Our agency assists low income seniors living on fixed incomes with mental health services. Our services include screenings for depression, dementia, and early onset of Alzheimer’s. We also provide mental health counseling services for grieving widows and work towards reconnecting individuals with support services in the community.
Alongside these nursing aids, our organization offers health concierge services to clients in their own comforts at home. Among the services we provide to our clients include Companion Care, which is designed to meet the needs of the elderly, Respite Care for patients recovering from illness, Jet Care geared towards the traveling customers, and Home Care that is focused on home-based care delivery. I have served in Home Care of La Jolla since its creation in 2002. The interest to offer a more customized and a need-based service to special groups in the society is the main reason for concentrating in the long-term care industry.
These groups focus on working through the emotional and sometimes physical dilemmas they may be encountering in reentering modern society after a long hospitalization. They are allowed to return back to the home after 2:30pm, and then they are monitored by counselors, which through modeling and observation teach or tweak basic routine living skills such as clean, cook, upkeep finances, learn transit systems, etc. The counselors also monitor each consumer’s progress and meets with them on a weekly basis to discuss treatment goals. Once a consumer has shown progress and feel they are ready for more responsibility and independence, they can then find employment in place of partial care or move into individual apartments (also provided by Mt. Carmel Guild).
Member stating she was discharged home from Hanover Health and Rehab on 03/14/2017 with home health services in place. Member stating she had requested Encompass Home Health Agency but Bon Secours Home Health came out on 03/18/2017 and she stated she told Nurse she did not want that Agency. CM placed a call to Hanover Health and Rehab and spoke with Discharge Planner and she stated that Encompass could not take the case because of a shortage of staff. Member stated she would agree to have Bon Secours Home Health. Discharge Planner stated she would send the referral back to Bon Secours Home Health. Member stating she had post hospital follow up with PCP yesterday with no new changes. Member stating she reported the pain she is having to PCP
