While shadowing occupational therapy professionals at Montgomery General Hospital, I gained much insight into the profession and the day-to-day duties and responsibilities of a COTA. The entire facility caseload consisted of geriatric patients, and many of the diagnoses were due to cognitive deficits or recent falls due to weak lower extremities. Diagnoses observed were dementia, CVA, a hip fracture, a total knee replacement, osteoarthritis of the shoulder, bilateral upper extremity/lower extremity weakness, and total blindness. Several patients suffered from dementia and some had accompanying diagnoses, such as hip fracture from falling or upper and lower extremity weakness. A bed mobility assessment was completed upon arrival of a new patient. She was asked to turn over on each of her sides as far as she could and lift her hips off the bed while lying down and pushing off with her feet. A graphesthesia assessment was given to a patient with a CVA who showed decreased sensation as evidenced by not differentiating the letter, number, or shape drawn on her hand. This course most prepared me for the actual work setting. There were many treatment interventions that were practiced that I would not have known if it were not for the training in the physical dysfunction course. Transfer safety, assessments, and various treatments were used on a daily basis in the hospital facility, and I felt as prepared as I possibly could be due to the physical dysfunction course. Throughout fieldwork, I felt mostly prepared, but I also learned a lot of new information and new techniques that will be useful out in the field as a licensed COTA. The only suggestion I would make would be to include hands-on experience with more modalities such as, paraffin wax, ultrasound therapy, and electrical nerve stimulation. Before the fieldwork experience, I was rather unfamiliar with the ultrasound therapy and electrical nerve stimulation, and it was very interesting to learn and have hands-on experience with these treatment modalities. Overall, level one fieldwork was an excellent experience. In only a week, I feel that I have learned immense amounts of information that I will apply to a clinical setting. Not only was fieldwork a learning experience, but I learned that I thoroughly enjoy the profession and enjoy working with the geriatric population. As an Occupational Therapy Assistant, I will work alongside an Occupational Therapist and work directly with patients. The difference between an OT and an OTA is that OT’s set up treatment plans and perform medical assessments while OTA’s carry out the treatment plans with the patients. When a person suffers from dementia then they are at an increased risk of falls. People with Alzheimer’s and other types of dementia tend to be at a high risk of falling. They are more than three times more likely to fracture their hip when they fall, which leads to surgery and immobility. The rate of death following a hip fracture for those with Alzheimer's is also increased. Thus, fall prevention for people with dementia is critical. There are many causes for a person with dementia to fall over and jeopardise their health, the first is Physical Weakness, Gait Changes and Poor Balance, Lack of Physical Exercise, Memory Impairment, Poor Judgment and Visual Misperception, Alzheimer’s can affect the visual-spatial abilities, a person can misinterpret what he sees and misjudge steps, uneven terrain, shiny areas on the floor or changes in floor color. Be sure to have his vision checked regularly.
Children with many different disabilities need the intervention of an occupational therapist; therefore, occupational therapists work in the school systems. Elderly people often begin to lose their physical ability to do certain tasks, so there are occupational therapists working in nursing homes or providing in home care. Many athletes suffer sports injuries that cause them to lose their ability to do daily activities, and occupational therapists are available to them in rehabilitation centers and hospitals. These are just a few of the many scenarios where occupational therapists are available; they can also work in orthopedic centers, colleges, mental health settings, and drug and alcohol settings (Hoffman & Harris, 2000, p. 405). Due to the fact that occupational therapists can often specialize to a certain type of patient, it may be helpful to hold a job in college where one could learn how to work with that group of people. When I graduated from high school I originally planned on being an Athletic Trainer. After being accepted into not only Shenandoah University but the pre-admit AT program which is a three plus two program, I was going to graduate with a masters degree in five years. In my sophomore year at Shenandoah I realized that I did not want to be an Athletic Trainer. I began looking into what careers were out there that I could pursue with an Exercise Science degree. After talking my advisor, who was an athletic training for most of her career, she asked if I ever looked into Occupational Therapy. The name of the society changed in 1923, due to hospitals wanting a national directory of qualified occupational therapists. There now was a minimal standard of training that had to be met before an individual was put in the directory (AOTA, 2009). The name was changed to the American Occupational Therapy Association. The American Medical Association worked with the American Occupational Therapy Association starting in 1933 on getting improved education programs for occupational therapists (AOTA, 2009).
This scholarship is very important to me in many ways.This scholarship will help me physically and emotionally if I receive money. When it comes to personal care, and all areas we have covered, I would have to say the most challenging part for me, is peri-care and lifts and transfers. Having the advantage of going to the simulation lab before going out of practicum was a big advantage for me that I am grateful for. Taking the skills of personal care, to lifts, transfers and repositioning, as well of how to be aware of my own body mechanics along with how to care for another individual, from the simlab, to working with people really opened my eyes of how difficult it can be, and how proper procedures must be executed correctly for the safety of everyone.
