DOI: 6/26/2002. The patient is a 64 -year-old female patrol officer who sustained a work-related injury to her back, shoulder and legs due to constant driving at work. As per OMNI, the patient is status post lumbar surgery on 1/13/11 and epidural steroid injection on 9/24/12. MRI of lumbar spine with and without contrast dated 6/6/12 revealed congenitally shortened pedicales at the L1-L2 level but no impingement on the thecal sac or nerve roots at this level is identified; desiccated L2-L3 disc with bilateral facet joint arthropathy resulting in moderate bilateral neural foraminal stenosis but no impingement on the thecal sac or nerve roots at this level is identified; desiccated L3-L4 disc with bilateral facet joint arthropathy resulting in moderate bilateral neural foraminal stenosis but no impingement on the thecal sac or nerve roots at this level is identified; …show more content…
It was noted that symptoms were improved from her lumbar epidural steroid injection done on 8/11/15. He was able to get up and down from chair more easily, able to get into and out of the bed and car more easily, able to walk, sit, stand for longer times, and stooping/bending improved. He was able to reduce oxycodone use by 50% following procedure. She feels that now the effects of the procedure are worn off. The patient states she is doing fine as long as she can get her medications filled. Musculoskeletal examination revealed laminectomy scar, tenderness at bilateral lumbar facet column, and positive lumbar pain with extension/ rotation. It was also noted that the patient is positive for pelvic tilt, left hip lower and that right sacroiliac joint and greater trochanter are tender. Lower extremities revealed mild edema at bilateral lower extremities, greater in left leg. There is also tightness noted bilaterally during seated straight leg raise. As noted above, this is an appeal to the previously denied request on 02/09/16. The appeal letter states that the patient has low back pain. MRI report showed disc bulge at L5-S1 with left neural foramina narrowing. However, 01/06/16 progress report noted that there was no radiculopathy with a negative SLR on exam. The guidelines require documentation of radiculopathy on exam. In addition, there is no documentation of a trial or failure of conservative treatment, including PT, as recommended by the guidelines. Furthermore, there is no documentation of any focal neurological deficits on the exam that would indicate nerve pathology. The motor and sensory exam was normal. Range of motion shows flexion of 85 degrees, extension of 30 degrees, and lateral tilt of 25 degrees bilaterally. Straight leg raise is positive on the right at 90 degrees for low back pain. Bechterew's test is positive on the right. The patient has diminished sensation in the right L4, L5 and S1 dermatomes. Deep tendon reflexes are absent in the right knee and right ankle.
Per the medical report dated 03/29/2016 by Dr. Waghmarae, the patient believes that her left buttock pain has increased over the last month. She describes her pain as aching, throbbing and stabbing. She rates her pain symptoms as 8/10. Pain is relieved by medication, heat, ice and use of a Transcutaneous Electrical Nerve Stimulator (TENS) unit, and is increased by movement and standing for long periods of time. The patient reported a 25-year history of intermittent low back pain since an initial fall in 1990 with exacerbation of symptoms upon aggravation of the patient’s comorbidities, or with an extreme decrease in activity. The patient’s medical history also included fibromyalgia, lumbar osteoarthritis, irritable bowel syndrome (IBS) and bilateral neuromas between metatarsals 2-3. The patient noticed onset of the bilateral lower extremity radicular symptoms after she slipped and fell on her wet garage floor, landing in a sitting position, 5 weeks prior to her physical therapy initial evaluation. As a result, she was experiencing constant sharp/shooting symptoms coupled with functional limitations including sitting, standing, walking, ascending/descending stairs, transfers, bending, and lying down, making it difficult for her to sustain any one position for any significant period of time. Consequently, the patient found herself significantly limited in her ability to complete her activities of daily living (ADLs). College and Professional Sports have been a major entertainment for people all over the globe. Baseball, football, hockey, basketball, golf, and soccer are a few examples of the greatest sports of this decade. These sports and more have the greatest athletes in the world involved and they get the opportunity to showcase their skills and talents each year. They only get the chance to do these things if they can pass a drug test that involves not taking steroids. Many years ago athletes were not forced to take any type of drug test thus causing many players to take steroids.
