Abstract
Ethics is part of the decision making process that a nurse uses and is a foundation of nursing. Nursing needs ethical standards to rely on in order to provide quality care for patients and to keep them from harm while respecting their wishes (da Silva Santos, Cartaxo, & Buriti 2016). Ethics is defined as a guide, or a code, that given populations use to model behaviors (Finkelman & Kenner 2016). When referring to the nursing profession, ethics are the driving factor when considering what is right and wrong. The goal of ethics in nursing is to provide a means to support the decision making process of the patient. Nursing informatics will be specifically discussed as a resource for the purposes of this paper. Only peer-reviewed articles from professional journals were reviewed. The ethical dilemmas the author will present surround the use of informatics in technology and how they now arise in the medical profession, with nursing in particular. The nursing code of ethics addresses the issues of patient rights. The objective of this synthesis paper is to present my readers what I have achieved during my graduate program at American Sentinel University (ASU). It assimilates all my course work, clinical experience, and nursing practice during this time, which presents a complete picture of how I accomplished my Master of Science in Nursing (MSN) consequences and Nursing Informatics track competencies. This paper will pinpoint my personal philosophy of nursing informatics and give a concise summary of my professional achievements, competencies developed during program of studies, and goals for life-long learning as an informatics nurse. The occupation of nursing is considered as both an art and science. Development of nursing comprises evidence‐based practice and fast integration of advances in technology. It is a field that has extensive uses in healthcare, as well as specialty practice. Educational curriculum and degree/certification are choices for nurses to pursue in this exciting field.
Ethical practice is another component of the social contract of nursing which is a reflection of the values, beliefs and moral principles of the nursing profession. In order to practice as a nurse informatics or NI specialist, one must know and understand the standard to which he or she is held. The purpose and function of scopes and standards of practice for nursing informatics is to provide an overview of the specialty, assist future specialists understand their scope of practice, and practice at a competent level to guide and support nursing care. The scopes and standards of practice further provides an insight into the foundation of clinical decision making processes and cognitive concepts as the nurse moves data to wisdom in the clinical setting. As a NI specialist, the informatics nurse follows the concepts, scopes and standards of practice to guide and define their profession. This paper will further discuss the principles that guides nurse informatics specialist, the scope and standards of practice, professional performance, functional areas for nurse informatics, competencies, evolution, progress, and the future the specialty.
Nursing informatics and technology are quickly becoming the hot buzz words for nursing in the twenty-first century. While performing research for this specific paper, the observations of how far technology has come from its inception is mind boggling. When looking back to the mid 1990’s every patient had paper charting. Nurses manually charted vital signs, nursing notes, treatments and all orders were manually written in the chart. The patient’s name, insurance information, and billing items were stored electronically. Fast forward twenty plus years and everything nurses do with, for or to a patient is filed electronically. This file today is known as the electronic health record (EHR) (Lavin, Harper, & Barr, 2015). This paper will be delving into the history of nursing informatics and technology, the pros and cons for nurses and what will be the big picture for informatics and technology in nursing today and in the future. Nursing informatics and the technology that has evolved over time are changing and quickly affecting how nurses treat, communicate, plan and document everything that they do for their patients.
Informatics is a specialty in the nursing field that is combined with certain science. As stated by ANA (2008) “nursing informatics (NI) is a specialty that integrates nursing science, computer science, and information science to manage and communicate data, information, knowledge, and wisdom in nursing practice” (p. 65). ANA further focus on the functional areas rather than the role that guides the scope of practice within NI. These functional areas include: “administration, leadership, and management; analysis; compliance and integrity management; consultation; coordination, facilitation, and integration; development; educational and professional development; research and evaluation; policy development and advocacy; and telehealth” (CCN, 2015, para. 1). With these functional areas in mind, a nursing informatics specialist can perform proficiently, depending on the specific task. Hence, the purpose of this paper is to summarize the result of an interview with a NI specialist and analyze the differences and similarities between the interviewee’s functional areas with that of scholarly ones.
