This made me nervous as to how I was going to communicate with her. My mentor stood back in order to allow me to administer the medications and I felt unable to ask the question, “How do I communicate with this patient?”
On 6/30/2016, CM met with the client to complete to Bi-Weekly ILP Review. In the meeting, client was dressed appropriately for the weather. She was very loquacious and client. Client appears to have difficulty sustaining attention, client does not seem to listen when spoken to directly and she is unable to follow through on tasks. Client affect is inappropriate and she denied suicidal or homicidal ideation.
Intervention: MHS reviewed the previous session. MHS discussed with client about her disrespectful behavior at school. The client’s teacher reported the client has not been doing her work and she has been being very disrespect to adults at school. MHS explained to the client that she will have to learn how to have respect adult at home and at school. MHS discussed listening and obeying and following the rules without any negative behaviors like talking back to teachers and school workers. MHS explained how people don't like to be around rude and disrespectful children and her behavior can be affecting her grades

CM Ayfer met the client at his home in Somerville, Client showed CM his apartment and his room it was clean and tidy. Client is a student at Banker Hill Community College and he has 4 classes during the week, Client reported that he has a difficulty with the Math classes, CM suggested maybe it will be helpful if we find a tutor so he can get an extra support. For this research paper my client will be known as H.C. This particular client has suffered from multiple diagnoses during his time at Sonoma Developmental Center (SDC). My client has lived at SDC since 1947, at the age of 13 years old. H.C. willingly came to SDC when his family could no longer cope with his behaviors. He liked to be independent and was an active worker when he worked at his job sites. H.C. had a bundle of past achievements that he enjoyed mentioning to his aides by pointing at pictures. At a frail age H.C. could do his activities of daily living, but needs assistance from time to time. Physically H.C. was a short man with balding white hair and had poor eyesight so he wore glasses. His walking gait was failing and his feet would shift against the floor, so he had a walker that he could use. He was socially withdrawn and would only communicate when he felt that his needs were pressed. He had trouble finding words to communicate what he wanted to say and would like to do things on his own rather than relying on staff. He also lived by his set schedule, so anytime there was an appointment set for him he needed to be reminded before his day began.
On 03/07/16 we received a broker invoice advising of an incurred increase in the amount of $1,180,064 (1,137,500 Loss & 42,564.14) along with a billing request for $1,269,607 (1,225,000 Loss & 44,606.94 in Lae).
