In order for hospitals and other health care facilities to prevent the thousands of deaths and injuries that occur every year due to medical errors; health care systems were required to implement new record keeping technology. This technology has made patient information and treatment accessible to all who needed to see it. This is especially important when a patient has more than one attending physician and their care relies on each doctor knowing what the other one has done, serving as the prime communication tool between doctors. If organizations do not centralize their technology and essentially their patient databases, the potential for duplicate work or inefficient patient care can exponentially increase. These high tech medical

Bespoke business documents are documents individually or customised to meet a specific requirement or purpose within a specific business. They are personalised to suit each business and can include forms, publicity materials, invoices, business cards, spread sheets, databases, letterheads, presentations, catalogues, reports, proposals etc. These are important because it places a professional image on your company, which will attract new potential customers or will keep your current customers interested. It is important to take into consideration when creating bespoke documents that the purpose is to give a good impression to other businesses or customers, therefore, documents need to be professional and well thought out. An untidy, poorly-designed document can severely affect an organisation’s reputation with its stakeholders. Documents need to be consistent throughout, this means the organisation’s logo needs to be the same on all documents produced, as well as the general theme, for example, you must use the same font and style like: bold/italic/underlined etc. Also, it is preferably that in business documents should use no more than two different fonts, as it may seem unprofessionally with more than two fonts. Fonts need to be readable, as well as in the correct size; headings need to be larger than the main body of the text in order to differentiate, and there should also be plenty of white space in

In 2009, the Health Technology for Electronic and Clinical Health Act (HITECH) of 1996 was expanded. This expansion included mandated guidelines for health care systems in the Unites States to continue implementing of Electronic Health Records (EHR) in health care settings by 2016 and added a provision to improve protection of patient health information through privacy and security Turk (2015) . The implementation of this program has created a debate in the medical community. In addition, many healthcare organizations and institutions have conducted research studies and surveys to evaluate the effects of the EHR on documentation of care and other aspects of the EHR. Challenges surrounding the HER include, the cost of implementing EHR’s, time spent performing documentation, and patient outcomes and safety and security concerns. Let’s further delve into a few of these challenges.
The author of the document is Oliver Dunn and James E. Kelley Jr. The document was originally written by Christopher Columbus and was later interpreted by a priest in his own words though he quoted certain passages from the original diary. He is writing about this topic to describe his first experience to the new world when he arrived. He wrote every detail seen in that island including the people he found. He tells in detail his encounter with the Indians and their behavior. He is also writing about this topic to share his experience with other including the King and Queen. Columbus describes their physical appearance, the animals found there, and setting which helps us understand what he first saw when he arrived in the Caribbean island.
The three methods that will be used for disseminating evidence are podium/oral presentation, grand rounds, and committee meetings. Knowledge is a key concept in today's healthcare society; it is important that there be multiple different kinds of communication strategies used to disseminate evidence (Yuan, Zhao, Liao, & Chi, 2013). The type of method that is chosen for disseminating evidence is also dependent on the audience and the type of evidence that is being disseminated. These three methods seem to fit the organization I currently work at best.
How to meet the challenges of integrating newly acquired physician groups in the health system environment
I learned a lot while researching for my topic electronic clinical documentation. As a nurse who has seen many changes in the medical world we live in , I braced myself for the challenges that came when I was informed our clinic would be going live with Epic in 1 year . It seemed like an eternity of classes but the extensive training and time to play in the learning playground of Epic , left me , the providers and the staff prepared for the challenge . Don’t get me wrong, we had some very frustrating days and some very extended hours but the positive aspects of the electronic medical record (EHR) soon out weighted the negative .The long extended hours documenting what we did in the first 3-4 months
We received a timesheet and Payroll Grid for PPE 10/24/15, after the pay period was processed. However, there was a discrepancy with the hours to be paid and the cost center it was being charged to, missed the pay date 10/30/15.
Over the years people have been building structure, repairing structures, and modifying structures. Every time something has changed with the structure or building, documentation is always needed to record with the changes and benefits of the structures. But why is it important that we record these documents. Does it play a factor if the future for the building or for future contractors, or even how does one save these documents so that we will not misplace them or destroy them over the many years they will be sitting? With this being said documenting all the information on these structures is most definitely one of the most important parts to the structure itself in many ways.
Connecting safety through communication with the help of informatics can lead to providing effective nursing care. Informatics is an expanding topic with technology being integrated more and more into health care with examples like tele health and interactive games. Therefore, the focus then changes to providing safe quality client care by gathering information. Having backup generators, and implementing a firewall system can also aid in the safe guarding of client information. Critical thinking is a skill that is used constantly in the health care field and when it comes to technology the same skill is beneficial. Coming up with new ideas on how to improve by observing what is not working is how I will make a difference for client safety measures.
Overall, the purpose of this paper was demonstrated the development of electronic health records and clinical documentation that is used by healthcare practitioners to provide the quality care. The Clinical Documentation has focused on providing care to patients and sharing patient information between healthcare providers and patient medical record to track their conditions. Essentially, future technologies will reduce the dependence from medical paper based industry to use fully an electronic clinical documentation and significantly contribute to these changes and assists health practitioners in meeting professional and organizational goals. This will provide a more efficient infrastructure, and it would be more beneficial for health care when the technology advances, which is helpful towards all patients, the care teams, and health care organizations.
