Overview of the community clinical site From august 30 to September 14th I have been going to Kansas City rescue mission located at 1520 Cherry St, Kansas City, Missouri. Kansas City rescue mission is a homeless shelter that provides a lot of services to homeless men. It provides shelter, meals, showers, healthcare, laundry services, bible study/support groups, and recovery services. The men that reside in this facility need a place to stay, healthcare assistance and jobs. Involvement with the Agency The healthcare service at Kansas City rescue mission is setup like a doctor’s offices. The clients would sign up to be seen by the nurse and once they come into the nurse’s office, I would interview them while charting the subjective data and then perform a focused assessment based on what the client came in for. I would then make a recommendation, educate, or give over the counter medication to the client under the supervision of my preceptor. While interacting with the men at the facility, I felt accepted by most of the clients. For example, during visits at the nurse 's office, most of the men would open up and share their stories and things that they are struggling with emotionally even though I hadn’t known them long enough to establish rapport. They would also engage me in conversation and applaud me for joining a nursing school. I really enjoyed getting to know some of the men in the recovery program and hearing their stories. As a teen, I had the opportunity to volunteer on a summer mission trip that focused on serving the homeless population in San Francisco. Stricken by poverty and drug addictions the Tenderloin District was unlike anything I had ever seen. Instead of passing judgment or feeling intimidated, I chose to serve these people with compassion and respect. What I experienced during this time was transformational and helped to direct me on a pathway towards service.
Name examples of each patient care setting in your community. What types of services might patients receive in each setting? In my Community in TX, We have Dermatology clinic, Cardiology, Urology, Surgery Center, Dental clinic, Pediatric Health Clinic, Urgent Care, Abortion Alternatives, Rehab. The Hunt Regional Medical Center includes an extensive list of outpatient and diagnostic services from surgery to cancer care to open imaging.
Kimbrough gives in my opinion a great opportunity for many health care professionals. The staff they need to have to perform all of these services could be tremendous. You would need to have physicians both primary and specialty along with nurses or trained technicians. IT is as important as health care professionals as the facility runs through its own insurance and keeps records through their system that are up to date and make it efficient for patients being seen in this facility. As hospitals run I would assume you could have the same type of governance with a facility such as this with a CEO, Board of Trustees, and medical directors. A chief of service would be needed to make sure the facility stays within regulation sanitary, safe, and following codes. Now being a military facility I am unsure of how they hire. A question I have about the facility is; does it have to be staffed by military, volunteer, or is it associated with an educational facility to teach. The website is helpful for a patient seeking care, but not for someone looking for a career.

The facility also has a dedicated consumer advisory group made up of representatives of the local community whose main task is to provide patients and carers perspective of hospital issues where feedback is necessary towards addressing the issues faced. The advisory group accomplishes this task through a variety of manner which includes, assisting and conducting client surveys, evaluating publications, participation in walk-around audits as well as service planning assistance.
This agency utilizes a sliding scale for client’s who are paying cash for treatment. Otherwise, they accept a variety of insurances, including Medicaid and their managed care options, as well as private insurance options. This is their main funding source. Julia reports that clients are referred through multiple sources, including other agencies, hospitals upon discharge, word of mouth, internet searches, and other avenues. For the most part, individuals contact the agency for assistance to begin the referral process, which includes completing consents and confidentiality paperwork, an assessment, and treatment planning. The referral process can be initiated through an online form submission process.
Overall, it was interesting researching the services and resources in each organization because all these organization in the community each provided different resources and services so there wasn't many duplications.
I don’t know if I would have ever decided to volunteer at a shelter in St Paul if it wasn’t for this assignment; I am very thankful for this
It goes without saying, regarding any homeless population, that any medical services brought to them will yield a positive health outcome. It is predicted that an MMU in Burbank for the homeless, will treat small medical events, diagnose new cases and treat existing conditions of chronic, communicable, ad acute diseases, along with making referrals to specialists and housing authorities. These types of services have an overall impact, in that they decrease emergency room and hospitalization rates, as well as the financial burden on the community. The partnerships that are forming to facilitate these services increase the longevity and sustainability of the community, while homeless individuals become more health literate, along with developing trust in their MMU staff and community.
I will be assisted by three employees: a registered nurse, a medical assistant, and a receptionist. Ongoing training and support for these employees will be provided by Phoebe-Sumter Medical Center.
In 1983, Urban Ministries Center in Durham, North Carolina was a host site for local area services for the homeless community of Durham. However, in 2001 three organizations, Durham Community Shelter for Hope, St. Phillip’s Community Café, and United Methodist Mission Society merged into one organization, Urban Ministries of Durham (UMD). UMD was established to address issues and to provide resources to the homeless community of Durham, North Carolina (“Mission and History,” 2009, para. 5). The organization changed their focus in 2014 to help people and families seeking temporary shelter, while addressing their barriers with hopes of obtaining permanent and sustainable housing. I interviewed Ted Wachsmuth, Portland Rescue Mission, to find out what programs they at their facilities. Portland Rescue Mission (PRM) offers a 3-4 month up to 6-month program for anyone that is homeless, but they have to apply to the program. The PRM will do a screening to make sure they are stable, mentally and physically. Once the person is accepted into the program, the PRM will provide them with a bed, food and they must work 26 hours a week. Some times they will even extend it another 4-6 weeks, if they are trying to help the person with housing or a job. Over 100 homeless men and women decided to come in off the streets and many of our neediest neighbors were given HOPE. Your gift helps make this possible. In addition to the residents we serve, each day an additional 500 hungry men, women, and children line up for a meal in our emergency food service line. Our residential programs are at capacity with over 700 formerly homeless enrolled. We have two additional outreach events coming up this summer where we hope to transform even more lives.
