Septoplasty is a surgical procedure in which a deviated nasal septum is corrected. The nasal septum is the structure that divides the nasal cavity into two chambers. The anterior portion of the septum is cartilaginous while the posterior portion of the septum has bony attachments to the ethmoid and vomer bones. Typically, the nasal septum is straight at birth; throughout the aging process the septum may tend to deviate to one side or the other. A deviated septum can also occur due to trauma. A severe deviated septum can cause breathing difficulties do to the obstruction of the nasal septum, and also sinusitis. The only way to fix a deviated septum is through surgery. Facial hair removal is typically unnecessary. Prepping for the procedure begins at the upper lip, extends to the hairlines and beyond the chin. When prepping the patient avoid getting prep in the patient’s eyes and ears. A turban style wrap is used to restrain the patient’s hair during the procedure. Three towels are placed in a triangular formation around the patients face. A bar drape is then placed across the forehead. A U-drape placed encircling the patients face and covering the patient’s body. Cocaine-soaked cottonoids are placed within the nose in preoperatively. The circulator makes a note as to how many cottonoids are placed in the nose. During interoperate a nasal speculum is placed within the nose; using bayonet forceps the cottonoids are removed. Make sure all cottonoids are removed. The nasal speculum is then placed in the affected nostril. A Cottle clamp maybe used to aid in exposure of the nasal cavity. An incision is made into the mucous membrane and also the perichondrium. The incision is made using a No.15 blade on a No.7 knife handle. When incising tissue remember to provide suction as needed. This type of therapy results in tumor antigen release through local trauma to the area and this results in having the potential to evoke an integral anti-melanoma immune response (Maverakis et. al, 2015). In the United States, radiation therapy (RT) is used within about 1 percent to 6 percent of patients that have melanoma (Maverakis et. al, 2015).
