Bangor Family Physicians is a partner based medical group practice located in Maine. The practice consists of four family practice physicians, and a medical support staff. The medical support staff is made up of a practice manager, two receptionists, four nurses, two medical assistants, two billing clerks, and a laboratory technician. Additionally, Bangor Family Physicians employs a CPA to assist with taxes and financial advising. The key stakeholders are the four family physician partners, in which each physician holds an equal stake in the practice.

Apart from her academic accomplishments, Dr. Ochoa has dedicated her life to the service of others through medicine and mentorship. “I’ve known since I could remember that I wanted a career with the purpose of helping others. I found the need to reinvent HUDA Clinic as a whole, which entailed adapting its policies to the changing landscape of healthcare. I learned quickly that I’d need to broaden HUDA’s vision, in that a solely primary care clinic was no longer the answer. Rather, HUDA would need to offer those services that continue to remain inaccessible and unaffordable. Working alongside physicians, I was able to expand our dental clinic, and introduced psychiatry and podiatry services. I recruited more volunteer physicians, enabling us to expand our services. Today, the organization is able to serve a considerably larger underserved and underinsured population.

The current health care sector is too costly and too fragmented with a lot of variation in care even with established evidence based guidelines. Providers lack the tools, support and information they need to offer the coordinated health management that can reduce cost and improve outcomes. Primary Care Physicians are constrained in their abilities to perform any proactive care that involves avoiding Hospital or ER visits, and influencing healthy lifestyles.

Mobile Integrated Healthcare, Community Paramedic (MIH/CP) is a nationally recognized program where local Emergency Medical Services (EMS) have become involved in providing patient-centered care in partnership with local resources such as social services and primary care (Mobile Integrated Healthcare and Community Paramedicine (MIH-CP): A National Survey, 2015). These health care professionals now compose a team of health care providers that are essential in a patient’s over all health care. The health care system must change to improve our nation’s health and takes strong steps to address the unsustainable growth of health care costs in America. We still have a long way to go before our health system become effective. We still have population that do not have insurance, have difficulties accessing their health care, or their needs are not met within the healthcare system. I was nine years old when my family made the stannous decision to come to America; with only a few clothes and barely any money, the pursuit of the American dream seemed more like a fantasy than anything else.
