She said that, on 10/31/16, she went to the Tigard APD office to reapply for her SNAP benefit and she asked to have the SNAP application before her interview appointment scheduled for 11/01/16, because she is a disable person. She stated that she disclosed that she is disable and wanted to have the SNAP application in advance because it takes her time for her to research the appropriate information due to her memory issues. The worker, Maria Shoop, denied her request and did not give her the application, even though she asked “five times.” She said that she could not have the application
Her personal information has been entered into the welfare system’s database, which may be accessed by law enforcement officers without any basis for suspicion […]. All of this has occurred before she has received a single welfare check (645).
SC received a telephone call on 10/16/2015 stared 9:34 and end at 9:41 am from Tricia Crooks at Liberty Resources Home Choices (LRHC) Community Outreach and Enrollment Leader. Entitlement/ Benefits: Case Manager confirmed and verified that Ms. Medina’s PA case is open and active. Ms. Medina is only receiving food stamps. Case Manager encouraged Ms. Medina to visit her PA center for the purpose of obtaining information about her case. Ms. Medina stated that she is still waiting for her Unemployment Benefits to be re-opened. Ms. Medina stated that she is attending to the back to work program.

On September 21, 2012, ACEDS Case Action Update/Display documented by Social Service Representative, Richard Gordon stated that Ms. Williams arrived to the agency to apply for Supplmental Nutrition Assistance Program (SNAP) benefits, and Medicaid for her child only. During the visit, CARE interface revealed that Ms. Williams had an open Medicaid case in Maryland (#47403882500). Maryland Social Services also reported that she was employed by CPR Medical as of August 9, 2012, and paid bi-weekly $850.00. Ms. Williams also reportedly received unearned monthly income from SSI in September 2012 in the amount of $440.34, and in October 2012, in the amount of $370.54. As a result, Ms. Williams was denied expedited Foodstamps pending paystubs and her application was marked as "pending."

GAO called Ms Lisa Everett. She was inquiring if her daughter, CAITLYNN, was removed from her case. She stated that received a letter from APD informing something about her SNAP, but did not understand if it was about her daughter been removed.
I called the constituent one more time and she said that is struggling with CW Roseburg since last 11 months. She said that her CW case got three different workers assigned and no one did their job.
In June, she called to follow-up and was informed still was not processed. Then in June she was informed that the APD office does not have records of her award letter because the worker that was processing her application does no longer works in that office. She provided it.
