This article gave us information on Healthy People 2020, which was launched on December 2, 2010 by the U.S. Department of Health and Human Services. The article states that Healthy People 2020 is a strategic approach by the federal government, states, communities, and many other public and private partners to improve the health of the US population. This plans states that improvement will be completed over a ten-year time span. The Healthy People strategy was designed to define and promote a common sense of purpose and goals. These goals include: attaining a longer high quality live free of preventable disease, disability, injury, and premature death, achieving health equity, eliminating disparities, and improving the health of all groups,

Healthy People 2020 is a promotion to help raise public awareness by providing measureable objectives and goals for individuals to obtain higher qualities of lives, decrease premature deaths, and promote a higher quality of life (US Department of Health and Human Services, 2014). Healthy People 2020 has monitored its progress for over three decades between communities over the nation to understand where prevention measures are needed most. Due to the accomplishments that has been achieved in the previous years under this program, the Healthy People 2020 expanded or rather broadened their goals and objectives beyond disease prevention, health promotion and reducing health disparities, but to also eliminate healthcare disparities that has plagued this nation by the year 2020. Healthy People 2020 was released in December 2010 to help the United States practice healthy lifestyles and learn to live a long healthy life. It is a set of goals that are designed to guide health promotion and disease prevention nationwide. These guidelines are set for a whole decade with the results being available after 2020. The vision and mission statement along with the goals of Health People 2020 are available on the website (healthypeople.gov) making it user friendly for everyone.

According to the Institute of Medicine (IOM) (2011), Healthy People has changed during the three decades of its existence. The mission of Healthy People 2020 is to: Determine healthcare priorities; Arrange for objectives and goals to be attainable at all levels; Classify research evaluation and data collection requirements; Increase community awareness and knowledge concerning healthy habits, illness, disability, and opportunities for advancement; in addition, Engage various divisions to strengthen guidelines and advance practices by using understanding and evidence-based practices.

Healthy people 2020 “ establishes national goals and objectives for policy, programs, and activities to address the major health challenges facing our country today, and seeks to assure conditions in which people can be healthy, both now and for generations to come” (Secretary Advisory Committee, 2010, pg., 2). This initiative helps supports people on their health and wellbeing. It strives to identify potential health risks and help support improvement priorities. Healthy people 2020 have elaborated with different sectors to strengthen these policies by proving goals and objectives.

But I think that a program that are targeted for a specific group or specific communities have more possibility of working. The health program that I think would work would include discussions on healthy eating and being active also professionals that would have lectures on why is so important to visit the doctor. I think having discussions as a community on the importance of having a higher education and explaining how everything that we do as people can affect our health. We already have great health programs but we should create more help for people that don’t have health insurance and people living in poverty levels. People who have it hardest in this country are those with the lowest incomes and they also have the worst health. Many racial minorities in the US suffer from poor health, but is not because they have bad genes, rather because they don’t have the education. My practicum cite, Department of Health, White River Junction, Vermont is a local public health agency. It serves part of Windsor County and part of Orange County. For this assignment I chose to elaborate 3 of the public health services provided by my practicum cite - monitoring and evaluation of the community to identify community health problems (Assessment), mobilizing community partnerships to identify and solve health problems (Policy Development), and linking people to needed health services and assure the provision of health care when otherwise unavailable (Assurance).

A hospital in San Francisco has a community based program called the Tenderloin Health Improvement Partnership that was founded by the St. Francis Foundation. The program focuses on increasing job opportunities, public safety and reducing gang violence. The key factors of this programs is that the population-health management model provides rewards to health care providers for keeping large numbers of enrolled patients in their program and the program provides community health initiatives such as safety within the neighborhoods, nutrition and housing. This is done through three system wide initiatives which are providing grants to the community to make awareness of the causes of chronic health issues, such as obesity, the program invests in infrastructure, and provides social innovation grants that promotes entrepreneurship within in low income

When conducting a needs assessment, people’s needs with in the community are assessed to see where gaps lie and improvements can be made. If carried out and reported correctly these needs are then bought to local government or relevant organisations/peoples attention, giving the opportunity for programs to be put in place to benefit the people of the community. This is where the third step of Finifter, Jensen, Wilson & Koenig (2005) process comes into place; the implementation of projects to assist where the gaps were found. “Implementation of recommended solutions should be the ultimate goal of any project to improve services and quality of life of a target population (Neely, T R).” The more efficient all three steps are carried out the stronger this method becomes. “The importance of disseminating results effectively using these guidelines cannot be stressed enough. A needs assessment will have achieved nothing if the results are not communicated effectively (Neely, T R).”
