Nurses are expected to place the care and treatment of each patient as their highest concern, but they may disagree on the exact method to use. Theory-guided practices involve patient care practices that are often too new to have developed research supporting their effectiveness, while evidence-based practices rely on tested and tried techniques to enhance patient care. Nursing practice theories are the narrowest in scope, detailing specific responses to situations that nurses may face, such as detailing a number of specific tasks a nurse can perform to reduce a patient 's stress while under the nurse 's care. Evidence-based nursing practices use tests techniques in patient care, such as specific rules dictating a nurse 's reaction to specific situations, exacting guidelines and tested techniques that are supported by research. Evidence-based practices work to provide reliable and efficient care techniques to each patient. “Evidence based medicine is the conscientious, explicit and judicious use of current best evidence in making decisions about the care of individual patients. Scott and Mcsherry (2008) supported the French’s assertion, proposing the key elements of EBP are that it is a theory-driven process, which involves the use, evaluation and application of research; identification of best evidence; evaluation of care; problem solving; decision-making; clinical expertise; and requires patient involvement. Evidence-based practice is made of evidence, clinical expertise, patient preference, the context of care (Barker, 2013). In brief, evidence-based practice is the parameter in the nursing practice that it requires that the nurses gather and use clinical evidence to make decision for the patients so that in the nursing process they can deliver the quality of care for the patients (Ellis, 2013). According to the Academy of Medical-Surgical Nurses, evidence based practice is defined as, “the conscientious use of current best evidence in making decisions about patient care.” (AMSN) The use of evidence based practice has drastically improved patient outcomes, increased quality and safety of healthcare, and reduced costs for facilities. (Melnyk, 2016) In this paper I will provide the history of evidence based practice, how it has already been incorporated and impacted healthcare, and why it is important to nursing and healthcare as a whole.
Evidence-based practice is the practice of making clinical decisions based off the best available research evidence coupled with the nurse’s own expertise, while also taking into account, the patient’s assessments and own personal preferences. This use of research has proven effective at providing better outcomes and lower healthcare costs, yet there are several barriers, such as time, education, and support, which prevent nurses from consistently using evidence-based practice (AJN, 2012). The top three barriers to the use of evidence-based practice are lack of time, education, and support in implementing new practices and using them consistently.

The purpose of this paper is to conduct an in depth exploration of the nursing care considerations of patients in a specific clinical area. Through the synthesis of prior knowledge, clinical experiences and skills, evidence based best practices, and care of patients a comprehensive care and teaching plan will be composed. Integration of critical thinking and clinical reasoning skills, combined with evidence-based research will provide confirmation of nursing process comprehension. The inclusion of reviewed literature will further support knowledge and understanding.
Itroduction: Evidence-based practice is an approach to medicine that uses scientific evidence to determine the best practice (Beyea & Slattery, 2006). As nurses perform their daily tasks they must continually ask themselves, “What is the evidence for this intervention?”. Nurses are well positioned to question current nursing practices and use evidence to make care more effective. In order to improve patients’ outcomes it is the responsibility of the nurse to transition evidence-based practice into the norm, through application of daily practice (Flynn Makic, Rauen, Watson & Will Poteet, 2014). Continual evaluation of current practice must be performed to ensure the use of evidence-based practice opposed to practice based upon tradition. The implementation of evidence-based practice standardizes healthcare practices and diminishes groundless variations within care. These variations lead to the production of uncertain health outcomes (Stevens, 2013).
Nursing was, for my sixteen year old self, taking care of the sick. Little did I know the complexities of that definition. Still, taking care of the sick was interesting enough to make nursing my major. I started practicing nursing years ago. However, I still struggle defining and explaining my profession to others. I usually start by differentiating nursing from medicine. Nurses see patients as humans rather than a disease that needs treatment (Zaccagnini & White, 2014, p. 15). However, as I advance my career, I must actively incorporate nursing theory into my practice. Nursing theory gives a foundation to understand patients and their health problems better. The use of nursing theory provides a framework to evaluate nurses’ interventions on a higher standard (Zaccagnini & White, 2014). Kenney described five steps to follow once the decision to include nursing theory has being made. This paper will explore the process of applying the Kenney’s five steps into my practice.

Nursing theory is the backbone of nursing. It is very important in the application of evidence-based practice (EBP). The functions of a theory are to narrow and provide specific information for the analysis of initially confusing behaviors, situations, and events (Fawcett & Madeya 2013). Moreover, nursing theory helps to improve not only the quality of care that we are providing to our patients but, they also help us make better clinical decisions. As a matter of fact, nursing theory gives the nurse a framework for organizing and analyzing information as well as a base for identifying with their patients throughout the nurse-patient relationship (Schick, 2015). I am very much interested in all the theories. The one that I relate to the most is Hildegard Peplau’s theory. It is a middle range theory that focuses on interpersonal relations. This theory teaches us about how to interact with our patients so that they feel more in control of their treatment. Advanced Practice Nurses (APN) not only focus on diagnosis and medical intervention but also considers family, environment and patient responses in their plan of care.
