Services are provided to individuals who wish to live in a group home setting, need significant assistance with activities of daily living, are 18 years or older, have a diagnosis of intellectual disability receive case management services through Henrico Area Mental Health & Developmental Services, or another CSB/BHA. Location/Days/Hours: Four group homes provide support and housing to a total of 20 individuals. Homes are located throughout Henrico County. Homes provide services 365 days a year. Individual residents are encouraged to develop and maintain friendships and may come and go from their group home, as they wish, with appropriate supervision. The homes are open as needed throughout holidays to provide support to individuals
On this date worker received a call from Tori Pettit. Ms. Pettit stated that Serv-Pro had visited Ms. Smith's home did an assessment and sent off the mold samples. The results came back that the house is not livable due to mold infestation. The mold is in the walls, ceiling, basement, and air vents. Ms. Pettis has spoken with Ms. Smith and Ms. Rachel Rodgers about Ms. Smith moving ASAP. Ms. Pettit stated that Tri-County could place Ms. Smith into a group home now due to her home being deemed unlivable. However, Ms. Smith is not interested in-group home placement and would rather live independently. Ms. Rachel Rodgers has agreed to assist Ms. Smith with apartment applications and any other thing need in order for MS. Smith to find residence.
Howard Terry Newman is 67 year old retired printer who suffered from Parkinson’s disease as well as arthritis, voice impairment, hiatal hernia, and heart trouble. Mr. Newman, the appellee, is contesting his stay between September 19, 1968 and November 11, 1968 at the appellant’s facility, Big Town Nursing Home. The plaintiff was admitted voluntarily by his nephew to the facility with the ability to leave as he pleased. However, on September 22nd, when Mr. Newman attempted to leave the facility, he was told that he could not use the phone or entertain visitors without the administrator’s permission. He proceeded to walk out; he was brought back and restrained on Wing 3 with “senile patients, drug addicts, alcoholics, mentally disturbed, incorrigibles and uncontrollables” according to the administrator testimony.

Wesley Glen Services are offered to adults 18 years of age or older, “Wesley Glen owns and maintains ten group homes and 1 apartment in Macon, one home in St. Marys, a twelve-unit apartment complex in Douglas, two homes in Lyons, two homes in Jesup, and 4 host homes. For the current residents in Macon and St. Marys according to,” (Wesleyglenministries.com, 2017).
A well-managed group living service will strive to be as inclusive as possible when it comes to making decisions about the physical environment. For example the cooker at our home broke down and we included all the service users in the decision of which cooker to buy, we also asked the opinions of our support workers and an occupational therapist along with family members.
Finding permanent supportive housing for single adults and families living with special needs, including chronic homelessness, HIV/AIDS and mental illness, as well as young adults aging out of the foster care system is an issue relevant to social workers. I would like to create an organization that provides counseling and supportive services to help clients stabilize their lives, maintain their housing stability and advance towards their individual goals, including financial independence. The organization’s services are comprehensive. We will provide person-centered case management, employment services, mental health treatment, including onsite psychiatric treatment at certain sites, health and wellness programming, nutrition coaching, food service, independent living planning and community referrals. On this date worker visited Ms. Brooke Henry at Pine Place Assisted Living Facility in Arley, Al, for the purpose of arranging services for Ms. Henry. Ms. Henry needed transportation to the SS office, Wal-Mart, group home in Curry, and to get her haircut. When worker arrived at the facility, Ms. Henry was sitting on the couch in the living room. She was appropriately dressed with good personal hygiene. She was very excited to see the worker and about leaving the facility for the day.

Heart Haven’s mission is to empower, follow core values of respect, integrity, empathy and accountability.
