The reporting party (RP) stated she has been bullied for the past 4 months by both the group home youth and the staff members. According to the RP she has been called names, physically hit and punch by other group youth, and threatened with physical harm from staff members. These incidents has causes the RP to feel unsafe and place her at risk for harm in the group home. On 1/22/17 the RP disclosed she was hit by another foster youth and reported the incident to a staff member named Dan McGirt, however, Mr. McGirt didn't believe her and stated to her "you are full
The reporting party (RP) received a call from mental health psychologist Shannon Salonga from Villa Park High School 18042 East Taft Ave., Villa Park CA 92861 (714) 532-8020 Cell (714) 628-4398. Shannon is resident Olawale's psychologist and sees him at his school. Olawale disclosed during the session that he feels unsafe and bullied by other children in the group home. Olawale revealed that on two different occasions two different kids in the group home have been physically aggressive with him. The names of the two children involved and the dates of incidents were not provided. The Olawale reported that the group home staff always took appropriate action. The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
EREPORT # 19021 stated the following: The incident happened last night, 2/8/2016. Destiny's mother has "taken off" as evidenced by empty drawers and closets. Destiny stayed home alone all night by herself. The concern is that Destiny is not supposed to be alone with her dad, per DHS. Dad never came home last night, but the concern is 1) dad comes home and she is alone with him or 2) dad doesn't come home and she is alone again by herself all night. The child in the Gann’s care was in six different homes in the year leading up to placement with them and it was the result of physical abuse; one removal was the result of physical abuse by the caretaker’s adult son. The agency needs to do more thorough background checks on anybody who will be in frequent contact with the child in everyday life to strive for more stability, so that the child does not endure multiple placements in such short amount of time.
CPS received CPS referral #0677-3482-7506-1042519 regarding minor Natalie Thompson (DOB: 10/22/01). On 4/7/17, minor threatened to kill herself by jumping in front of a train because she was upset over what happened in her new placement at the group home. Minor stated that she no longer wanted to live and did not want to return to group home. Minor stated that she expressed to staff that she wanted to be left alone, but staff disregarded her request and held her. Two female staff held her back so she proceeded to strike them. During the restrain, staff grabbed minor by her arms, legs and shirt. One of the staff members (unknown) pulled her hair and held her tight so during the restraint causing the minor to sustain multiple bruises on her body.
The investigations revealed that Mr. Smith failed to investigate any incidents that were reported by Ryan with regards to his being bullied. Mr. Smith told Mrs. Bright that he would personally handle this situation. If was revealed that one of the bullies was Mr. Bright’s distant cousin. Surveillance tapes were pulled from the time frame the bullying happened. Most of the reported bullying incidents were recorded on the cameras. It showed exactly what Ryan had reported to administrators. Ryan, through his parents, filed a lawsuit against Mr. Smith and the HCSD. The RP stated the teacher in the "Lavender" classroom is consistently out of ratio. The RP stated the teacher provides care to 13-14 children without assistance. The RP stated a child named Natalie who is allergic to cinnamon was given "Cinnamon Toast Crunch" cereal and broke out in hives. The RP stated the diaper changing schedule which provides that diapers are changed every two hours is not followed.
MSTT met with Jake at the Valley Youth House to see how he was doing. Jake stated he is doing pretty good there because he has a lot more freedom there then he does at home. When asked what he means by that Jake explained his mother trys to control everything he does and at the Valley Youth house he as some freedom. MSTT and Jake discussed what his plans are once he returns home. Jake stated he does know at this time if he wants to go home. Jake explained Cody from CYS came to speak with him and explained to him he could be placed in a indenpedent living situation. The reporting party (RP) visited foster family on 7/6/15 and found foster father exhibiting mental health behaviors. The RP stated the foster father disclosed he suffers from epilepsy and doesn't take his medication since he doesn't need it. The RP also stated the foster father is prescribe medication due to anxiety and refuses to take the medication. The foster father believes the medication will cause him to "pass out." The RP states the home is a mess with "things all over the place." The RP did not give specific details regarding the "mess." According to the RP the resident became agitated with the lack of care he was received, tossed a pillow at a caregiver on 9/5/16. The facility informed the RP that due to the resident's behavior he required one on one care. The reporting party (RP) stated on 5/31/16 at 6:20PM the RP responded to a call from Rialto Police Department regarding a conflict between resident Dorothy Goforth DOB: 8/12/52 and her roommate (name not provided). The administrator Willicia Fuller was observed yelling at Dorothy and giving her an attitude. According to the RP Dorothy was having difficulty pulling up her pants and required assistance. The administrator refused to assist the resident with pulling up her pants. Upon knocking on the door the RP inquired about his son and was told that he was napping. The RP stated he wanted to come into the home and obtain his son without waking him. Reason for Referral: Jane, Joann, and Jessica were referred to me for group counseling due to their disruptive behavior during lunch and recess. All three girls are from different classes who sit on opposite ends of the cafeteria and become disruptive with inappropriate use of words and actions towards each other. Their behaviors have been addressed by the lunch aide and school safety but no changes have been made.
