He reports the patient’s roommate was subsequently evicted from his home after the landlord inquired about the ambulance visiting. He reports he contacted her father concerning the patient residing within the family home, but the father has said no. In addition, he reports he has attempted to contact her uncle but has been unable to make contact with him. He reports he has attempted to make contact with her said friend who is considering allowing her the opportunity to reside with her but she has not answered her phone and he has been unable to leave a voice message. He reports no one wants her in their home, and the patient has “burned her bridges” with family members. He reports her family would benefit from counseling. CM spoke to Theresa (FSO partner) regarding FSO services for the family. CM informed FSO partner that the family is interested in FSO services and the scheduled 30-Day CFT meeting is on 12/12/16 at 3:30pm in the family’s home. FSO partner reported that she would not be able to attend today’s meeting due to a schedule conflict and I she will try to get someone to cover the meeting.
Social Services Meeting: On 03/01/2017, Ms. Hawkins and her daughter Emoni met with her assigned Case Manager for the family ILP Document Review. Ms. Hawkins’ next ILP Document Review appointment is on 03/14/2017. Ms. Hawkins is in-compliance with the terms of her ILP. Ms. Hawkins was reminded that she is expected to attend all scheduled meetings with assigned Case Manager and failed to do it would considered non-compliance and warning will be issued. Ms. Hawkins stated that she is aware. Case Manager asked Ms. Hawkins if there are any issues or concerns that she will like to discuss, Ms. Hawkins stated no.

On 11/06/2015 Ms. Tameca Chandler came to the SSO to meet with CM Watterson for an ILP meeting. HS De La Torre approach client to ask her if it is possible to stay extra half an hour in order to develop a Housing Plan and discuss barriers, housing opportunities to transition into permanent housing. Client agreed to stay to discuss a housing plan.
On 7/15/2015, CM conducted a search via Security to locate client. Client was found sitting in the patio. Security asked the client to go upstairs to the 3rd floor to see social services. CM met with the client and completed Bi-Weekly ILP Review. In the meeting the client was well groomed and dressed appropriately for the weather. She appears to be calm, cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly IlP. Client replies she is doing well and she is still waiting for DHS Manifest if she was approved or disapproved for the apartment” CM reiterates to the client as per P4P meeting on 7/9/2015, client was informed to be “open minded” and also there is a possibility she may not be accepted for the housing.
Client came into social service after receiving a SUS appointment slip left at the front desk by CM 10/3/17 to meet on 10/5/17 at 4pm. Client stated she only had a few minutes to talk because she had to cook dinner. Client informed CM her PA account was closed and she replied once again. Client also reports she has to visit Connecticut, PA office for a letter stating her PA case is closed in that state. Client informed her CM all documents requested will be submitted within a weeks’ time. Client reports two of her four daughters Selah McKenzie and Olivia Lue have a doctor appointment tomorrow October, 5, 2017. The other two daughters, Trinity and Nyan McKenzie have appointments next week Tuesday on October 10,

SSA received a phone call from Verna Goecke-PRPV inquiring if the Bond family was aware of Katlyn’s discharge from the facility. She stated that she had not heard from Mr. Bond regarding the discharge notice and according to Katlyn she had not discussed the discharge arrangements with her parents. SSA shared that two messages had been left on Karen Kohn’s cell phone. We discussed that in the event the family did not arrive to the meeting location this morning, PRPV was willing to transport Katlyn to the family home. Verna asked SSA to email the family’s home address in the even they needed to transport Katlyn to the home.

This officer has maintained limited contact with Ms. Fee through telephone contacts and face to face contacts. Ms. Fee's communication with this officer has declined as she has missed several scheduled appointments and has not returned telephone calls.
