From supporting a patient whose goal is to learn to walk again, to developing the most felicitous therapeutic intervention for a patient wanting to get back on the road after a motorcycle accident, I love the profession of Physical therapy. As a Physical Therapist Assistant, I fill many roles as I advocate for my patients. A cheerleader, a teacher, a motivator, a consultant, and sometimes just being an empathetic friend. Filling these roles are all valuable qualities which help me to guide my patients as they accomplish their goals. However, I feel that expecting that my patients strive for their goals while failing to address my own goals would be hypocritical and gainsay my personal views of continual self-improvement. My father was a foreigner from Germany, my mother from Japan. I grew up with in Ogden Utah, a town known for having a 30% Hispanic population, large catholic, and large LDS (Mormon) communities. Ogden has a poverty rate almost double Utah’s average. I grew up surrounded by diversity and personally understand the need for empathy and cultural competency. I strive to treat my patients and everyone whom I meet the way that THEY wish to be treated. Diversity provides an opportunity for me to learn from the backgrounds of each of my patients. For example, to best serve my Hispanic patients, I have spent significant time studying to become as fluent as possible in Spanish. While studying, I supported myself with gratuities from teaching martial arts classes and donating plasma. It took me a few semesters to accommodate to the increased rigor of undergraduate studies, however every semester my GPA steadily improved. After earning my bachelor’s degree and completing necessary requirements, I spent all the money sI had to apply to multiple physical therapy programs. A known prerequisite that has been observed in order for collaboration to occur is the recognition of the importance of other professionals for patient care as argued by (Curran, Kabene and Orchards, 2005; Cohen, Henneman and Lee, 1995). Upon further realisation an Occupational Therapy Assistant states that there is no single field that can successfully meet all of a patient’s needs and which then requires that there be collaborative efforts. Another recognition that the Occupational Therapy Assistant expressed was the importance of expertise that the patients bring to the health care process. Ever since I was young my life goals were to help others when I grew up, however I wasn’t exactly sure how. Throughout the past couple years, with a wonderful support system; I have been able to make the decision about wanting to achieve the goal of becoming a physical therapist. It is my belief that I have a high spiritual calling to be a physical therapist, because I have the intention to be able to guide others. I gather that this can be spiritual, because helping others is what God’s intentions are, and would recommend others to follow in His footsteps. My mindset is indeed compassionate, empathetic, selfless, and strong with a thirst for knowledge. Being a physical therapist has many rewards such as happiness, equality, and a sense of accomplishment which can be very valuable and will affect my life in a positive manner.
In this case, although the error started from the transmission of the discharge note to the home health care agency, Physical Therapist and Physical Therapist Assistant made number of mistakes in terms of clinical practice and delegation. First of all, completion and accuracy of patient’s physical therapy record is a professional responsibility of a supervising Physical therapist.1(p4) However, the PT failed to read the hospital discharge referral and documented incomplete and inaccurate plan of care. As a result, important information such as un-cemented prosthesis in situ and weight bearing limitations were not documented. In addition to that PTA elected to follow a protocol for cemented hip arthroplasty without referring back to PT for the missing information in the assessment. According to Texas Board of Physical Therapy examiners rule, “PTA may not specify and/or perform definitive (decisive, conclusive, final) evaluative and assessment procedures.”1(p5) Hence, it was also a mistake of the PTA to conclude the assessment and elect the inappropriate plan of care.
I believe that I have significantly strengthened my application since the last PTCAS cycle. In August of 2014, I gained admittance into Delgado Community College’s physical therapist assistant (PTA ) program. I completed the program in July of 2015. Over the course of eleven months, I excelled academically and maintained a 4.0 GPA. Prior to attending DCC, the only observation hours I had completed were in two different out-patient clinics. During my time in DCC’s PTA program, I was assigned to four different clinical rotations. Three of my rotations granted me the opportunity to observe the practice of physical therapy in a skilled nursing facility and in two different acute care hospital settings. For example, culturally, individuals from China don’t like people invading their personal space so a case manager who is going in the room to discuss a discharge plan with them would need to be aware of that so that they are sure not to make them uncomfortable or put up a wall. I realized that this is important because in this profession, communication is the main source of understanding the patients’ needs and what happens when the patients’ leave the hospital happens all through having an open and honest conversation. While the idea of being a physician assistant or a physical therapist isn’t a bad one at all, I always felt that somehow, something was always missing. I was missing an objective. I was missing what I really wanted to do as a physician assistant. The practice of curing and assisting people without exposing how to prevent future outcomes of illnesses and the spread of diseases to them didn’t really sit well with me at all. I began to doubt my potential future career choice as a physician assistant in general. They say variety is the spice of life; although I knew and loved the knowledge of assisting a medical physician, it was the serious lack of variety within that specific field and the limited options of growth that gave me heavy thoughts day in and day out. The human experience is not a vague suggestion of what everyone strives for. This is a very individualized set of preferences and priorities that each person desires in life. We are all unique beings and as a physical therapist, there is a duty to create a specific strategy of care that is centered on the needs of the consumer. Movement is the basis of everything we do. Optimizing movement by improving mobility and motion, managing pain, and regaining original capabilities will guide society to a healthier and more active lifestyle.
