CHIEF COMPLAINT: Left shoulder and left lower leg injury.
HISTORY OF PRESENT ILLNESS: Ms. Branson is here today for followup evaluation of her left tibial shaft fracture for which she underwent operative stabilization with intramedullary nailing and small medial plate, three weeks ago. She also sustained a grade 3 AC joint separation on the left at the time of injury. She has been attending physical therapy and has shown excellent progress with her range of motion in the shoulder. She remains nonweightbearing on the left lower extremity, as instructed, and is tolerating this well. She has not required any pain medication, other than over the counter anti-inflammatories for the last one to two weeks and feels good with her progress.
I met Mr. Eigner at the office of Dr. Taha. Mr. Eigner reports he is not taking any pain medications at this time. He reports he has a jolting shooting pain to the right and left legs only occasionally. He denied any pain to his right forearm. X-rays taken showed good alignment and healing of the fracture. The incisions are all healed except for a couple small spots on the right ankle. There is some swelling to the right ankle which Dr. Taha said is to be expected. The range of motion to the left ankle and toes was good. The range of motion to the right stores was limited. Dr. Taha said there is scar tissue at times from this type of repair and he would like physical therapy to start working on that. He is still going to be non-weight bearing for another 6 to 8 weeks on the right leg. He is now allowed full weight bearing as tolerated to the left leg. Dr. Taha ordered a rolling scooter to aid with ambulation and stop using the wheelchair. I have contacted Reverence physical therapy and faxed the new orders so the service can begin. I will process the rolled scooter with directions from the adjuster. The attendant care and replacement services will continue through to the next appointment.

Grade 2 injuries result in symptoms of moderate to severe pain with severe difficulty in walking. The athlete is unlikely to be able to play on and will often limp heavily. Minor bruising and swelling may present immediately but can take several hours (up to 48) to develop. The ankle will feel very stiff but may also feel unstable resulting from a number of torn ligament fibers (this can be tested more accurately by a professional therapist). Recovery time for a moderate ankle sprain (grade 2) takes between 4 and 8 weeks.

Christina sent to physical therapy after acute injury two weeks ago. Reportedly slipped on a wet surface went airborne and landed violently on her buttock. I do not believe this was documented but this was what described to me. She notes prior to that she had no real history of any real back issues and was working in the maintenance job. Oddly, she was in the hospital back in early fall and they did a lumbar MRI which is fairly negative. I do not have the results in the chart at this point. Apparently, it was done because she said she had some leg tingling. I am not sure what to make of that. Regardless, she does not report a lot of history and had a high level of function. Stating she would like to work and be active. She states since his acute fall she has been fairly miserable. She notes intense right buttock and right leg numbness and pain. Pain can fluctuate from a 9/10 to a 0 depending on activity and position. Notes increased pain with Valsalva, coughing, or sneezing, but particularly Valsalva maneuvers. HISTORY OF PRESENT ILLNESS: Ruby Pearce follows up today for reevaluation of her left proximal humerus fracture sustained secondary to a fall on July 9, 2015. She was seen in the office on July 15, 2015 and a course of nonoperative treatment for proximal humerus fracture was begun. She was given a prescription for physical therapy and instructed on home exercise program including pendulum motions and wall walking. She has not attended physical therapy, but has been diligent with her home exercise program. Her pain is intermittent and sometimes sharp, but is easily controlled with medications.