After reading the ANA code of ethics there are various aspects that can relate back to nursing informatics. However, the main sections I would like to discuss would be; provisions two, three, seven, eight and nine.
As healthcare providers, maintaining a patient’s confidentiality, human dignity and privacy is expected at all times. Nurses are faced with maintaining patient confidentiality on a daily basis. The Coded of Ethics for Nurses is the framework of nonnegotiable ethical standards and obligations that all nurses are to uphold. Nurses are to be accountable for their actions and are expected to advocate and strive to protect the rights, health and safety of patients (American Nurses Association, 2011).
There are five principles to ethical nursing. The first principle, nonmaleficence, or do no harm, it is directly tied to a nurse's duty to protect the patient's safety. This principle dictates that we do not cause injury to our patients. A way that harm can occur to patients is through communication failures. These failures can be intentional or as a result of electronic or human error. Failing to convey accurate information, giving wrong messages, and breaking down of equipment, can cause harm to patients. Some of these communication problems may certainly occur whether a patient is at a neighborhood clinic or 500 miles away, but distance and high reliance on electronic technology make close examination of communication and ethical
`Ethics' is defined as ."..the basis on which people...decide that certain actions are right or wrong and whether one ought to do something or has a right to something"(Rumbold, 1986). In relating `ethics' to nursing care, "Nursing decisions affect people... nurses have the power to good or harm to their patients" (Bandman et al, 2002). Ethical issues in nursing will always be an ongoing learning process. Nurses are taught in nursing school what should be done and how. Scenarios are given on tests with one right answer. However, there are situations that nurses may encounter that may have multiple answers and it is hard to choose one. “Ethical directives are not always clearly evident and people sometimes disagree about what is right and wrong” (Butts & Rich, 2016). When an ethical decision is made by a nurse, there must be a logical justification and not just emotions.
Engulfed by a cloud of grey, there are many situations that are not unblemished in the profession of nursing. Ethical dilemmas are often the most challenging to handle as they are interlinked with our own personal values and morals, requiring knowledge and attention to many factors (Burkhardt, Nathaniel, & Walton, 2014). Ethical predicaments will cross our paths daily and we need to utilize ethical frameworks, codes and our personal decision making to come to a well-informed conclusion. When we are in the vicinity of an uncertain quandary our human nature is tested, and we thoroughly scrutinise who we are ethically and morally. In the article “Nursing Students’ Use of Electronic and Social Media: Law, Ethics, and E- Professionalism,” Westrick (2016) described the importance of professionalism in nursing with the use of electronics and social media. She pointed out the consequences of improper use of the social media, including program dismissal for nursing students. In addition, Westrick brought up the American Nurses Association (ANA) code of ethics, explaining the nurse’s duty to maintain patient’s privacy, and the National Council of State Boards of Nursing (NCSBN) white paper, guiding nurses with the use of social media. Soon there will be no need to fill out tedious paperwork, to try to remember medications or past medical history. I know it exists in hospitals across America, I have seen it in action many times. Nurses often encountered various ethical dilemmas in the practice setting. Both virtue ethics and caring ethics support good ethical decision making for nurses (Park, 2012, p. 149) but these are inadequate to assist in solving an ethical dilemma (Park, 2012, p. 149). For that reason an ethical decision making tool is helpful for the nurses or clinicians to come up with an ethical decision (Kelly, 2012, p. 571) that allows them to gather information, identify any gap of understanding on the issue or the disagreements between the involved parties through a clear communication (Park, 2012, p. 140). Several authors presented an ethical decision making processes (Park, 2012, p. 141), here presented the two processes that can be applied in resolving an ethical dilemma. One example of ethical decision making process is the DECIDE model by Thompson, Melia & Boyd (Allen, Chapman, Francis, & O’Connor, 2008, p. 5) and the Integrated ethical decision-making model which was derived from the combination of the different ethical decision-making models strengths (Park, 2012, p. 140). These two ethical decision-making model steps are identical to each but differ on the detailed instruction on how the steps are to be done or used in actual case. By comparing the two models the integrated ethical decision-making model have a detailed instruction. The
