R/s Rebecca (mom) and Hunter (11) are living in a building with no running water. R/s the environment is not stable for Hunter and Haley (16). R/s Rebecca and her girlfriend, Lilly drink alcohol excessively. R/s there is possible drug use as well. R/s Rebecca and Lilly fight when they drink alcohol. R/s Rebecca allows Haley to take Hunter with her to NC. R/s Hunter is observed wearing filthy clothes that are tore. R/s Hunter is behind in school because Rebecca transfers him to different schools a lot. R/s there is suspicion of verbal abuse. R'reiion advised that she and Rahgquan were at Washington Park playing basketball when a young boy came and got up in Rahgquan's face and started calling him a "bitch". The reporting party (RP) is group home youth Laura Qi DOB: 4/11/03 DOP: 9/22/16 SCHOOL: Clyde L Fischer Middle, 1720 Hopkins Dr., San Jose CA 95122-1632 SW: Irene Lee (408) 501-6650. The reporting party (RP) stated she has been bullied for the past 4 months by both the group home youth and the staff members. According to the RP she has been called names, physically hit and punch by other group youth, and threatened with physical harm from staff members. These incidents has causes the RP to feel unsafe and place her at risk for harm in the group home. On 1/22/17 the RP disclosed she was hit by another foster youth and reported the incident to a staff member named Dan McGirt, however, Mr. McGirt didn't believe her and stated to her "you are full
Nicholas (father) called the reporter (paternal grandmother) on the morning of 8-10-15, around 12am. He stated the mother and her friend was using ice. The child was in the bed asleep. The reporter went to the home and Candace (mother) flushed the drug down the toilet right before Law Enforcement arrived. Law Enforcement stated it was the same thing as crystal meth. Nothing was done by Law Enforcement because drugs was not in sight. Crystal meth is not being made at the home that the reporter knew of. Autumn S. is a twelve-year old girl who presented at a short-term residential facility after a violent assault on her adoptive mother. This was not her first violent episode. She has a long history of violence towards herself, most notably a history of hitting her head against the wall when she is upset. This behavior is serious and has led to her being treated in the Emergency Room for concussions. She has also assaulted a number of same-age peers and younger children. She and her older brother engaged in a group assault on one of their good friends, hitting him with backpacks full of books. She also attempted to drown her younger cousin after an altercation in a wading pool. It required two adults to pull her off of the younger child. This violence has resulted in various problems in Autumn's life. She has been in trouble in school because of violence towards classmates, and has been targeted by other children because of her violent behavior towards others. She has been in trouble in the home because of her violence. She has even been in trouble with the law, as her most recent violent attack against her adoptive mother was of sufficient severity that a neighbor intervened and called the police.

Leighann states she doesn't know where her private parts are. She pointed to her arm and leg. Caseworker gave an example of where her privates are such where her bathing suit or underwear touches. R/s 14-year-old ran away to be with her boyfriend Hakeem (19). R/s LE was notified regarding the runaway incident. R/s it was reported that Mary (mom) smokes crack and drink all day. R/s reportedly 9 people are living in a 2 bedroom house and they are going to be evicted soon. R/s the 7-year-old son and 6-year-old daughter has to walk home alone from school because Mary is on Hampton St smoking crack.
REPORTER: The reporter/ anonymous called with concerns for the four victims, Colton, and three unknown males. According to the reporter, Kelly is in Burnsville 4 nights a week. The reporter said, a couple of months ago, Kelly and Matthew (boyfriend) was in Corinth, MS stealing a trailer. Matthew was driving the car, and “he slung the baby out of the car seat onto the floorboard”. The reporter said Matthew took off running and called someone to pick him up. Kelly was passed out in the front seat, and she was arrested. The police department called Human Services to pick up the baby, and they called the grandmother to come and get the child. R/s the home has mold and mildew. R/s the ceiling is leaking and foundation is cracked. R/s rodents are in the home. R/s Joseph smokes marijuana every day. R/s sometimes on the weekend Joseph gets drunk. R/s Joseph’s twin brother Moses also is verbally abusive to Jada.
R/s Emilee (10) is an autistic child who needs Special Ed services. R/s on 01/06/2017, Emilie was withdrawn from school. R/s from 10/24/2016 to 01/06/2017, Emilie has 10 unexcused absences. R/s Herman (8) is severe autistic and he has violent rants. R/s Herman has 23 unexcused absences and 24 tardies. R/s reportedly Herman is physical with Valencia (mom) and Emilie. R/s Emily has no family support, but she has declined other services.
Clayton's mom, Patricia's and her boyfriend Sonny is an alcoholic. Patricia and Sonny drink beer and whiskey with Clayton in the home. Patricia provided Clayton with the beer and whiskey. Clayton fights Patricia. Clayton fights Sonny, then called the police on Sonny. When Patricia is drunk, she tell Clayton to gets out of her way or goes played with his friend. The home has no light, gas, food, and water. The lights have been off about a month. The home has never had gas. The water has been off for four months. Tray stays in Wilkinson County and has MDDD. His mom name is Stacey and the reporter stated Marion would have more information for that family. Mom said Ja’Kevion told her about the incident and she told him not to go around Tray. She did not report the allegation. Reporter stated mom does not leave the children home alone with each other. She makes Ja’Kevion sit by himself and she keeps the younger children with her. She is afraid to leave the children around Ja’Kevion. Reporter feels there is something wrong with Ja’Kevion.
