Person: patients, family, or community who need assistant in health concern
Environment: physical surrounding, psychological influences, food and medication, and cultural background and belief of the person. Environment can affect the person’s health, in both negative and positive ways.
Health: a state of well-being of a person, can be in a normal or impaired state
Nursing: nursing practice required to assist the person to recover, or to maintain the well-being state of health
Over the course of my nursing career, the two concepts of nursing that relevant to my own practice are patient/ family-centered care (PFCC) and collaborative concepts. The concept of PFCC lies on my personal belief is that patients and/or families should be the ones who make decisions regarding their health. PFCC is an approach that patients and their families are considered as parts of the components of healthcare decision making and delivery processes (Hughes, 2011). It is important that patients are provided necessary information when making their health care decisions.

Boykin (2014) explained that a communication involves a two-way process, sending and receiving the message; an effective communication occurs when health care providers and patients exchange information in a meaningful way so that the message is understood and patients are able to actively participate in their care. I always keep in mind that communication is an important part of PFCC. Every shift, I explain the plan of care to my patients using terms that they and their families can easily understand and encourage them to ask questions. I also actively listen to patients about concerns, include these concerns, and adjust their care plans
The definition of health is the state of being free from illness and injury, it can also be used to explain a person’s mental or physical condition.
Under the medical model, health is defined as a complete state of physical, mental, and social well-being, and not just the absence of disease or infirmity.

The Department of Health (2014) defines health not only as a persons physical and mental state but of their well being too. For them they do not just want to know what illness or condition a person has, but how well they are feeling within themselves. In the National Statistics Publication (2011)
Health is a situation where by someone is free from illnesses or diseases and free from any physical pain. It is very essential that everyone have a positive mind, body and sprite which enable us to be active in our daily activities.
The nursing process steps in assessment, diagnosis, planning, implementing, and evaluation is the framework for patient-centered care. As I reflect back through my 44 three years of nursing practice with 34 of them as an OR nurse, I am most passionate about pain management even with all the challenges it imposes, and this is where I feel that my strongest assessment skills are. Nursing is very complex and includes a lot of knowledge with many skills. One skill is particular is how we approach families as we care for their loved ones. Unfortunately, family does not have a true definition because it is so varied around the world (Sharma, 2013). However, there are four approaches to use with family nursing. However, with anything there will be obstacles and caring for family is often times complex. The purpose of this paper is to discuss the four approaches and provide examples.

Nursing is the capacity to give excellent care and a source of insurance to those who are suffering from illnesses. Nursing does not just fixate on the physical properties of a customer's well-being however withal involves minding and reducing the agony related with noetic sicknesses or gregarious misery. “The unique function of the nurse is to assist the individual, sick or well, in the performance of those activities contributing to health or its recovery (or to a peaceful death) that he would perform unaided if he had the necessary strength, will or knowledge.” (Scott and Kirwan, 2014).
My definition of health is the ability to be able to balance the physical and mental aspects of one’s life by being able to adapt to new environments and situations. In other words, health is much more than being free from ailments and sickness, but it is a way of life. Over the years, this description has shape shifted in correlation to my surroundings. I will go into deeper detail later on how I derived to my present definition, but I will start by introducing the one that I grew up to know.
