On the afternoon of November 14, 2015 at approximately 5:07 P.M. Deputy Singleton responded to a domestic disturbance call at 4547 Old Salkahatchie Hwy in Early Branch SC. When I Deputy Singleton arrived on scene I interviewed Sheree Washington who stated that after Deputy Ayer had left the scene from the previous call that her boyfriend Christopher Williams who resides at the residence said "bitch I should kill" you then proceeded outside, and retreived a light with a piece of pipe attached to it, and then struck her in the chest. At this time Washington called dispatch, and asked for a Deputy to come back out to her residence. CW went to 1261 Everton Dr, Apt 9 Akron, OH 44307 because they were called out by Akron Police Department (APD) for allegations of abuse towards 2 children. CW Jonathon Henry and CW Sonya Cole pulled up to the apartment complex and observed that there were over 15 people sitting outside. Both CW could tell that this was the correct address because there were 3 police cars that were sitting outside. As the CW's were walking towards the apartment complex an APD officer asked if the two workers were from CSB. CW Cold responded that they were. The officer then told CW's that there were not 2 children, however, there was 3. The officer stated that there was one child in the bedroom sleeping that they did not know was in the home. CW's were also told by the officer that Mr. Brian Burrell (AP) was in the home and that he was very agitated. The officer told the CW that they had called him down, however, suspected that he would get upset after entering the home. The officer also said that he already knew that the kids were in the custody of CSB and that he would most likely not be happy that the two CW's were at the home. The officer the led both the CW upstairs to apartment 9.
On Wednesday, March 22, 2017, I was dispatched to 3 Brightleaf Lane on a report of Obscene material call. Upon arrival, I was met by Mrs. Nasheena Quick Davis. Mrs. Davis advised me on May 17, 2016, her cousin Rossia Harris and her girlfriend Denaisha DeGraffenreid stole a video and several photos from her cell phone. They sent the video and photos to Rossia’s phone and she, in turn, sent them to her estranged husband. There was a sexually explicit video sent to her husband. He attempted to show the video to others. She went to the magistrate’s office, however, she didn't have an address for her cousin and her girlfriend so she couldn’t obtain a warrant. On 09/02/2015, at approximately 1620 hours, your affiant and officer Moyer received a complaint at the police department from Wayne V. Kechula. Kechula was greeted at the front door by Officer Moyer. Kechula began screaming about a construction tool owned by Wesley Kacilowicz. Kechula stated the male in the vehicle he arrived with borrowed a tool and has not returned it. Kechula stated these events had occurred in Conyngham, Pennsylvania. Officer Moyer explained to Kechula several times that we have no jurisdiction of that area. Kechula became verbally hostile with Officer Moyer.

Lacey Blvd., Hanford, CA 93230 (559) 852-2345 who reported that on 12/21/16 at approximately 3PM while conducting a placement visit for youth Andrew Yanez DOB: 12/18/01 the youth and the social worker took a walk. During the walk Andrew disclosed that he and a staff member named Shawn Wilcox were involved in an altercation. According to Andrew at approximately 2:45PM he was in the office of the ground home with 2 other staff members named James Boston and Nicole Juarez. I responded to Sunoco on Three Notch Street in reference to a assault call. Upon arrival, I made contact with Stringer. Stringer was being very uncoropertive while being questioned about what was going on. I observed blood running down Stringer's face. Troy Fire and Haynes responded to the scene and checked Stringer out. Once Troy Fire whiped Griffins's face off, I observed a deep laceration about his right eyebrow. On 10/22/2017 I, Deputy Mignone #95 was assigned to Intake/Release at approximately 0130 hours, I was pulling an inmate out of DMH to be released. When I opened the door, I witnesses inmate Victor Collazo J#9739840 yelling and walking over to the left side corner where inmate John Nalls J#11602851 was sitting in the corner. Inmate Nalls jumped up and they got into each other’s face, I then instructed both of them to move away when inmate Nalls struck inmate Collazo in the face with a closed fist to the right side of his face.
On 04/18/2016 I assisted Sgt. Patterson on a traffic stop in the 900 block of East K-53 HWY, Sumner County, Kansas. When I drove past I observed an older four door passenger vehicle stopped on the north side of the road with Sgt. Patterson's patrol vehicle behind it with emergency equipment activated. I turned around of K--53 and pulled in behind Sgt. Patterson, I activated my rear emergency lights and walked to the passenger side window of Sgt. Patterson's patrol vehicle. In the vehicle he had stopped I could see two white males sitting in the front driver and passenger seats, Sgt. I responded to Suntrace MHP Lot 37 in reference to a domestic call. Upon arrival, I made contact with Jessica Ray. Ray stated that this moring around 04:00 her boyfriend (Carlos Stewart) hit her in back of the head. Ray stated that he always hit her but this time sheve had enough and want something done about it. Ray stated that Stewart did hit her on back of the head this morning but officers didn’t see anysigns of a physical altercation on Ray. Ray did have some bruising but she advised that it was from a past altercation.
This agent received a telephone call from the victim, Ms. Andrea Anderson and she alleges that Mr. Brian Champion has falsely accused her for the charges of ASSAULT-SEC DEGREE, RECKLESS ENDANGERMENT, and MACE/CHEM DEVICE-INT/INJURE (Case #3B02312446) on November 19, 2015. She also reports that she was visiting her mother on the date of this incident and had not contact with Mr. Champion. I responded to Magnolia Wood Lodge in reference to a dirorderly call. Upon arrival, I made contact with Nathan Mosley. Mosley stated that one of the residence(Cedrice Barnes) was being very un cooperative and attempting to hit them with closed fist. Nathan Mosley, Angela Lewis and Cessiley Colwell, were all victims of the situation. Officer attempted to walk Barnes back to his room. On 04/02/17, at 12:08pm, I Deputy Warden N. Christian was dispatched to 2453 Lexington Ave on an injury-possible dangerous or vicious dog, owner known. I arrived at the location and spoke to Cherri Mitchell mother of victim Jordan Mitchell. On 04/01/17 at approximately 8:50pm, Jordan Mitchell came home and told him his mother that he was bit by a dog at his friend’s house. Ms. Mitchell stated she went to the house to talk to dog owner but no adult was home at the time of the incident only a minor.
