problems because the other youth has her presents.
Action: MHS reported Liam medical condition is stable. MHS explained Liam has a cold and not feeling well for the past few days. Liam was less alert and active today. MHP obverse Liam walking without assistance. MHS placed Liam in the highchair to eat breakfast. MHS serve Liam cheese girts with apple juice. Liam is eating more with his mouth close. When Liam is not eating Liam’s tongue sits on the top of his bottom lip. Liam was turning around in the highchair trying to get out. MHS explained Liam knowns how to get out of the highchair. MHS provided paperwork of therapy
Therapist met with individual mother to discuss progression or regression of individual. Individual mom states that individual was in trouble for being disrespectful toward his teacher. Therapist informed individual mom that she will work with individual on positive communication skills to assist individual with positive response toward authority figures. Individual mom states she will continue to make sure individual uses the skills to help him do better with respecting authority figures.
