LR is an 43-year-old moderately obese African-American woman from Gautier, MS. She history of keloids and recently diagnosed in March with another keloid. I chose this case because I felt her diagnosis was interesting because we had just learned about this disease in class. When I first learned that my clinical site had a keloid case I was excited to learn about the planning process. Keloids are a benign condition that is result of an overgrowth of fibrous tissue that usually develops after healing of a skin injury. The tissue continues to grow extends beyond the borders of the original wound. They can appear anywhere on the body, but usually form on the shoulders, head and neck. They can last for years and sometimes don't form until months or years after the initial injury. It can happen to anybody, but is more common in people with dark skin, such as people from African, African-Caribbean and south Indian communities. It's thought that keloid scarring may run in families. New keloid scars are sometimes red or purple. They're not usually painful, but some people feel a tingling or itchy sensation. The goals and side effects such irritation within treatment field, dry or moist peeling, possible blistering, telangiectasis, and hyperpigmentation of area were discussed. The early onset of these symptoms due to the combination of the treatments were also reviewed with the patient. Surgery was performed on April 8, followed by radiation therapy on April 9. Treatment dose includes using an electron block cutout, using an energy of 12E along with a 1.5 cm bolus. She was prescribed 3fx at 400 cGy per fraction with a total dose of 1200 cGy.
